[Current status of minimally invasive treatment methods in sialolithiasis].

Newly available minimally invasive methods have changed established treatment of human sialolithiasis during the past several years. After basic in vitro and in vivo investigations, two systems of shockwave treatment proved to be useful for clinical application: extracorporeal shockwave lithotripsy (piezoelectric system) and intracorporeal laser lithotripsy (using a rhodamine-6G dye laser). Following our clinical experiences with these methods a differential scheme for managing sialolithiasis is recommended depending on localization of the calculi and their maximal diameters. [Intra-corporeal shockwave lithotripsy in the treatment of complex lithiasis of the bile ducts. Comparison of endoscopic techniques and long-term results].

OBJECTIVES: About 2% of common bile duct stones and most intra-hepatic stones cannot be removed by conventional endoscopy. Intra-corporeal lithotripsy is an alternative technique for these patients. Contact lithotripsy can be obtained by a pulsed dye laser or by electro-hydraulic shockwaves. We compared and assessed the results of these two methods. METHODS: Thirty-seven patients (79 +/- 9.8 years, 25 women and 12 men) underwent laser lithotripsy (n = 21), electro-hydraulic lithotripsy (n = 9) or both methods consecutively (n = 7) for common bile duct stones (n = 31), intra-hepatic stones (n

Lasertripsy in the treatment of ureteral lithiasis.

UNLABELLED: We submit our experience with laser treatment for ureteral lithiasis. We used the laser Candela MDL 2000 for the treatment of 62 lithiases (40 at the pelvic ureter, 16 at the iliac ureter and 6 at the lumbar ureter) in 58 patients. A semi-rigid Dretler or Gautier multiscope were used. Of the 62 calculi complete fragmentation was not achieved in 4 and they were ascended to the renal pelvis for subsequent ESWL. In 51% of the patients the ureteral catheter was left for 24 hours, and a double-J stent was used in two cases. COMPLICATIONS: 2 simple perforations of the ureter that were s

Is Bigger Better? Comparison of 150 µm and 200 µm Thulium Fiber for In Situ Lower-Pole Lithotripsy.

Introduction: The thulium fiber laser (TFL) generates a focused beam, which can be transmitted to laser fibers with small core diameters and may facilitate in situ lower-pole lithotripsy. This study compares lithotripsy performance of the 150 and 200 µm TFL in a lower pole benchtop kidney model. Materials and Methods: Using a 3D model printed from an actual kidney, in situ laser lithotripsy was performed on 1 cm lower-pole BegoStones (calcium oxalate monohydrate consistency) using four different settings (all 20W) and two fiber sizes (150 and 200 µm). Fragmentation of salivary stones with a 980nm diode laser.

OBJECTIVE: Intraductal laser lithotripsy is a preferred method to fragment large, immobile intraglandular salivary stones. A number of different laser systems has been investigated for this purpose. It was our aim to study the effectiveness of a 980nm diode laser when fragmenting salivary stones in an experimental set up. METHODS: In an experimental set up we used a 980nm diode laser for the lithotripsy of 9 salivary stones. The temperature circle around the laser fibre tip was measured and stone remnants were chemically analysed for their composition. RESULTS: The salivary stones had a mean

Biliary and pancreatic lithotripsy devices.

BACKGROUND AND AIMS: Lithotripsy is a procedure for fragmentation or destruction of stones to facilitate their removal or passage from the biliary or pancreatic ducts. Although most stones may be removed endoscopically using conventional techniques such as endoscopic sphincterotomy in combination with balloon or basket extraction, lithotripsy may be required for clearance of large, impacted, or irregularly shaped stones. Several modalities have been described, including intracorporeal techniques such as mechanical lithotripsy (ML), electrohydraulic lithotripsy (EHL), and laser lithotripsy, as

Efficacy of the Stone Cone for treatment of proximal ureteral stones.

BACKGROUND AND PURPOSE: Whereas ureteroscopic lithotripsy is more efficacious than SWL in treating lower-ureteral stones, the same has not been universally said of its ability to treat proximal ureteral stones. Because failed proximal lithotripsy is often attributable to the complications associated with stone migration into the renal pelvis and calices, an instrument that can prevent this migration is a potentially important tool in the ureteroscopic armamentarium. This study sought to assess the role of just such an instrument, the Stone Cone, in proximal-ureteral lithotripsy. PATIENTS AND

Utility of Direct Peroral Cholangioscopy Using a Multibending Ultraslim Endoscope for Difficult Common Bile Duct Stones.

BACKGROUND/AIMS: Treatment options for difficult bile duct stones are limited. Direct peroral cholangioscopy (POC)-guided lithotripsy may be an option. A newly developed multibending (MB) ultraslim endoscope has several structural features optimized for direct POC. We evaluated the utility of direct POC using an MB ultraslim endoscope for lithotripsy in patients with difficult bile duct stones. METHODS: Twenty patients with difficult bile duct stones, in whom stone removal using conventional endoscopic methods, including mechanical lithotripsy, had failed were enrolled from March 2018 to Augu

Validation of the Symu™ Flexible Ureteroscopy Platform Based on Real-Life Pulsed Tm:YAG Lithotripsy Using Specific 3D-Printed Artificial Stones.

INTRODUCTION: Simulation has become central in flexible ureteroscopy training and research; however, the lack of standardized and reproducible models limits objective performance assessment. We aimed to evaluate the reliability and reproducibility of laser lithotripsy using morphology-specific artificial stones (AS) within a standardized ex vivo platform and to assess whether the model could detect operator-dependent differences in lithotripsy performance. METHODS: This study was conducted using AS derived from 3D-printed images of human kidney stones. Six stone volume categories (F1-F6; 250-

Lasers in urology.

Ever since Mulvany first described use of Ruby laser for lithotripsy, urologists have been exploiting every possible application of this technology. Laser lithotripsy in the 1980s and now laser prostatectomy in the 1990s have dominated laser usage in urology. Applications of lasers for superficial lesions (e.g., condylomata acuminata and carcinoma of penis) have found an established role. Interests in laser welding, photodynamic therapy and fluorescence continues to grow and evolve. The laser industry at the same time is striving to provide more efficient lasers. High power lasers (Holmium:YAG

Getting hot in here! Comparison of Holmium vs. thulium laser in an anatomic hydrogel kidney model.

As laser technology has advanced, high-power lasers have become increasingly common. The Holmium: yttrium-aluminum-garnet (Ho:YAG) laser has long been accepted as the standard for laser lithotripsy. The thulium fiber laser (TFL) has recently been established as a viable option. The aim of this study is to evaluate thermal dose and temperature for the Ho:YAG laser to the TFL at four different laser settings while varying energy, frequency, operator duty cycle (ODC). Utilizing high-fidelity, 3D-printed hydrogel models of a pelvicalyceal collecting system (PCS) with a synthetic BegoStone implante

[The treatment of patients with ureteral calculi by ureteroscopy and intracorporeal lithotripsy].

Ureteroscopy with endocorporeal urinary stone lithotripsy (EUSL) implies an "in situ" stone fragmentation including or not a basket extraction of the fragments. EUSL needs three conditions: ureteronephroscopy (antegrade or retrograde), sources of energy (ultrasonic, electrohydraulic, laser), extraction devices (basket, grasping forceps). The paper reports the data on 120 ureteral stones treated by ureteroscopy and laser lithotripsy with an overall effect of 84%.
