Acute liver failure (ALF) is a relatively uncommon but dramatic clinical syndrome with high mortality rates, in which a previously normal liver fails within days or weeks. Paracetamol overdose remains the major cause of ALF in the UK, while viral hepatitis is the commonest cause world-wide. Cerebral oedema is the leading cause of death in patients with ALF. Despite advances in intensive care and the development of new treatment modalities, ALF remains a condition of high mortality best managed in specialist centres. Orthotopic liver transplantation is the only new treatment modality that has m…

For fulminant hepatic failure the prognosis is depending on the onset of severe encephalopathy (coma grade III to IV), cerebral oedema, renal and respiratory failure. Treatment strategies must be devoted to limit these complications and proceed with an urgent liver transplantation. Overall 1-year survival rates after hepatic transplantation in fulminant liver failure are as high as 80%.

Thyroid storm is a rare, life-threatening endocrine emergency that can cause multiorgan dysfunction, although acute liver failure (ALF) remains an uncommon manifestation. A 52-year-old woman with Graves' disease who had discontinued methimazole presented with three days of nausea, vomiting, diarrhea, melena, generalized weakness, and chest pain. She was lethargic and tachycardic, with a diffusely enlarged thyroid gland, warm skin, bilateral hand tremors, and hypoglycemia. Laboratory testing demonstrated severe thyrotoxicosis, marked hepatocellular injury with an aspartate aminotransferase conc…

Acute liver failure (ALF) is a rapidly progressive and potentially fatal condition in which successful treatment depends on the prompt identification of the underlying cause. We report the case of a 66-year-old woman who presented with severe hepatitis, marked coagulopathy, hyperbilirubinemia, and bicytopenia, while common etiologies of ALF were excluded. Imaging studies and initial bone marrow diagnostics did not provide a conclusive explanation. Subsequent histopathological evaluation of liver and bone marrow biopsies ultimately revealed hepatosplenic T-cell lymphoma, a rare and diagnostical…

Liver failure, whether due to alcoholic liver disease or hepatic malignancy, often leads to acute-on-chronic liver failure (ACLF), necessitating liver transplantation. A major challenge in managing these patients is the acute decompensation phase, during which hepatic toxins and inflammatory mediators accumulate, resulting in multi-system dysfunction. While liver transplantation remains the most effective treatment, artificial liver support systems, particularly extracorporeal blood purification, offer potential benefits. Hemoperfusion, utilizing solute adsorption via a solid agent, provides a…

This case report presents a newborn boy with hypoglycaemia, anaemia, jaundice and severe coagulopathy during the first day of his life, imitating sepsis and disseminated intravascular coagulation. One week after the birth he was diagnosed with acute liver failure due to gestational alloimmune liver disease (GALD). Despite the fact that GALD is rare, it must be suspected in all unexplained stillborn children and infants with severe liver disease. If diagnosed, it is possible to prevent death and severe liver failure in future newborns by treating the affected women with immunoglobulin during pr…

OBJECTIVES: No extensive investigation has been performed and thus no representative data are available regarding acute liver failure (ALF) in China. This study aims to investigate the causes and outcomes of ALF in China and establish a prognostic model.
METHODS: Patients diagnosed as ALF in seven hospitals in different areas of China from January 2007 to December 2012 were retrospectively selected.
RESULTS: Of the 177 patients included in this study, 112 (63.28%) eventually died. The common causes of ALF were drug toxicity (43.50%), indeterminate etiology (29.38%) and acute viral hepatitis …

BACKGROUND & AIMS: Acute liver failure (ALF) is a severe form of acute liver injury that can progress to multiple organ failure. We investigated causes and outcomes of ALF.
METHODS: Eleven university medical centers in Germany were asked to report patients with (primary) severe acute liver injury (sALI) (international normalized ratio [INR] >1.5 but no hepatic encephalopathy) and primary ALF (INR >1.5 with overt hepatic encephalopathy) treated from 2008 to 2009. Data were analyzed from 46 patients with sALI and 109 patients with ALF.
RESULTS: The most frequent etiologies of primary ALF were …

At present, the most successful treatment of acute liver failure is orthotopic liver transplantation, with survival rates ranging from 70% to 85%. However, mortality rates for liver failure remain high because of the shortage of available donor organs. Therefore, there has been renewed interest in temporary treatment methods for patients with acute liver failure to either allow liver regeneration or await liver transplantation. It is thought that the function of the liver can only be replaced with the biological substrate, e.g. liver cells or a whole liver specimen, which requires the availabi…

Liver injury induced by various pathogenic factors (such as hepatitis virus, ethanol, drugs and hepatotoxicants, etc.) through their respect … lammatory responses, with serious hepatic necrosis, further severe hepatitis and even induces acute liver failure. The milder IETM successively precipitates a cascade, including repeated and persistent hepatocytic impairment accompanied by infiltration of inflammatory cells, hepatic fibrosis, cirrhosis and hepatocarcinoma. Generally, the milder IETM ends with c …

Acute liver failure was diagnosed in a 12-year-old cat. Fine needle aspirate cytology revealed high numbers of unsheathed microfilariae and a hepatocellular reaction with no evidence of bacterial infection. The microfilariae were identified as those of Dirofilaria repens by acid phosphatase staining. The high number of microfilariae seen in both the blood and the liver aspirate samples as well as the favourable response to ivermectin amongst other drugs administered, is suggestive that D. repens was the cause of the liver insult. A positive result obtained with an antigen-capture ELISA (Diroch…

This study aimed to share the case caused by N,N-Dimethylacetamide (DMAC) exposure. We report a 30-year-old man who died from DMAC exposure-induced acute liver failure after working in the cutting and cleansing of hollow fiber membranes for 5 days. While he was exposed to low-level DMAC by inhalation as the measurement of periodic working environment monitoring, considering his working method, considerable DMAC exposure via dermal absorption was suspected. Therefore, short-term, short-latency DMAC exposure via dermal absorption can cause fatal acute liver failure owing to idiosyncratic reactio…

Paediatric acute liver failure (PALF) is often characterised by its rapidity of onset and potential for significant morbidity and even mortality. Patients often develop multiorgan dysfunction/failure, including severe acute kidney injury (AKI). Whilst the management of PALF focuses on complications of hepatic dysfunction, the associated kidney impairment can significantly affect patient outcomes. Severe AKI requiring continuous kidney replacement therapy (CKRT) is a common complication of both PALF and liver transplantation. In both scenarios, the need for CKRT is a poor prognostic indicator. …

BACKGROUND: Living-donor liver transplantation (LDLT) has been increasing in the USA. While data exist on longer-term patient and graft outcomes, a contemporary analysis of short-term outcomes is needed.
AIM: Evaluate short-term (30-day) graft failure rates and identify predictors associated with these outcomes.
METHODS: Adult (≥ 18) LDLT recipients from 01/2004 to 12/2021 were analyzed from the United States Scientific Registry of Transplant Recipients. Graft status at 30 days was assessed with graft failure defined as retransplantation or death. Comparison of continuous and categorical var
