Cysteamine is an aminothiol naturally present in cells of the human body as an antioxidant resulting from the degradation of Coenzyme A. Physiologically it is well distributed in mammalian tissues. Highly concentrated in human milk, cysteamine acts as an intrinsic antioxidant and is known for its protective role. Multiple studies now document that cysteamine is a potent skin depigmenting agent. Historically, its rapid oxidation and very offensive odor made it difficult for topical use until recently when stabilization of cysteamine was achieved. A man aged 32 years was referred to a dermatological department for alopecia areata. In addition, the patient had multiple lentigines which, together with other abnormalities, suggested the leopard syndrome. Alopecia areata does not appear to have been described previously in this syndrome.

A 71-year-old woman presented in our out-patients department with pigmentation of the nail bed of her left large toe, which had persisted after a trauma two years earlier. An inconclusive biopsy showed melanoma in situ. The lesion was excised with amputation of the big toe at the IP-joint and closed primarily. The pathological diagnosis was melanoma in situ and lentigo maligna. The lesion had been radically excised.

Between 1960 and 1990, a total of 998 patients were treated at the Sydney Melanoma Unit for cutaneous melanoma of the head and neck. Light therapy has been incorporated into the art of healing and cosmesis for thousands of years and currently has found utility in many areas of medicine. Various modalities of cosmetic phototherapy are detailed, as well as the indications and mechanism of action for each modality. These modalities can be used to treat many common cosmetic conditions, including acne vulgaris, solar lentigo, and melasma. Amelanotic melanoma of the gingiva in a 59-year-old Japanese woman is presented. The histologic diagnosis of melanoma was confirmed by the concurrent lentigo maligna in the palatal mucosa and desmoplastic malignant melanoma (DMM) in the metastatic foci of a cervical lymph node. Electron-microscopical observations revealed that DMM cells exhibited mesenchymal metaplasia or myofibroblastic differentiation. Accordingly, collagen synthesis of the tumor cells was suspected. A review of 31 reported DMM cases is also presented.

Imiquimod activates the immune system when applied to a local area. We report a patient with a history of well-controlled myasthenia gravis who was prescribed imiquimod for lentigo maligna. Treatment was halted after 2 weeks when the patient reported itching and irritating sensations in his throat, consistent with previous myasthenia exacerbations. The symptoms improved once imiquimod use was discontinued. We advise clinicians to be cautious when prescribing imiquimod to a patient with a history of myasthenia gravis.

Junctional nevi of the oral mucosa are rare and may be precancerous. A patient who had an enlarging junctional nevus of the labial mucosa with an adjacent lentigo simplex was studied by light and electron microscopy. On the basis of morphologic similarities--dentritic appearance, lack of desmonsomes, proliferative melanin production, and lack of cytoplasmic fibrils--it appears that the nevus cell most likely develops from melanocytes. The reason for the transformation from melanocyte to nevus cell or junctional nevus cell hyperplasia is unknown.

The diagnosis of actinic keratosis (AK) is seemingly easy, but errors by excess or default are multiple and frequent, even if they generally do not entail short-term harmful consequences. A review of possible errors and simple rules allowing to reduce them is presented here. From a practical point of view, the main risk is to misinterpret an invasive carcinoma as an AK, and to treat it as an AK, and over all a lentigo maligna. However, the misappreciation of the nosology of AK also exposes to frequent erroneous interpretations that are potentially harmful to the patients.

Progressive cardiomyopathic lentiginosis was first described in 1972 by Polani and Moynahan. It is characterised by pigmentation of the skin (multiple symmetrical lentigines), hypertrophic obstructive cardiomyopathy and retarded growth. There may also be mental retardation. All characteristics of the syndrome were present in a 46-year-old woman. In addition there were also other features: lentigo developed at only 35 years of age, there was hypertelorism, thoracic kyphosis, and intermittent severe depression. BACKGROUND: The dermoscopic criteria of extrafacial melanomas are well-known.
OBJECTIVE: To determine the frequency of dermatoscopic findings in head and neck melanomas (HNM) and to assess the distinguishing dermoscopic criteria of facial and extrafacial melanoma.
