Sonographic detection, evaluation and aspiration of infected acromioclavicular joints.

OBJECTIVE: To describe the sonographic findings of septic arthritis of the acromioclavicular joint. DESIGN AND PATIENTS: A retrospective study of five male patients was carried out. Four of the patients were referred because of signs and symptoms suggestive of glenohumeral joint septic arthritis, one for signs and symptoms suggestive of septic arthritis of the acromioclavicular joint. All the acromioclavicular joints were evaluated with ultrasound, aspirated and the aspirate cultured. RESULTS: All patients had normal ultrasound findings of their glenohumeral joints and distended acromioclavi…

Successful manual reduction of locked metacarpophalangeal joints in fingers.

BACKGROUND: Many studies on the etiology and operative treatment of locked metacarpophalangeal joints in fingers have been reported, but there have been few investigations on manual reduction. The rate of success of manual reduction in previous reports has been low, and no consensus has been reached with regard to the best method of manual reduction. On the basis of our experience with operative treatment, we devised a safe method of manual reduction. METHODS: Between January 1987 and December 1995, we reduced a locked metacarpophalangeal joint in twelve female patients; every locked finger w…

Calcification of lumbar ligamentum flavum and facet joints capsule.

STUDY DESIGN: The incidence of calcification in ligamentum flavum and posterior capsule demonstrated by computed tomography was reviewed in a series of 147 patients in whom 419 lumbar levels were studied. OBJECTIVES: To find a relationship between mechanical lumbar pathology and the calcification of the ligamentum flavum and posterior capsule. SUMMARY OF BACKGROUND DATA: Calcification of the ligamentum flavum and posterior capsule has been found in diffuse idiopathic skeletal hyperostosis and in metabolic disorders. Although this fact is considered a manifestation of degenerative disease of

Primary fusion of fracture dislocations of central carpometacarpal joints.

Six patients with dislocations or fracture dislocations of at least the second and third carpometacarpal joints are presented. Closed reduction was attempted in all cases. Two dislocations with minimal periarticular fractures were stable and were treated with cast immobilization. Four fracture dislocations were unstable and were treated with primary arthrodesis of the injured joints. One patient was lost to followup and the other 5 were observed for an average of 24 months. Establishment of a stable or rigid central carpometacarpal joint produced an excellent result in 4 patients and a satisfa…

Spatial and morphologic aspects of temporomandibular joints with sounds.

Various spatial and morphological aspects of the temporomandibular joint may be involved in the development of TMJ sounds. The present study examines their contribution to the prediction of sounds. A non-patient sample was studied using sensitive sound measurement and panoramic radiography. It was found that temporomandibular joint sounds are common in asymptomatic subjects showing substantial variation in several spatial and morphological parameters. An autonomous contribution of spatial relationships, asymmetries or morphology of bony structures to the prediction of joint sounds could not be

[Diagnostic imaging in arthrosis deformans of the peripheral joints].

Conventional radiology is the most important diagnostic imaging method for the arthrosis deformans of the little peripheral joints, sometimes radionuclide bone imaging is used as a supplement. We deal with the classification and the radiological signs of the arthrosis deformans in general and of the peripheral joints in particular. Great importance is attached to typical findings and finding constellations in text and pictures. We try to explain the complex differential diagnosis and the implication with other disorders of the joints in a brief and poignant way.

[Magnetic resonance of the cartilages of the large joints].

MRI of the articular cartilage requires a careful technical approach since this structure is very thin, with a peculiar internal architecture between the supporting matrix and the cell component. On MR images the normal articular cartilage has a zonal appearance. To optimize the variables for best visualization of the internal architecture of the hyaline articular cartilage, an ex vivo and in vivo study was carried out. Accurate T1 and T2 relaxation times of the articular cartilage were obtained with a particular mixed sequence and then used to create isocontrast intensity graphs. The latter a

[Treatment of bilateral dysplastic instability of the hip joints].

In the article are described the peculiarities of the treatment tactics and presented an integral assessment of treatment efficiency of 563 patients with bilateral dysplastic instability of the hip joints (bilateral dislocation, bilateral primary subluxation, dislocation of one and subluxation of the other joint), carried out in the hospital of the institute named after M. I. Sitenko in 1955-1989. There is noted a necessity of achievement of anatomo-functional symmetry of the hip joints as a strategic aim of treatment and an expediency of application of "anticipation estimate" principle while …

Simultaneous dislocation of both interphalangeal joints in a finger.

Unlike dislocations of the distal or proximal interphalangeal joints, the simultaneous dislocation in a single finger of both seems to be a rare occurrence. Reduction and checking of the collateral ligament, extensor tendon, and volar plate, followed by 3 weeks of splinting in the intrinsic plus position seems to be the treatment of choice.

Humeral head retroversion in patients with unstable humeroscapular joints.

Humeral head retroversion and shoulder rotation in both the frontal and scapular plane were studied in 34 patients with anterior glenohumeral instability. Twenty-two patients had traumatic anterior shoulder dislocations and another 12 patients had nontraumatic dislocations with generalized joint laxity. Patients with traumatic recurrent dislocations had a smaller than normal retroversion angle in the unstable shoulder. The angles were 26 degrees on the dominant side and 23 degrees on the nondominant side compared with 33 degrees and 29 degrees, respectively, in normal shoulders. The stable con

[Magnetic resonance tomography of the feet and ankle joints].

21 patients with suspected, or known, abnormalities of the feet or ankles were examined by magnetic resonance tomography. The indications were: osteochondritis dissecans of the talus (three cases), osteomyelitis (three cases), suspected tumour recurrence (three cases), undiagnosed pain (six cases) and a further miscellaneous six cases. Although most of these problems could have been solved with other imaging methods, magnetic resonance tomography did add some valuable diagnostic information. Because of its ability to demonstrate bones, synovia, tendons, vessels and subcutaneous fascia in a mul

Preparation of whole rabbit knee joints for microscopic examination.

A paraffin embedding method to prepare whole rabbit knee joints for histological examination is described. This method provides good quality microscopic sections thin enough for the study of cellular detail and does not require prolonged processing. When examining pathologic changes in experimental arthritis, it is advantageous to be able to examine the intact joint with the structural relations of the joint components preserved. Sections of the whole joint provide numerous areas where bone, cartilage and synovium are contiguous for examination. Having obtained poor results using methods recom…

Group G streptococcal infection of joints and joint prostheses.

Eight cases of septic arthritis caused by beta-haemolytic streptococci of Lancefield group G are presented and compared with others previously reported. Involvement of prosthetic joints is notable (25%) as is accompanying cellulitis (75%) which is probably related to the portal of entry of the organism. Other associated conditions were rheumatoid arthritis (38%) and malignant neoplastic disease (25%). Carriage of the group G streptococcus was detected in two of the eight patients. Serological tests for streptococcal antibodies were found to be less useful in the diagnosis of septic arthritis d…

Total knee arthroplasty in Charcot and Charcot-like joints.

Nine total knee arthroplasties were performed in seven patients with the diagnosis of neuropathic arthropathy. The patients were divided into two groups, classical Charcot and Charcot-like. The histopathological findings in all of the knees, however, were essentially the same. These included hyperplastic synovium with bone and/or cartilage detritus, severe disorganization of the articular cartilage with invasion by a fibrous pannus, and hemosiderin deposits in synovial macrophages. Chronic inflammatory synovitis was noted in all cases. The results of total knee arthroplasty an average of three

Hypermobility of the joints in juvenile episodic arthritis/arthralgia.

It has been suggested that hypermobility of the joints may predispose children to the development of arthritis or arthralgia. To determine the normal frequency of hypermobility, 260 normal schoolchildren (5 to 17 years of age) were examined. In addition, 34 patients with juvenile rheumatoid arthritis (JRA) and 32 children with juvenile episodic arthritis/arthralgia (JEA) were tested. Any child who met at least three of the following criteria was considered to have joint hypermobility: (1) passive apposition of the thumbs to the flexor aspect of the forearms; (2) passive hyperextension of the f…

Cruciate ligament rupture in young adult broiler knee joints.

Derangement of the caudal cruciate ligament was identified in 11 broiler fowls aged between 18 and 38 weeks. In some cases, the caudal cruciate ligament alone was ruptured, whereas, in others, the caudal meniscofemoral and meniscotibial ligaments and medial meniscus were also deranged. In some birds, ligaments were only partially ruptured but, in 5 cases, complete rupture had occurred in both knee joints. All birds showed severe lameness which, in 2 cases, could only be ascribed to caudal cruciate derangement. Degenerative changes were present in many of the ligaments which suggested that rupt…
