Low early complication rates after arthroscopic meniscus repair and meniscectomy.

PURPOSE: To evaluate the 30-day complication rates after arthroscopic meniscus repair and meniscectomy using the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database, with subgroup analysis of patients aged > 40 years. METHODS: NSQIP registries between 2006 and 2019 were queried using Current Procedural Terminology codes to identify patients undergoing arthroscopic meniscus repair (CPT 29882, 29883) and meniscectomy (29880, 29881). The following 30-day complications were assessed: pulmonary embolism (PE), venous thromboembolism (VTE), surgical site i…

Meniscus Radial Tears: Current Concepts on Management and Repair Techniques.

PURPOSE OF REVIEW: This review provides a historical perspective on the approach to radial tears and collates the currently available evidence on repair techniques, rehabilitation, and outcomes following the treatment of meniscus radial tears. RECENT FINDINGS: Recent literature shows that the repair of meniscus radial tears reports improved patient-reported outcome scores with high return to function and activity. However, no single technique nor construct was proven better than the other. Various methods of repairing radial tears can be employed, with biomechanical research supporting all-in…

Hybrid Repair of a Medial Meniscus Mid-Body Radial Tear.

BACKGROUND: Meniscal radial tears are associated with altered contact mechanics, as they have the potential to disrupt the meniscal hoop stress mechanism. In prior studies, radial tears are associated with higher grade of cartilage damage, as well as higher rate and severity of meniscal extrusion in knees with a radial tear relative to other types of tears. Improved understanding of the meniscus biological potential together with modern developments in surgical technique has paved the way for the current emphasis on repairing even radial tears. INDICATION: Large or complete radial tears of th…

Transient inhibition of meniscus cell migration following acute inflammatory challenge.

Meniscus tears represent a common orthopedic injury that often requires surgery to restore pain-free function. The need for surgical intervention is due, in part, to the inflammatory and catabolic environment that inhibits meniscus healing after injury. In other organ systems, healing is dependent on the migration of cells to the site of injury; however, in the meniscus, it is currently unknown how the microenvironment dictates cell migration in the postinjury inflamed setting. Here, we investigated how inflammatory cytokines alter meniscal fibrochondrocyte (MFC) migration and sensation of mic…

Chinese Experts Consensus and Practice Guideline on Discoid Lateral Meniscus.

Discoid lateral meniscus (DLM) is the most common congenital variant of the lateral meniscus, which is prone to degeneration and lesions, and often leads to knee osteoarthritis. At present, there is no consensus on the clinical practice of DLM, and this expert consensus and practice guidelines on DLM was developed and approved by Chinese Society of Sports Medicine according to the Delphi method. Among 32 statements drafted, 14 statements were excluded for redundant information, and 18 statements achieved consensus. This expert consensus focused on the definition, epidemiology, etiology, classi…

All-Soft Tissue Meniscus Allograft Transplantation: Indications, Techniques, and Results.

BACKGROUND: Meniscus allograft transplantation (MAT) may be indicated for young patients with joint line pain following subtotal or total meniscectomy. Several different approaches for performing MAT have been described in the literature and are influenced by appropriate patient selection, graft-sizing, and soft-tissue fixation techniques. Clinical studies demonstrate favorable results regarding pain relief and knee function in young patients undergoing MAT, making it a viable option for the treatment of postmeniscectomy syndrome. INDICATIONS: Meniscus allograft transplantation is indicated f…

The meniscal ossicle associated with medial meniscus posterior root tear.

Ossicle of the meniscus is an uncommon discovery often misdiagnosed as a loose body, which may lead to intermittent knee discomfort. We present a rare case of meniscal ossicle accompanied by the medial meniscus posterior root tear. A 46-year-old female experienced intermittent left knee pain and after coming to the hospital was diagnosed with a meniscal ossicle. The patient underwent arthroscopic ossicle resection followed by meniscal root repair. The patient had not experienced any complications post-operatively and remains asymptomatic 8 months after the surgery. The purpose of this article …

[Internal meniscus resections do not affect anterior knee joint stability].

BACKGROUND: Meniscus injuries lead to increased knee joint instability. Currently, however, it is unclear whether a relevant medial meniscus part resection leads to an increased ventral tibia translation with intact anterior cruciate ligament. The aim of our study was therefore to clinically examine the stabilizer function of at least 30% resected medial meniscus for anterior tibial translation. MATERIALS AND METHODS: In this prospective study, 18 patients with unilateral medial meniscus lesion were treated before and after arthroscopic medial meniscus resection. They were treated on the heal…

Alternative All-Inside Technique of Repairing a Vertical Meniscus Tear.

Lateral meniscus vertical tears are often seen with acute anterior cruciate ligament injuries. The inside-out technique of repairing the meniscal tear is currently the gold standard treatment. However, every technique has its own drawbacks. The inside-out technique has possible risk of infection and neurovascular complications. All-inside techniques are becoming increasingly popular. A novel all-inside technique and a variation of it for meniscus repair using an antegrade suture passer like a Knee Scorpion are described. These are cost effective using a 2-0 FiberWire for repair of vertical men…

Rare presentation of lateral meniscus tear with pathognomonic MRI finding.

INTRODUCTION: Meniscal tears are common knee injuries that occur in different populations and particularly among athletes. Either isolated or accompanied with anterior cruciate ligament injury, meniscal tears can be a source of significant knee pain, locking and instability. Different patterns of meniscal tears are described radiologically and arthroscopically, however displacement of the torn part of the meniscus can cause characteristic appearance on MRI. PRESENTATION OF THE CASE: The reported case is of a 27-year-old, previously healthy gentleman who presented to our care with acute sharp …

Biomechanical properties of murine meniscus surface via AFM-based nanoindentation.

This study aimed to quantify the biomechanical properties of murine meniscus surface. Atomic force microscopy (AFM)-based nanoindentation was performed on the central region, proximal side of menisci from 6- to 24-week old male C57BL/6 mice using microspherical tips (Rtip≈5µm) in PBS. A unique, linear correlation between indentation depth, D, and response force, F, was found on menisci from all age groups. This non-Hertzian behavior is likely due to the dominance of tensile resistance by the collagen fibril bundles on meniscus surface that are mostly aligned along the circumferential direction…

Unusual appearance of an osteochondral lesion accompanying medial meniscus injury.

An osteochondral lesion in the knee joint is caused by a focal traumatic osteochondral defect, osteochondritis dissecans, an isolated degenerative lesion, or diffuse degenerative disease. An osteochondral lesion with a cleft-like appearance accompanying medial meniscus injury is rare without trauma. We report the case of a 13-year-old boy who complained of right knee pain and swelling, with radiographic findings of an osteochondral defect. Arthroscopic inspection showed an osteochondral lesion in the medial condyle of the femur and tibial plateau accompanying a partial medial meniscus discoid …

Dislocated double-layered lateral meniscus mimicking the bucket-handle tear.

Various shapes of congenital abnormalities of the meniscus have been reported. Among them, the double-layered meniscus is rare. This article describes a 22-year-old man with a double-layered lateral meniscus who reported right knee pain with no history of trauma. The double-layered lateral meniscus included both the upper and lower meniscus. The anterior and posterior edge of the upper meniscus was attached to the lower lateral meniscus, and its periphery was not connected to the capsule and the lower meniscus. In addition, the upper meniscus was dislocated into the intercondylar notch, mimick…

Bilateral double-layered lateral meniscus: a report of two cases.

Only a few cases of double-layered meniscus have been described in the English literature. We report two cases of bilateral double-layered lateral meniscus, where an additional semicircular meniscus was observed over the normal lateral meniscus. One of the patients exhibited a bucket-handle tear with a double-layered meniscus. To our knowledge, this abnormality is extremely rare and the incidence of double layered meniscus with bucket-handle tear has not been previously reported.

Symptomatic ring-shaped lateral meniscus: magnetic resonance imaging and arthroscopy.

A ring-shaped lateral meniscus is a very rare entity within the spectrum of abnormalities of lateral meniscus. We present the second case in the literature of a symptomatic ring-shaped lateral meniscus in combination with a meniscal cyst and in association with an osteochondritis dissecans lesion of the medial femoral condyle. A potential pitfall of magnetic resonance imaging (i.e., displaced meniscal tear) is addressed in the diagnosis of this entity, as well as the role of magnetic resonance imaging in revealing associated abnormalities.

Open meniscus repair. Technique and two to nine year results.

An average 4.6 year follow-up study has been done on 80 of 104 open meniscus repairs of unstable peripheral tears. Two of nine retears were treated in the acute stage and seven were treated in the chronic stage. Eight retears were medial, one was lateral (repair ratio, 1.5:1 medial to lateral), three occurred through the repair site, and six occurred in different areas. Retears occurred in 1 of 26 isolated repairs, 2 of 38 repairs done with an ACL stabilization procedure, and 6 of 16 repairs done in ACL deficient knees that were not stabilized. Standing radiographs revealed normal compartments…
