Prevalence, patterns and factors associated with over-the-counter antibiotic purchase: a cross-sectional study in private pharmacies in Montserrado County, Liberia.

INTRODUCTION: Antibiotics are prescription-only medicines, but their over-the-counter (OTC) purchase is common in low-income countries such as Liberia. Private pharmacies are an important source for OTC purchases. This study investigates the prevalence, patterns and factors affecting the OTC purchase of antibiotics from private retail pharmacies in Liberia. METHODS: 14 private pharmacies were selected through stratified random sampling in Montserrado County, Liberia, for the cross-sectional study conducted in 2023. Exit interviews were conducted among adult clients who consented and purchased…

Efficacy and safety of artemether-lumefantrine (AL) and artesunate-amodiaquine (ASAQ) for the treatment of uncomplicated Plasmodium falciparum malaria in Liberia, 2022-2023.

BACKGROUND: Artemether-lumefantrine (AL) is currently the first-line treatment for uncomplicated Plasmodium falciparum malaria in Liberia. To ensure antimalarial treatments are effective, the World Health Organization (WHO) recommends routinely testing the efficacy of oral artemisinin-based combination therapy (ACT) for uncomplicated malaria using therapeutic efficacy studies (TES) every two years. The most recent TES in Liberia conducted from December 2017 to May 2018 reported a PCR-corrected adequate clinical and parasitological response (ACPR) of 90.2% in Bensonville and 92.7% in Saclepea f…

Monitoring intrapartum fetal heart rates by mothers in labour in two public hospitals: an initiative to improve maternal and neonatal healthcare in Liberia.

BACKGROUND: In low-resource settings with few health workers, Fetal Heart Rate (FHR) monitoring in labour can be inconsistent and unreliable. An initiative to improve fetal monitoring was implemented in two public hospitals in rural Liberia; the country with the second lowest number of midwives and nurses in the world (1.007 per 10,000 of the population). The initiative assessed the feasibility of educating women in labour to monitor their own FHR and alert a midwife of changes detected. METHODS: Four hundred seventy-four women admitted in labour without obstetric complications were approache…

Conventional Wisdom versus Actual Outcomes: Challenges in the Conduct of an Ebola Vaccine Trial in Liberia during the International Public Health Emergency.

Clinical trials are challenging endeavors. Planning and implementing an investigational vaccine trial in Liberia, in the midst of an Ebola virus disease (EVD) epidemic that World Health Organization classified a public health emergency of international concern, presented extraordinary challenges. Normally, years of preparation and a litany of tasks lay the groundwork for a successful, randomized, blinded, placebo-controlled trial focused on safety and efficacy. Difficult research settings, unpredictable events, and other unique circumstances can add complexity. The setting in Liberia was espec…

'We cannot just keep it in our palm': A policy analysis of the integration of the case management of neglected tropical diseases into the health system of Liberia.

This study investigates the development and implementation of Liberia's policy to integrate the case management of neglected tropical diseases (NTDs) into the broader health system. The policy responded to persistent challenges of inequitable access and fragmented services, as outlined in Liberia's 2016 national strategic plan for integrated NTD case management. Using a qualitative case study approach, the research draws on stakeholder perspectives to explore how integration was pursued and how policy was translated into practice. Guided by the Policy Analysis Triangle, this study examines the…

Combining cash transfers and cognitive behavioral therapy to reduce antisocial behavior in young men: A mediation analysis of a randomized controlled trial in Liberia.

BACKGROUND: Interventions that combine cognitive behavioral therapy (CBT) with unconditional cash transfers (UCT) reduce the risk of antisocial behavior (ASB), but the underlying mechanisms are unclear. In this paper, we test the role of psychological and cognitive mechanisms in explaining this effect. We assessed the mediating role of executive function, self-control, and time preferences. METHODS: We used data from the Sustainable Transformation of Youth in Liberia, a community-based randomized controlled trial of criminally engaged men. The men were randomized into: Group-1: control (n = 2…

A syndemic born of war: Combining intersectionality and structural violence to explore the biosocial interactions of neglected tropical diseases, disability and mental distress in Liberia.

The intersections between NTDs, disability, and mental ill-health are increasingly recognised globally. Chronic morbidity resultant from many NTDs, particularly those affecting the skin-including lymphatic filariasis (LF), leprosy, Buruli ulcer (BU) and onchocerciasis-is well known and largely documented from a medicalised perspective. However less is known about the complex biosocial interaction shaping interconnected morbidities. We apply syndemic theory to explain the biosocial relationship between NTDs and mental distress in the context of structural violence in Liberia. By advancing synde…

Infection rates and parasitic loads of Onchocerca volvulus, and other filariae, in Simulium sanctipauli s.l. and S. yahense in a rain-forest area of Liberia.

Simulium sanctipauli s.l. and S. yahense are common and widespread in the rain-forest zone of Liberia, but differ with regard to their biting densities and contribution to the transmission of Onchocerca volvulus. Although, in a study area on the St. Pauli River, S. sanctipauli s.l. (presumably S. soubrense in the sense of Post) was the predominant ma-biting species (74.3% of 30,855 females examined), S. yahense was shown to be the important vector. While 1000 biting females of S. yahense carried 96 3rd stage larvae indistinguishable from O. volvulus, only 14 were found per 1000 females of S. s…

Studies on nodules and adult Onchocerca volvulus during a nodulectomy trial in hyperendemic villages in Liberia and Upper Volta. I. Palpable and impalpable onchocercomata.

In hyperendemic onchocerciasis areas of West Africa nodulectomy trials were performed. 163 nodule carriers from four villages in the Liberian rain-forest and 52 nodule carriers from a savanna village in Upper Volta were operated on. During the surveys in the villages in Liberia and Upper Volta, an average of three and five nodules respectively per patient were palpated. Before each operation a further palpation was done and two more nodules per patient from both countries were found. The nodulectomies revealed 75% more nodules in the Liberian patients and 53% more in the patients from Upper Vo…

'He who knows it, feels it.' Stepping stones towards fulfilling a peer-advocate role among people with chronic stigmatising conditions in Liberia.

BACKGROUND: People affected by skin neglected tropical diseases (NTDs) are often stigmatised, experiencing discrimination in consequence of their condition. Peer advocates can help progress a person-centred agenda for people affected by NTDs, however, evidence gaps in the steps to becoming an advocate persist. METHODS: We wanted to understand the steps towards a peer advocacy role for skin NTDs. We used the participatory Stepping Stones method to identify steps on the journey to becoming an advocate with 48 participants, followed by three in-depth life histories with peer advocates for other

Interruption of tuberculosis detection and care during the Ebola virus disease epidemic (2014-2015) in Liberia: time-series analyses for 2013-2017.

OBJECTIVE: Interrupted time-series analyses, using 5 years of routinely collected health information system data, were conducted to estimate the magnitude of impact of the 2014-2015 Ebola virus disease (EVD) epidemic and determine trends in tuberculosis (TB) care services in Liberia. METHODS: A segmented linear regression model was used to generate estimates and predictions for trends for three TB service indicators before, during, and after EVD, from January 2013 to December 2017. RESULTS: It was found that the number of presumptive TB cases declined significantly at the start of the EVD ou…

Beating the Odds: Successful Establishment of a Phase II/III Clinical Research Trial in Resource-Poor Liberia during the Largest-Ever Ebola Outbreak.

It has been argued that a country such as Liberia, not fully recovered from the devastation of decades of civil unrest, lacked the appropriate ethical and regulatory framework, basic human and health care services, and infrastructure to carry out clinical trials according to international standards of quality during a public health emergency. However, as Liberia, Sierra Leone, and Guinea were being ravaged by the largest and most devastating Ebola Virus Disease (EVD) outbreak ever recorded, the topic of conducting clinical trials of experimental vaccine and treatment candidates in these resour…
