A 40-year-old woman involved in a motor vehicle accident presented to the emergency room with signs and symptoms of acute left hand ischemia. The mechanism of injury consisted of a severe crushing component limited to a linear zone across the left hand metacarpals. After patient management, and compartment decompressions and stabilization of fractures, an ischemic state persisted that failed to improve after extensive vessel exploration and bathing in vasodilating solutions. Only after extensive peripheral sympathectomy was appropriate flow re-established to the hand. The need to employ periar

Congenital synostosis between the fourth and fifth metacarpals is a rare congenital anomaly. We present a case of type IIIB metacarpal synostosis according to the Buck Gramcko's classification with the camptodactyly of the little finger. It was treated successfully with the phalangeal angulation osteotomy and soft tissue release procedures.

Carpal coalition may involve more than two carpal bones which is more common when coalition occurs as part of a syndrome of congenital malformations. We describe an unusual case of congenital complete entire carpal coalition with massive carpometacarpal coalition except the first carpometacarpal joint and abnormal distal radioulnar joint bilaterally. In addition, radiocarpal joint surfaces were slightly irregular but compatible. There were abnormal distal radioulnar joints and ulnar styloids articulated with the ulnar side of the lunate of the both wrists.

Injuries to the carpometacarpal (CMC) joints are uncommon injuries and isolated dislocation of these joints occurs very infrequently. The 4th and 5th CMC joints are predominantly involved in CMC joint injuries. The detection of CMC joint injuries requires a high index of suspicion and often depends on special radiographic views. This article present a case report of a mid-shaft fracture of the 4th metacarpal which was associated with an isolated dislocation of the 5th CMC joint. An overview of the incidence, mechanism of injury, anatomy, clinical and radiological assessment of injuries involvi

Both experimental and clinical studies showed that muscle flap transposition is a reliable tool in treating chronic infections of the bone. The major advantage of local muscle flaps is the treatment with well perfused autogenous tissue without using any implants. The primary closure of the operation site and the cure of infection within four weeks allow a short immobilisation time. Nevertheless, the loss of muscle function with a possible functional and aesthetic defect in the donor site has to be accepted. The first dorsal interosseus muscle and the abductor digiti minimi muscle have been use

We report on a patient of Guatemalan descent whose physical and radiological findings are consistent with a diagnosis of spondyloepimetaphyseal dysplasia with joint laxity (SEMDJL). This is a rare, autosomal recessive skeletal dysplasia with short limbs, severe scoliosis, high dorsal kyphoscoliosis, and joint hypermobility. Most described patients with SEMDJL are from the Afrikaans-speaking communities of South Africa. Patients with SEMDJL have an oval face, prominent eyes, and blue sclerae. Our patient's height and weight were below the fifth centile. She had prominent eyes with blue sclerae,

Incomplete fracture of the distal palmar aspect of the third metacarpal bone was diagnosed in 5 horses. Mild to moderate lameness and positive response to flexion of the metacarpophalangeal joint were evident in all horses. Fractures were not detectable in 4 horses when the routine 30 degrees proximodorsal-palmarodistal radiographic view was used. Visualization of the fractures in these 4 horses required imaging of the palmar condylar surface by use of a dorsopalmar view of the flexed joint or a 125 degrees dorsoproximal-dorsodistal view. The dorsopalmar view of the flexed joint was used in a

Isolated dorsal dislocation and rotatory dislocation of the second carpometacarpal joint is rare. A case of rotatory dislocation of this joint is presented, where reduction was blocked by entrapment of the extensor carpi radialis longus tendon in the joint. A roentgenographic technique for evaluating dislocation of carpometacarpal joints is also described.

28 patients with definite, probable or phenotypic Marfan syndrome underwent manual and computerised measurement of their metacarpal indices. Two methods were used from among the several variations quoted in the literature, one involving the average length/width of metacarpals 2-5 (method I), and the other dividing the sum of the lengths by the sum of the widths of these metacarpals (method II). The two methods are not mathematically identical, and a statistically significant difference was noted between them. Only 50% of the children in each group had elevated metacarpal indices, making it a t

Metacarpal fractures are common injuries. Most can be treated successfully by closed reduction and cast or splint immobilization. Unstable fractures, however, require internal fixation. Many such techniques are discussed. Metacarpal dislocations are more difficult to diagnose and treat than are metacarpal fractures. Careful attention to detail will help ensure a successful result.

Bilateral, midshaft metacarpal osteotomies were performed in 11 sheep and bilateral, midshaft radial osteotomies were performed in 7 sheep. The lesions were repaired with bone plates. One of each pair of plates was luted with polymethylmethacrylate and all screws were tightened uniformly with a torque screwdriver. Sheep were allowed unrestricted exercise after surgery. At 8 weeks, 10 of 11 sheep with metacarpal osteotomies were sound and both osteotomies were healing. Seven were lame on the limb with the unluted plate during the first 3 weeks; 4 were never lame on either limb. The screws of th

Having performed 100 anatomical dissections we found that in the first dorsal compartment of the wrist besides other tendons there are one or two tendons belonging to a musculo-tendinous unit, not yet described, inserting in the trapezium and acting almost together with the other units going to the first metacarpal. This is present in 71% of cases. Its function is to abduct the carpus. Therefore we suggest to call this unit the "abductor carpi" muscle. If it is missing, which is "abnormal", instability and arthritis of the trapezio-metacarpal joint occur in a higher percentage because the othe

After thumb amputation distal to the MCP-1 joint, good results can be achieved with lengthening of the first metacarpal bone by an external fixateur if a correct technique is applied. In all our patients the lengthened thumb was mechanically stable and sensitive, thus improving the patient's ability to use the hand to grasp. Another advantage of this procedure is the relative simplicity of the method. Furthermore, the original nerve and vascular supply can be preserved.

Eleven horses with acute or chronic incomplete cortical fractures of the left or right third metacarpal bone (McIII) were treated with surgical puncture (osteostixis). The fractures were diagnosed by physical examination and radiography. Four to eight holes, 2.7 or 3.5 mm in diameter, were drilled in the fractured bone. Radiographically, the fractures were healed by month 3, and the drill holes were inapparent by month 7. Nine horses (82%) returned to race competition, and two horses were retired, one the result of a surgical complication. The mean time between surgery and the first race was 9

Five horses with long incomplete longitudinal fractures of the third metacarpal bone and three horses with similar fractures of the third metatarsal bone were examined. In three of the metacarpal fractures a long incomplete fissure extended proximomedially, in association with the more common lateral condylar fracture of the third metacarpal bone, and in the other two cases the fracture originated from the medial aspect of the distal articular surface. In the three horses with fractures of the third metatarsal bone the fractures had a consistent spiral configuration.

We report on a 13-yr-old boy with acrodysostosis, a review of 30 cases in the literature, and metacarpophalangeal pattern profile (MCPP) analysis. The prominent manifestations (present in greater than 75% of cases) of this condition include nasal and maxillary hypoplasia, peripheral dysostosis, first ray hyperplasia of the foot, acromesomelic brachymelia, decreased interpedicular distance, advanced skeletal maturation and mental retardation. Results of chromosome studies have been normal. An autosomal dominant inheritance pattern was reported in two families. Maternal and paternal ages were 2
