Penetrating injury of pelvis, abdomen and thorax in a child with a trident (trishula).

Penetrating abdominal injuries are uncommon in the pediatric age group and are associated with a high mortality. A seven year old girl suffered penetrating injury to perineum when she fell onto the trident (trishula - traditional three pronged spear) implanted alongside Lord Shiva's idol. The rod caused perforations of the transverse mesocolon, ileum at 3 places, right lobe of liver, diaphragm, parietal pleura, and exited from 7th intercostal space. Surgical repair of each damaged site was undertaken. Despite delayed presentation, the child survived following surgery.

A case of extragastrointestinal anisakiasis involving a mesocolic lymph node.

In a 43-year-old Korean man who underwent radical gastrectomy due to a malignant stromal tumor, was found to have an enlarged lymph node at transverse mesocolon. The lymph node exhibited histologically necrotizing eosinophilic granuloma formed around a track containing sections of a nematode larva. The well preserved nematode sections revealed polymyarian muscle cells, Y-shaped lateral cord, a large excretory gland cell, intestine and eosinophilic cuticle. The nematode sections were identified as a larva of Anisakis species. In Korea, this is the first case of extragastrointestinal anisakiasis…

Mesenteric panniculitis: report of two cases.

Mesenteric panniculitis is a rare inflammatory process of the mesenteric fat with unknown etiology. We report two pathologically proven cases of mesenteric panniculitis. Both patients presented with abdominal pain and palpable masses. In one patient, computed tomography (CT) demonstrated a well-defined heterogenous mass containing fat density at the mesocolon. In the other patient, CT showed a fatty mass encompassing mesenteric vessels at the root of the mesentery. Although the definite diagnosis of mesenteric panniculitis depends on histopathology, recognition of CT features is helpful in the…

[Clinical-diagnostic and therapeutic considerations in a rare case of mesenteric cyst].

The authors report a rare case of mesenteric cyst in a 36 years old woman. These cysts have a pathogenesis that primarily may be ectopic lymphatic tissue and their most common site is in the small bowel mesentery, especially of the ileum. In the case reported the cyst was located in the right mesocolon. Mesenteric cysts can appear as chronic abdominal pain, a painless abdominal mass, or acute abdomen. Diagnostic aids include abdominal computed tomography and sonography, that usually make diagnosis of mesenteric cyst. Treatment of choice is enucleation; resection of the adjacent bowel may occas…

[Ganglioneuromatosis with carcinoid foci in the large bowel].

A rare case of a 27-year-old man who suffered from ganglioneuromatosis with five carcinoid foci arising from the colon and underwent subtotal colectomy is reported. There was no malignancy. The tumors extended to the mucosa and/or submucosa, and in most parts, tumors created polypoid lesions protruding into the intestinal lumen. In the rectum the tumors showed circumscribed extension around the wall. In the sigmoid and descending colon, polypoid lesions were seen on the side opposite from the attachment line of the mesocolon. In one of five carcinoid foci, a transitional pattern between carcin…

[Retroperitoneal rupture of the duodenum caused by closed abdominal injury].

Preoperative diagnosis of retroperitoneal rupture of the duodenum is very difficult. Radiographical observation of air surrounding the right kidney or psoas muscle is of great assistance in this respect, but may only be expected in 30% of cases. Examination of the first segment of the digestive tract with the aid of "gastropgrafin" is advised. With the abdomen open, the presence of air, haematoma or bile in the transverse mesocolon or mesentery of the small intestine may be noted. In most cases, the breach can be closed with a double layer, leaving a drainage tube in the retroperitoneum. Measu…

Extended Lymphadenectomy for Proximal Transverse Colon Cancer: Is There a Place for Standardization?

Background and Objectives: Complete mesocolon excision and high vascular ligation have become a standard procedure in the treatment of colon cancer. The transverse colon has certain embryological and anatomical particularities which require special attention in case of oncological surgeries. Proximal transverse colon cancer (TCC) can metastasize to the lymph nodes in the gastrocolic ligament. The aim of this study is to assess the tumor involvement of these lymph nodes and to determine the applicability of gastrocolic ligament lymph nodes dissection as the standard approach for proximal transv…

Standardization of the surgical technique and reporting for radical right colectomy with central vascular ligation and complete mesocolic excision (RRoC-STAR): Delphi consensus.

BACKGROUND: Complete mesocolic excision refers to a radical right hemicolectomy for cancer following embryologically defined anatomical planes. However, heterogeneity in definitions and techniques is a barrier to research. The aim of the Radical Right Colectomy-Surgical Technique Approved Report (RRoC-STAR) collaborative is to provide international expert consensus-based definitions and standardized terminology and surgical steps for right hemicolectomy for locally advanced colon cancer. METHODS: Authors of publications reporting on radical right hemicolectomy techniques were invited to compl

[Mesocolic and mesenteric lymph node granulomatosis together with a sarcoidosis (author's transl)].

The authors report the case of a 22 year old woman with a recognized pulmonary sarcoidosis with disorders of the alveolo-capillary diffusion, admitted to hospital for feverish diarrhea. The discovery of right colic radiological lesions with no obvious origin, led to an exploring laparotomy which revealed large mesocolic and mesenteric adenopathies: histologically they were granulomatous lesions, of epithelioid and giant cells, tending to sclerosis. Colic and ileal mucosae were normal. From this observation the authors analyze the difficulties in diagnosing granulomatoses of the intestine and m

A review of internal hernias related to congenital peritoneal fossae and apertures.

Congenital-type internal hernias have the potential to cause small bowel obstruction well into adulthood. Congenital-type internal hernias include left paraduodenal, right paraduodenal, foramen of Winslow, pericecal, sigmoid mesocolon, transomental, small bowel mesentery, and broad ligament hernias. This review summarizes CT imaging features and complications of congenital internal hernias using a systematic approach based on abdominopelvic quadrants and key anatomic features. CT imaging will continue to be commonly used to evaluate abdominal pain. Anatomical landmarks and characteristic CT fi…

Internal hernia after an open gastrectomy: an unusual treatment approach and a systematic review of the literature.

Internal hernia (IH) is a serious complication that can occur after both laparoscopic and open surgery for the treatment of gastric cancer; the transverse colon and mesocolon, act as a natural partition between stomach and the small intestine and, once any type of gastrojejunal anastomosis is constructed, a potential space for internal hernia is created. We present the case of a 68-year-old patient diagnosed with intestinal ischemia due to an IH in the site of the jejunojejunostomy after an open gastrectomy for gastric cancer, treated with negative wound pressure therapy (NWPT) on open abdomen…

A case of small bowel obstruction due to infection with Bolbosoma sp. (Acanthocephala: Polymorphidae).

A case of small bowel obstruction caused by Bolbosoma sp. infection is reported. A 27-year-old woman admitted with abdominal pain was diagnosed as small bowel obstruction. Laparoscopic surgery revealed induration in jejunum at ca. 120 cm distal to the ligament of Treiz, attributed to a band connecting the serosa to the ascending mesocolon. Resected band contained an acanthocephalan accompanying foreign body reaction with abscess formation. The parasite belonged to the genus Bolbosoma, of which identification was made by DNA sequence analysis. This is the eighth case of Bolbosoma infection in h…

Spontaneous colonic mesenteric hemorrhage: report of an unusual case of abdominal apoplexy.

We describe a case of idiopathic abdominal apoplexy secondary to a spontaneous mesocolic bleed. Preoperative computed tomography of the abdomen suggested a ruptured colonic mass at the splenic flexure. The diagnosis was made when exploratory laparotomy revealed hemoperitoneum and an extensive hematoma in the transverse mesocolon, with no apparent source. Idiopathic abdominal apoplexy is rare, the reported mortality has historically been high, and a short hospital stay and rapid, complete recovery are uncommon, although outcomes may be improving in recent years. We present such a case and exami…

[Carcinoma of the splenic flexure. Nosological and therapeutic contribution].

Twenty-one patients undergoing colectomy for carcinoma of the splenic flexure from November 1996 throughout October 1993 were studied retrospectively. Clinical and prognostic features were compared with those of other colon cancers treated in the same period at the same Institution. No differences in symptoms at onset and stage distribution were found in the two groups. The complex removal of the cancer tissue was achieved with the resection of the major part of the transverse colon, splenic flexure, descending colon and its mesocolon. The splenic flexure cancer did not show a worse prognosis …

Anatomic correlations in radiogallium imaging of the peritoneum and retroperitoneum.

Radiogallium (67Ga) imaging of the abdomen and pelvis has been useful not only in detecting inflammations in these regions, but in pointing out their precise anatomic localization. Once the anatomic site is determined, it is often possible to infer the source of origin of the problem (such as ruptured viscus or pancreatitis). Interpretation of the images depends on recognition of patterns that define known anatomic boundaries such as the transverse mesocolon, root of the small mesentery, perirenal space, and pararenal space, or else show diffuse peritoneal uptake. The anatomic patterns may hav…
