For about three quarters of a century, radical mastectomy was considered standard initial treatment for invasive breast cancer. Within the past 15 years, modified radical mastectomy (total mastectomy with axillary dissection) has replaced radical mastectomy as standard treatment, even though superiority of the former has not been scientifically proven, with the exception of a single study. During the same period, total mastectomy with postoperative irradiation of axillary lymph nodes came into vogue, and debate centered on whether irradiation of nodes was as efficacious as surgical extirpation…

Galactorrhea is a recognized sequel of chest injury, but serum PRL levels in these patients have not been systematically documented. Therefore, we examined the PRL responses over 5 days in patients undergoing either mastectomy (10 patients) or thoracotomy (10) and in seven patients undergoing elective laparotomy (controls). Basal serum PRL levels were normal in every subject. There were no consistent or significant alterations in PRL levels after laparotomy or thoracotomy. After mastectomy, PRL levels rose from a mean preoperative level of 7.1 +/- 1.3 to 16.0 +/- 3.3 ng/ml (P < 0.01) on the fi…

Post-mastectomy breast reconstruction (PMBR) plays a critical role in restoring physical appearance and psychosocial well-being. However, pronounced geographic and socioeconomic disparities in PMBR access persist worldwide, largely driven by insurance coverage variations. We reviewed English-language literature examining regional insurance disparities in PMBR access through PubMed searches and reference screening. Access to PMBR varies dramatically across regions. Africa demonstrates the lowest utilization; fewer than 10% of eligible women undergo reconstruction, primarily through out-of-pocke…

BACKGROUND: Hematoma formation is a common complication following gender-affirming mastectomy, yet the contribution of perioperative hemodynamics to hematoma risk in this population remains unclear.
METHODS: A retrospective cohort study was performed of transgender patients undergoing bilateral gender-affirming mastectomy by a single surgeon from 2016 to 2023. Hematoma was defined as a postoperative collection requiring surgical evacuation. MAP was calculated at baseline, preoperatively, intraoperatively, and within one hour postoperatively. Group comparisons and multivariable logistic regres…

BACKGROUND: Although autologous reconstruction after nipple-sparing mastectomy (NSM) is well established, the subset of buried-free flap reconstructions remains less well characterised. With evolving surgical techniques understanding the safety, efficacy, and outcomes of buried-free flap reconstructions is increasingly important.
AIM: To review recent literature regarding autologous buried-free flap reconstructions post mastectomy, including demographics, indications, outcomes and complications.
METHODS: A search of PubMed and Medline databases was performed using the keywords "deep inferior…

BACKGROUND: Mastectomy and its associated treatments may alter shoulder biomechanics and soft-tissue integrity, yet their impact on total shoulder arthroplasty (TSA) outcomes remains unclear. This study evaluated short- and long-term complications following TSA in patients with a history of mastectomy. METHODS: A retrospective cohort analysis was performed using the TriNetX Research Network, including patients who underwent primary TSA. Individuals with a history of mastectomy were identified using procedural and diagnostic coding, and those with contralateral mastectomy and TSA were excluded.…

BACKGROUND: Wound complications are known to occur after nipple-sparing mastectomy (NSM), especially in procedures involving a periareolar incision. We sought to determine the independent risk of infection and overall wound complications associated with NSM vs skin-sparing mastectomy (SSM) in a contemporary cohort of women undergoing immediate implant reconstruction.
STUDY DESIGN: We identified 352 women 21 years of age or older undergoing mastectomy and immediate implant reconstruction from September 2021 to December 2022 using electronic health records from 2 hospitals. The relative risk (R…

BACKGROUND: Women who have a mastectomy for breast cancer treatment or risk reduction may be offered different options for breast reconstruction, including use of implants or the woman's own tissue (autologous tissue flaps). The choice of technique depends on factors such as the woman's preferences, breast characteristics, preoperative imaging, comorbidities, smoking habits, prior chest or breast irradiation, and planned adjuvant therapies.
OBJECTIVES: To assess the effects of implants versus autologous tissue flaps for postmastectomy breast reconstruction on women's quality of life, satisfac…

BACKGROUND: Every year, more than 90,000 U.S. women undergo mastectomy. More than 40% have reconstruction. Following reconstruction, most women experience persistent partial or complete numbness of the reconstructed breasts, and many experience pain. Yet, breast reconstruction procedures focus largely on esthetic outcomes with mixed impact on sensory outcomes and little attention to pain. This study examines whether and how breast sensation is important to women.
MATERIALS AND METHODS: Conventional content analysis of extant qualitative data from a clinical registry (29 women with prior breas…

BACKGROUND: Infection after nipple-sparing mastectomy (NSM) and implant-based reconstruction (IBR) can be a devastating complication. The retained nipple may act as a portal or nidus for different ductal organisms, and as such, the bacteriology of surgical-site infections (SSIs) in this setting may not be adequately covered by current antibiotic recommendations. This study sought to evaluate SSI and reconstruction outcomes in relation to antibiotic choice and identify the different microbial species implicated.
METHODS: A prospective database was reviewed for patients who underwent NSM with I…

BACKGROUND: Although the benefits of gender-affirming surgery may be apparent to patients and providers, there remains a paucity of studies assessing the impact of these procedures. As an initial step, preoperative patient-reported outcomes using validated measures of depression, anxiety, and body image were used and compared to cisgender normative data.
METHODS: Patients presenting for gender-affirming mastectomy were approached and surveyed using validated instruments measuring anxiety, depression, and body image. In addition, clinical data were collected from the medical record. Results we…

Germline variations in the BRCA-1 and BRCA-2 genes are associated with an increased risk of breast cancer. These variants are found in 5% of all breast cancer cases. Prophylactic mastectomy is the most effective risk-reducing method and shows high rates of patient satisfaction and acceptance. We established a registry of Austrian BRCA-1 and BRCA-2 mutation carriers who had undergone mastectomy for oncologic or prophylactic reasons. Data were collected on the type of operation, complications, and type of reconstructive surgery for patients between 2014 and 2017. The complication rate in patient…

Mastectomy may be performed to treat breast cancer or as a prophylactic approach in women with a high risk of developing breast cancer. In addition, mastectomies may be performed with or without reconstruction. Reconstruction approaches differ and may be autologous, involving a transfer of tissue (skin, subcutaneous fat, and muscle) from other parts of the body to the chest wall. Reconstruction may also involve implants. Implant reconstruction may occur as a single procedure or as multistep procedures with initial use of an adjustable tissue expander allowing the mastectomy tissues to be stret…

INTRODUCTION: The number of patients requesting prophylactic mastectomy with immediate reconstruction is rising. The oncological safety of techniques preserving the nipple and/or areola complex is still controversial. Nevertheless, nipple-sparing mastectomy (NSM) and areola-sparing mastectomy (ASM) are becoming increasingly popular. After ASM, traditional nipple reconstruction techniques can be a disappointment and can lead to a deep groove around the new nipple. We describe a technique to overcome these issues and analyzed how three types of mastectomy (skin-sparing mastectomy or SSM, ASM, an…

Opioid-free anesthesia (OFA) is being implemented in breast surgery due to increased awareness of adverse effects and the national opioid crisis. The objective of this study was to examine the effect of OFA on postoperative pain and postoperative nausea and vomiting (PONV) in mastectomy patients. A single-institution matched-cohort study was conducted from 2014 to 2017 on 48 women undergoing mastectomy, with the majority also undergoing immediate prosthetic-based reconstruction. Patients received either conventional anesthesia (CA) or a novel OFA regimen. Primary outcomes included postoperativ…
