The use of laser in gastrointestinal diseases started in the mid-70's. The 3 types of laser in use today are CO2, argon and Nd:YAG. We present the first clinical results in Israel of the use of the Nd:YAG laser in anorectal surgery. Between Oct. '91-Dec. '92, 275 patients with anorectal diseases (mostly hemorrhoids, anal fissures and fistulas) and 67 with various perineal lesions were operated on. In a double blind study, out of 31 newly abstinent patients who underwent opioid free detoxification, 15 were started on the opiate antagonist Naltrexone for 2 months, and 16 were put on placebo for the same period. Given Naltrexone's known efficacy in blocking euphorigenic effects of opioid intake, and in decreasing cravings for the drug, the authors have tried to assess eventual drug taking, specifically in the context of suburban housing projects, and evaluate the rate of retention in a rehabilitative program for both the Naltrexone and placebo groups. HLA Class I antigens were determined in the 49 subacute sclerosing panencephalitis (SSPE) patients. In analyzing the Arabs and the Jews separately, there is a decrease in HLA-A2 in the Jewish patients (X2 = 3.96; P less than 0.05), a significant increase in the HLA-A29 in the Arab SSPE patients (X2 = 6.29; P less than 0.02), and a slight, insignificant increase of HLA-A29 in the Jewish patients. The present study supports previous findings of Kurent's and needs following studies in larger groups of Arab SSPE patients.

Following sporadic reports on persistent Schistosoma mansoni (S.m) infections in Israelis of Yemeni origin, we systematically surveyed a group of 129 individuals who immigrated to Israel 38 years ago. Physical examination was uneventful in all members of the study group. A single stool examination revealed S.m eggs in 15 of 129 subjects (12%). Specific anti-S.m. We found that an abnormal hemoglobin with a very low oxygen affinity was responsible for overt cyanosis in an otherwise healthy adolescent. During the period 1968-73, 46 patients with subacute sclerosing panencephalitis (SSPE), a fatal childhood disease related to rubeola, were encountered in Israel. The incidence per million population was 3.4 for Sephardic Jews, 3.2 for Arabs and only 0.5 for Ashkenazic Jews. A countrywide search for Creutzfeldt-Jakob disease in Israel disclosed 29 cases with onset between 1963 and 1972. Incidence in various ethnic groups varied in the narrow range of 0.4 to 1.9 per million population except among Jewish immigrants from Libya, among whom the incidence was 31.3 per million. An extraordinary excess of Creutzfeldt-Jakob disease exists in this ethnic group.

We report 5 patients in Israel with cat-scratch disease (CSD) who had unilateral sudden sensorineural hearing loss. Several mechanisms are plausible, but further research is needed to clarify pathogenesis. The cases highlight a previously underrecognized association between CSD and sudden sensorineural hearing loss, expanding the spectrum of cranial nerve neuropathies in CSD.
