Severe maternal bradycardia and asystole after combined spinal-epidural labor analgesia in a morbidly obese parturient.

Serious maternal bradycardia and asystole in laboring parturients after combined spinal-epidural labor analgesia are rare. We report such a case in a morbidly obese laboring parturient after receiving combined spinal-epidural labor analgesia. The differential diagnosis, risk factors, potential contributing factors, and the successful management of the complications with our positive patient outcome are discussed. Even with the low dose of neuraxial drugs commonly administered in combined spinal-epidural labor analgesia, this case underscores the importance of vigilance, frequent monitoring, pr

Uterine contractility in women whose fetus is delivered in the occipitoposterior position.

OBJECTIVE: Most fetuses in the occipitoposterior position rotate spontaneously after striking the pelvic floor. The increased prevalence of prolonged labor, operative delivery, and oxytocin augmentation in women with an occipitoposterior fetal position seems consistent with decreased uterine contractility. We sought to test the hypothesis that women with a persistent occipitoposterior fetal position have inadequate intrauterine pressure. STUDY DESIGN: Intrauterine pressure was measured prospectively electronically in 94 women whose labor pain was controlled by patient-requested epidural analg

Hemodynamic patterns in preterm and term labor: working hypothesis.

This report describes hemodynamic patterns recorded from patients who had spontaneous abortion, preterm, and term labor. A working hypothesis theorizes that specific patterns observed in these situations can be identified and correlated with those present in the preovulatory and postovulatory phases of the menstrual cycle and thereby provide a clue to endocrine imbalances that may occur during pregnancy and play a role in the pathophysiology of these complications.

Effects of an Interdisciplinary Practice Bundle for Second-Stage Labor on Clinical Outcomes.

BACKGROUND: There is renewed interest in second-stage labor practices as recent evidence has challenged historical perspectives on safe duration of second-stage labor. Traditional practices and routine interventions during second-stage have uncertain benefit for low-risk women and may result in cesarean birth. PURPOSE: The purpose of this quality improvement project was to implement an interdisciplinary second-stage practice bundle to promote safe outcomes including method of birth and women's birth experience. METHODS: Standardized second-stage labor evidence-based practice recommendations

Effectiveness of Dinoprostone and Cook's Balloon for Labor Induction in Primipara Women at Term.

Labor induction is commonly used for achieving successful vaginal delivery. This study aimed to compare the effectiveness of dinoprostone and Cook's balloon as labor-inducing agents in primipara women at term. A retrospective cohort study among primipara women was conducted in Hubei Maternity and Child Health Hospital. Basic clinical characteristics were collected. The main outcomes were vaginal delivery rate, cesarean section rate and forceps delivery rate. Obstetric and perinatal outcomes were also compared. Univariate and multivariate analyses were further performed to evaluate the predicto

Obstetric violence and associated factors among women who gave birth at public hospitals in Addis Ababa city administration, Ethiopia.

BACKGROUND: Obstetric violence during labor and delivery is one of the main reasons that women do not seek care from health caregivers in health facilities. Developing respectful maternity care services for women is the most important approach to ensure better newborn and maternal outcomes. OBJECTIVE: This study aimed to assess the magnitude of obstetric violence and associated factors among women who gave birth at public hospitals in Addis Adaba city administration, Ethiopia. METHODS: An institution-based cross-sectional study was carried out among 409 mothers who had given birth at two pub

Postpartum urinary retention after vaginal delivery.

OBJECTIVE: To determine the incidence of covert and overt postpartum urinary retention (PUR) after vaginal delivery and to determine obstetric variables contributing to PUR. METHODS: In a cross-sectional study, women who delivered vaginally underwent a transabdominal ultrasound scan for estimation of postvoid residual bladder volume and diagnosis of PUR. Patient data, including age, obstetric history, mode of delivery, and duration of labor, were compared between women with and those without PUR. RESULTS: Of the 771 participants recruited, 84 (10. 9%) had PUR: 82 (10.6%) with covert PUR and

Factors Affecting Maternal and Fetal Outcomes of Non-Obstetric Surgery and Anesthesia during Pregnancy: a Retrospective Review of Data at a Single Tertiary University Hospital.

BACKGROUND: Anesthesia during pregnancy for non-obstetric surgery is generally known to have a negative impact on maternal and fetal outcomes. We assessed the risk of adverse outcomes in fetuses and mothers associated with non-obstetric surgery. METHODS: This retrospective study analyzed clinical data on pregnant women who received non-obstetric surgeries at a tertiary university hospital. We reviewed maternity admissions using hospital administrative data during the last 16 years. The outcome assessment included the presence of preterm labor, premature birth, abortion, or stillbirth and the

Pre-cervical ripening and hygroscopic cervical dilators in pre-labor induction.

INTRODUCTION: Induction of labor (IOL) is becoming a universal topic in Obstetrics, when the risk of continuing a pregnancy outweighs the benefits. Preinduction is a more recent tool to prepare the cervix when the BISHOP-score is low. About one-third of IOL cases require cervical ripening, which is the physical softening, thinning, and dilation of the cervix in preparation for labor and birth. We report a single center experience regarding the use of hygroscopic dilators in the pre-labor phase to obtain cervical ripening before labor induction. MATERIALS & METHODS: We conducted a retrospectiv

Discrimination, racism, and bias in childbirth pain management in the United States: a scoping review and directions for research and clinical care.

Racial and ethnic disparities in obstetric analgesia care persist in the United States. There is a paucity of validated tools to measure how discrimination, racism, and bias affect obstetric anesthesia care. As a result, little is known about how racism and other forms of discrimination impact pain management during childbirth. In this scoping review, we evaluated 11 studies that quantitatively measured or qualitatively described experiences of discrimination, racism, or bias in childbirth pain management among birthing people in the United States. Studies focused on birthing people with margi

Obstetric cholestasis. A 14 year review.

A 14 year review of obstetric cholestasis in the Women's Centre, Winnipeg, revealed 42 cases (incidence one in 1,293 deliveries). The perinatal outcome in these 42 patients was compared to that in a group of control subjects and showed a significantly higher incidence of premature labor, low-birth-weight babies, and postpartum hemorrhage. There was no particular association of intrapartum fetal distress or intrauterine growth retardation with the cases of obstetric cholestasis.

Vaginal birth after cesarean.

Vaginal birth or trial of labor after previous cesarean delivery represents one of the most significant changes in obstetric practice. There are numerous reasons that influence the decision to proceed with either a trial of labor after previous cesarean delivery or elective repeat cesarean delivery. For the majority of women with a previous cesarean delivery, a trial of labor should be encouraged. There are few absolute contraindications. Women with a previous classical uterine incision should not undergo a trial of labor and should be delivered once fetal lung maturity is documented. An attem

Maternal and Delivery Characteristics and Self-Reported Perceived Control During Labor.

OBJECTIVE: To evaluate the association between maternal and delivery characteristics and self-reported perceived control during childbirth. METHODS: A secondary analysis of a multicenter randomized trial was conducted to compare labor induction at 39 weeks of gestation with expectant management in low-risk nulliparous people. Six to 96 hours after delivery, participants who experienced labor completed the Labor Agentry Scale, a validated self-administered questionnaire to ascertain perceived control during childbirth. Scores range from 29 to 203, with higher scores indicating a sense of great

[Correcting occiput posterior position during labor: the role of maternal positions].

Occipito-posterior presentation represents 10 to 34% of cephalic presentations in early labor. Spontaneous rotation during labor to occipito-anterior mode occurs in most cases, but 5 to 8% of fetuses will persist in posterior position for the expulsive phase of delivery. Previous research has shown that this presentation carries an increased risk of unusually long labor, maternal and fetal exhaustion, instrument-assisted delivery, severe perineal injury, and cesarean section. The diagnosis of posterior variety is usually made quite late at the end of dilation. Several researchers have reported

Destructive operations--a vanishing art in modern obstetrics: 25 year experience at a tertiary care center in India.

BACKGROUND: Destructive operations have a limited role in modern day obstetrics. In the developed countries, obstetrics has become so advanced that these instruments have actually been put away. However, in developing countries like India, these procedures have a limited role where obstructed labor still continues to plague thousands of women every year and accounts for 8% of maternal deaths. This study was planned to define the changing role of destructive operations in obstetrics over the years as more number of abdominal deliveries are conducted in modern day obstetrics than these procedure
