Reversible surgical model of biliary inflammation and obstructive jaundice in mice.

Common bile duct (CBD) ligation is used in animal models to induce biliary inflammation, fibrosis, and cholestatic liver injury, but results in a high early postoperative mortality rate, probably from traumatic pancreatitis. We modified the CBD ligation model in mice by placing a small metal clip across the lower end of the CBD. To reverse biliary obstruction, a suture was incorporated within the clip during its placement. The suture and clip were removed on postoperative d 5 or 10 for biliary decompression. After 5 d of biliary obstruction, the gallbladder showed an 8-fold increase in wall th

CT findings of cystic duct carcinoma two years before obstructive jaundice.

We describe a case of cystic duct carcinoma, the computed tomography findings of which had been incidentally obtained several times over the preceding three years before obstructive jaundice appeared. Under the diagnosis of extrahepatic bile duct carcinoma, a 75-year-old man underwent surgery. Two years before obstructive jaundice appeared, the arterio-portal phase of enhanced computed tomography demonstrated an ovoid mass with peripheral rim enhancement, measuring 1.5 cm in diameter, beside the extrahepatic bile duct. One year later, a repeat enhanced computed tomography consistently showed t

Perioperative renal protection in patients with obstructive jaundice using drug combinations.

BACKGROUND/AIMS: The effectiveness of dopamine alone or in combination with mannitol or furosemide in preventing postoperative renal dysfunction in patients with obstructive jaundice was assessed in this study. METHODOLOGY: Forty patients having obstructive jaundice were randomly allocated into 4 equal groups. Preoperative hydration was performed by infusing all patients 1L of 5% dextrose the night before surgery and another 1L in the morning before surgery. Intra- and postoperative maintenance of adequate intravascular volume was assured by fluid and blood replacement guided by the monitorin

Recovery of liver functions following surgical biliary decompression in obstructive jaundice.

BACKGROUND/AIMS: Derangement of liver functions in obstructive jaundice has been known to influence surgical outcome. The pattern and time frame of liver function recovery in patients with surgical obstructive jaundice undergoing a bilioenteric anastomosis has not been comprehensively defined in human beings. METHODOLOGY: Fifty patients of obstructive jaundice who underwent a bilioenteric anastomosis had their liver function evaluated done by biochemistry (pre-operatively and postoperatively on day 1,4,7 and 6 weeks) and radionuclide mebrofenin scan (preoperatively and 6 weeks postoperative).

[Obstructive jaundice as a presentation form of bronchogenic small cell carcinoma].

We report a patient having a non-diagnosed small cell lung cancer, presenting with a clinical picture comprising emaciation, hepatomegaly, progressive jaundice, bilirubinuria and clay-colored stools. Computed tomography showed intrahepatic and extrahepatic biliary duct dilatation. The cause was compression of the common bile duct by metastatic enlarged lymph nodes in the porta hepatis. Clinical suspicion arose because of a mass found on the chest X-ray film and a positive cytologic sputum examination. The definitive diagnosis was obtained by bronchoscopy and biopsy.

Diagnosis of atretic prolonged obstructive jaundice; technetium 99m hepatolite excretion study.

Since 1982 we have developed a diagnostic method for differentiation between atretic prolonged obstructive jaundice (biliary atresia) and nonatretic prolonged obstructive jaundice in a neonate. The first report was presented at the Fukuoka Meeting in 1983. Analysis of duodenal juice (excretion test of intravenous injected hepatolite into the duodenum) has proved to be extremely reliable to differentiate biliary atresia from nonatretic prolonged obstructive jaundice. The major diagnostic features of the study are (1) biliary tract to bowel transit of radiotracer, and (2) observation on color of

Obstructive jaundice secondary to intra-biliary rupture of hepatic hydatid cyst.

Of 58 cases of obstructive jaundice treated in a three year period, six were due to the intrabiliary rupture of hydatid cysts of the liver. Eosinophilia of more than 10%, raised alkaline phosphatase, positive indirect haemagglutination test were noted in all six cases. Plain X-ray of the abdomen, ultrasound, endoscopic retrograde cholangio-pancreatography and CT scan were useful modalities for definite pre-operative diagnosis. All patients had operative treatment which consisted of cyst drainage, partial pericystectomy, curettage of the remaining cavity which was stitched with tube drain, chol

Percutaneous transhepatic drainage and insertion of an endoprosthesis for obstructive jaundice.

As a consequence of the good results with percutaneous transhepatic cholangiography using the Chiba needle, a similar technique was employed for percutaneous transhepatic drainage and insertion of a percutaneous transhepatic endoprostheses. Herein we have described the technique and results of percutaneous transhepatic cholangiography with the Chiba needle in 45 patients and of combined percutaneous transhepatic drainage and insertion of an endoprosthesis in a consecutive group of 69 patients with obstructive jaundice. In the latter group, 50 patients had a malignant lesion. Of these, 38 were

Hepatobiliary imaging using Tc-pyridoxylideneglutamate in the diagnosis of obstructive jaundice.

In 77 of 114 consecutive patients with suspected hepatobiliary disease undergoing cholescintigraphy a firm clinical or operative diagnosis was possible. These patients were classified as normal, or as having extra-hepatic biliary obstruction (partial or complete) or hepatocellular disease. On a double-blind basis cholescintigraphy correctly interpreted 18 of 20 (90%) normal controls, 12 of 14 (86%) of those with partial obstruction, 16 of 16 (100%) of those with complete obstruction and 23 of 27 (85%) of those with hepatocellular disease giving an overall diagnostic accuracy of 69 of 77 (90%).

Duodenal intubation in the differential diagnosis of obstructive jaundice in infants.

The aim of this work was to analyse the value of duodenal intubation in the differential diagnosis of obstructive jaundice in infants. Thirty-six patients were studied, ages varying between one and five months (average 2.4), 21 males and 15 females, all presenting cholestasis characterized by jaundice, choluria and fecal acholia. Of the 36 patients, 15 with negative intubations had during surgery a confirmation of a diagnosis of biliary atresia. The other 21 infants with positive duodenal intubation received at that moment the diagnosis of "neonatal hepatitis syndrome". Because it is efficient

Progressive neurological disorder associated with obstructive jaundice and vitamin E deficiency.

A ten-year-old girl had obstructive jaundice in the newborn period which persisted for 4 years despite choledochojejunostomy at 6 weeks. From the age of 6 years she developed a progressive neurological syndrome characterized mainly by dysarthria and ataxia. A causal relationship with her profound vitamin E deficiency seemed likely. Treatment with vitamin E over a 2 1/2 year period appeared to arrest the progression of the neurological deficit and subsequent increase in dosage produced some improvement in her ataxia.

Obstructive jaundice due to congenital stenosis of the ampulla of Vater.

Two infants who had obstructive jaundice due to congenital stenosis of the ampulla of Vater improved dramatically with sphincteroplasty. In one, the jaundice was complicated by hepatocellular dysfunction and mild biliary cirrhosis. Only a few cases of such an anomaly have been reported, mostly with different modes of clinical presentation.

[Surgery in patients with obstructive jaundice caused by carcinoma (author's transl)].

Malignant disease obstructing the bile ducts is diagnosed in most cases at a time, when radical surgery is not possibly any more. Diagnostic procedures applied up to now are not suited for early diagnosis. 5 years survival rates of patients with carcinoma of the bile ducts are maximally 6%, in patients with carcinoma of the papilla 36%. In the Department of Surgery of the Münster University, 126 such patients have been treated in the recent time; in 104 cases jaundice was apparent clinically. Radical pancreatoduodenectomy could be done only in 24 patients.

[Hypermethioninemia in the differential diagnosis of infantile obstructive jaundice (author's transl)].

In 7 infants suffering from obstructive jaundice we found transient high levels of methionine in serum. All cases had only intrahepatic cholestasis, especially with intrahepatic biliary hypoplasia, whereas other patients with extrahepatic biliary atresia and/or combination of extra- and intrahepatic obstructive jaundice always showed normal levels of methionine. Therefore hypermethioninemia seems to be helpful in differentiating the various causes of infantile obstructive jaundice.
