Zygomatic Implants: A Solution for the Atrophic Maxilla: 2021 Update.

Restoring the dentition of an edentulous patient is often challenging. Endosseous dental implants have allowed for far more versatility in this area but still require adequate maxillary and mandibular alveolar bone. Often, unless significant bone grafting techniques are used, true restoration of the dentition can be impossible with traditional endosseous implants. The advent of zygomatic implants, however, may provide a viable, predictable, and stable alternative for the restoration of the dentition in patients with severe maxillary alveolar bone loss.

Pycnodysostosis with Osteomyelitis of Maxilla: Case Report of Radiological Analysis.

Pycnodysostosis is an autosomal recessive, rare genetic osteosclerotic disorder that caused by mutation in gene coding for Cathepsin K. The bones in pycnodysostosis are abnormally dense and brittle because of insufficient reabsorption process. This syndrome has a number of characteristic clinical and radiographic signs that differentiate it from other osteosclerotic conditions. This is a rare case report of a male patient with a history of multiple fractures of bones and osteomyelitis of maxilla which is a rare entity.

Extra-short implants in the prosthetic rehabilitation of the posterior maxilla.

AIM: To compare clinical outcomes of 'extra-short' and regular bone level implants in the posterior maxilla for 12 months after loading. MATERIALS AND METHODS: Twenty-three systemically healthy, non-smoking patients received 30 extra-short, 24 regular bone level implants. Acrylic stents were fabricated for each patient for correct implant positioning. Implant lengths were 4-6 mm in the test, 8/10 mm in the control group. Radiographic evaluation was performed at baseline, 6, and 12 months after loading. Crestal bone level (CBL), CBL change (CBLC), true crown length (TCL), implant/crown ratio (…

Oral Epignathus with Maxilla Duplication: Report of a Rare Case.

Epignathus is a rare congenital oropharyngeal teratoma that arises from the oropharynx, especially the sphenoid, palatine, and ethmoid bones … scan showed a contrast-enhanced solid mass with areas of calcification simulating a unique case of maxilla duplication. Surgery was performed, the mass was excised successfully, and microscopic analysis confirmed the diagnosis of mature teratoma. The patient evolved with good general health and showed no clinical signs of recurrence. Although epignathus …

Ossifying fibroma of the maxilla and sinonasal tract: Case series.

BACKGROUND: Head and neck ossifying fibroma (OF) is a rare, benign, locally aggressive, fibro-osseous tumor. The mandible is the most common site of involvement, followed by the maxilla, and, less frequently, the sinonasal cavities, orbit, skull base, and calvarium. In this study, we aimed to expand our understanding of this entity by presenting a case series of OF that involved the maxilla and sinonasal tract. METHODS: A multicenter retrospective review was performed on all the patients with a diagnosis of OF from 2004 to 2013. Data were collected with respect to age, sex, clinical presentat…

Aggressive Osteoblastoma in the Maxilla: Unusual Lesion in the Craniofacial Skeleton.

Osteoblastomas are benign bone tumors, which are unusual in the craniofacial skeleton, being most often observed in the axial skeleton and long bones. The most common site in the maxillofacial region is the mandible and the involvement of the maxilla and paranasal sinuses is extremely rare. Although it is a benign lesion, the aggressive variant raises concerns due to its huge local destructive potential and tendency to relapse. In this clinical case, an aggressive osteoblastoma is described in a 7-year-old patient. The lesion was large and fully involved the left maxilla, including the maxilla…

Histological and radiographic study of human edentulous and dentulous maxilla.

Data on the bone trabecular structure and density of the edentulous regions of the first upper molars are important for designing successful dental treatments, especially dental implants. However, no detailed defined morphometric properties on the human maxilla are available at the immunohistochemical and radiographic levels. Cone-beam computed tomography analysis and immunohistochemical observation were applied to the maxillary first molar region of 91 cadavers (46 males and 45 females). The edentulous maxilla can be classified into the following three forms: fully edentulous (FE), partially …

Ectopic third molar tooth on the infratemporal surface of maxilla.

Ectopic eruption of a tooth within the dentate region of the jaws is often noticed in clinical practice and is well documented in the literature but the ectopic eruption into the non dentate region is rare. Diverse anatomical locations such as nasal cavity, nasal septum, mandibular condyle, coronoid process, the palate and the maxillary sinus can infrequently be non dentate sites of ectopic eruptions of teeth. While conducting an anthropometric study on 100 skulls incidental and interesting case of ectopic maxillary third molar tooth was found. The tooth was partially erupted with the unerupte…

Juvenile Trabecular Ossifying Fibroma of the Maxilla: a Case Report.

INTRODUCTION: Juvenile ossifying fibromas are uncommon benign tumors. Their aggressiveness added to their high tendency to recur, provoke real diagnostic and therapeutic challenges for the dental practitioner and make a postoperative follow-up over the years indispensable. CASE REPORT: In this report, we present a case of a seven-year-old girl presented with a swelling in the face at the upper right maxillary region. After clinical, radiological, and histopathological examinations the diagnosis of trabecular juvenile ossifying fibroma was made. The lesion was surgically excised and followed u…

Haemophilic pseudotumour in two parts of the maxilla: case report.

Haemophilic pseudotumour is a rare disease that occurs most often in femur, tibia, ilium or pelvic bone of a patient with haemophilia. Thus far, there have been only 31 reported cases in jaw bones and paranasal sinuses. Among them, the mandible is a more common site than the maxilla or paranasal sinuses. Here, we report a case of haemophilic pseudotumour in two parts of the maxilla. Contrast-enhanced CT showed an expansive and thinly corticated lesion with fluid attenuation at the left anterior maxilla which seemed like a post-operative maxillary cyst, ameloblastoma or odontogenic cyst. In add…

A rare case of paediatric histiocytic sarcoma of the maxilla and mandible.

Histiocytic sarcoma is an extremely rare malignant neoplastic proliferation of the haematopoietic cells. Very few cases have been reported in the paediatric age group. Imaging features have been rarely described in the literature. It can involve any region of the body; however, it most commonly involves the lymph nodes. Its imaging appearance can mimic lymphoproliferative disorders; however, with the advent of new immunohistochemical markers, the diagnosis of HS has become more reliable. We report an unusual case of primary bone involvement by HS with multiple lesions in the facial bones of a

A Rare Pathology: Low-Grade Fibromyxoid Sarcoma of the Maxilla.

Low-grade fibromyxoid sarcoma (LGFMS) is a rare tumor with a benign histologic appearance and fully malignant behavior. To date, only 5 cases of LGFMS in the maxillofacial region have been reported. This report describes the case of a 16-year-old boy who was referred to the authors' hospital with an intraosseous myxofibroblastic tumor, probably of the LGFMS type, of the right maxilla. Radical resection with wide safe margins and secondary reconstruction with a free forearm flap were performed. Six-month follow-up showed no sign of recurrence or metastasis. The authors review the scientific lit…

Osteomyelitis of Maxilla in Infantile With Periorbital Cellulitis: A Case Report.

Infantile osteomyelitis (IO) is an uncommon and life-threatening disease that can be misdiagnosed. Early diagnosis and treatment can reduce the incidence of sequel. In this case report, we present a 25-day-old male infant with apparent edema in the entire left periorbital region. Intraorally, the edema occurred in the mucosa of the upper left alveolar region, and 2 draining fistulas with exuded yellow-white pus were present in the left alveolar region. The patient received constant monitoring after admission, and was diagnosed as IO of the maxilla with periorbital cellulitis and sepsis. He als…

Primary malignant melanoma of maxilla: report of a case with discussion.

Primary oral malignant melanoma, very rare neoplasm of melanocytic origin, usually presents as a bluish black to tan-brown colored lesion Which is accounting for 0.2 to 8% of all melanomas, 1.6% of all head and neck malignancies, and 0.5% of all oral neoplasia. In general, the prognosis of oral melanoma is poor and worse than that of cutaneous melanoma. Here a case of oral malignant melanoma is presented, which was undetected during the first visit to a dental clinic. When a simple oral surgical treatment was carried out in that region, it resulted in the appearance of a massive pigmented lesi

Arteries of the anterior wall of the maxilla in sinus lift surgery.

Knowledge of the anatomical course of the anterior maxillary wall and alveolar process arteries is essential for sinus lift procedures. The aim of this study was to analyse the localization of anastomoses between the infraorbital and posterior superior alveolar artery on 101 computed tomography (CT) scans of maxillary sinuses (patients aged 18-70 years). The distance from the anastomosis to the bone point was measured on CT scans for each tooth location; for edentulous patients measurements were made to the lower edge of the alveolar process, and for dentate patients to the neck of the tooth. …

Evaluation of anatomical considerations in the posterior maxillae for sinus augmentation.

The edentulous posterior maxilla is considered a clinical challenge during dental implant treatment for many dental practitioners. This is because its insufficient bone quality, deficient alveolar ridge, spiny ridges, undercuts, and sinus pneumatization are often encountered after tooth loss. To overcome these problems, several approaches have been developed and are currently used, including sinus augmentation and bone augmentation. Today, two main procedures of sinus floor elevation for dental implant placement are in use: a two-stage technique using the lateral window approach, and a one-sta…

Bisphosphonate-induced osteonecrosis of the maxilla presenting as a cicatricial ectropion.

A 63-year-old gentleman, who was being treated with bisphosphonates for multiple myeloma, presented with a cicatricial ectropion of the lower eyelid, without exposure keratopathy. A CT scan demonstrated extensive destruction of bone with an infraorbital fracture surrounded by sclerotic bony changes. The patient was managed conservatively with discontinuation of bisphosphonate therapy and topical ocular lubricants. The patient's condition remained unchanged a year after this initial management.

Unusual presentation of an intraosseous hemangioma of the maxilla and displaced canine.

Intraosseous hemangiomas are benign vascular malformations which are extremely rare in the maxilla, but have been reported in the mandible, zygoma and orbital region.
