Leiomyosarcomas are cancers of smooth muscle cells that can arise from any location but occur most often in the uterus, retroperitoneum, or intraabdominal region. Primary leiomyosarcomas of the lung are extremely rare and are often diagnosed as a mass on routine chest radiography. Metastasis is uncommon and typically occurs late in the disease process, indicating the importance of early detection. Surgical excision is generally curative, offering patients optimal prognoses. Leiomyosarcoma of the larynx is very rare. We herein report an especially rare case of leiomyosarcoma originating in the subglottic mucosa. The majority of cases reported in the literature indicate that laryngeal leiomyosarcomas originate in the supraglottic or glottic area. Our patient underwent simple total laryngectomy and is free of disease 5 years after the operation. The literature is reviewed regarding the choice of treatment and the distribution of the reported sites of origin of laryngeal leiomyosarcoma.

A 46-year-old man was presented with a firm mass of the spermatic cord. He underwent left radical orchiectomy with clinical diagnosis of spermatic cord tumor. PURPOSE: To report a case of spermatic cord leiomyosarcoma metastatic to the orbit.
DESIGN: Case report.
METHODS: A 78-year-old man presented with progressive redness, proptosis, and decreasing vision of the left eye of 3 weeks' duration. Computed tomography revealed an intraconal mass (1.5 x 2 cm) inferior to and displacing the left optic nerve.
RESULTS: A left lateral orbitotomy was performed, and a well-demarcated mass was removed from the surrounding tissue. A follow-up computed tomography of the chest and abdomen was consistent with metastatic disease to the lungs and liver.

A patient passed a large tissue mass at home on postpartum day 1. Pathologic examination demonstrated leiomyosarcoma. A total abdominal hysterectomy was performed, which demonstrated residual disease. All tissue passed in the puerperium should be evaluated to ensure that significant pathologic conditions are not overlooked. Conservative management of leiomyosarcomas is controversial.

Between 1960 and 1995 the University of Leipzig Women's Hospital encountered 44 cases of primary malignant tumors of the Fallopian tube, including four malignant Müller mixed tumors and one sarcoma. A patient with recurrent severe hypoglycaemia attacks due to a large, irresectable retroperitoneal leiomyosarcoma was treated with radiotherapy (60 Gy). The blood glucose level gradually and steadily improved as the cumulative radiation dose was increased. Weaning of hyperalimentation was started when the cumulative dose reached 21.6 Gy. We report the case of a 22-year-old woman who was admitted for inflammatory right-sided sciatica with a decline in general health. Laboratory tests showed severe inflammation. We present a case of uterine leiomyosarcoma in a 60-year-old woman, who had severe hypertension with hypokalaemia and metabolic alkalosis. Investigation revealed an elevated serum renin level; the tumour stained for renin on immunocytochemistry. The serum renin level fell on successful treatment of the primary. We suggest that this is the first reported case of uterine leiomyosarcoma shown to be producing renin.
