Interaction of carcinogenic metals with tissue and body fluids.

In addition to cobalt, metallic nickel, cadmium and other metals (e.g. zinc and copper) dissolve when incubated with horse serum at 37°. The dissolving property of copper in serum resembles that of cobalt, its solubility being increased greatly in the presence of oxygen, whereas the solubilities of cadmium, zinc and most preparations of nickel are the same aerobically and anaerobically. In all of these "metal-sera" the cations are bound, although in different proportions, both by proteins and by small diffusible molecules.Although Co(2+) ions and cobalt-serum cause limited catalytic oxidation …

Separation of uranium from other metals by partition chromatography.

Uranium(VI) can be separated quantitatively from most other metal ions by partition chromatography on a silica-gel column. The column is treated with aqueous 6M nitric acid; after sorption of the sample, uranium(VI) is selectively and rapidly eluted by methyl isobutyl ketone. In addition to the separation of macro quantities of metal ions, the method has been used successfully for the isolation of trace amounts of metal ions from uranium(VI).

Metal transformation as a strategy for bacterial detoxification of heavy metals.

Microorganisms can modify the chemical and physical characters of metals leading to an alteration in their speciation, mobility, and toxicity. Aqueous heavy metals solutions (Hg, Cd, Pb, Ag, Cu, and Zn) were treated with the volatile metabolic products (VMPs) of Escherichia coli Z3 for 24 h using aerobic bioreactor. The effect of the metals treated with VMPs in comparison to the untreated metals on the growth of E. coli S1 and Staphylococcus aureus S2 (local isolates) was examined. Moreover, the toxic properties of the treated and untreated metals were monitored using minimum inhibitory concen…

Is Metal-On-Metal Total Hip Arthroplasty Associated With Neurotoxicity?

BACKGROUND: Isolated case reports in the literature describe systemic neurologic side effects associated with metal-on-metal (MOM) bearing surfaces, yet the incidence of these effects have not been evaluated beyond individual cases. The purpose of this study was to compare new diagnoses of these side effects described in isolated cases in large patient cohorts of MOM vs metal on polyethylene (MOP). METHODS: We queried the entire Medicare database from 2005 to 2012. Total hip arthroplasty (THA) and bearing surface were determined using International Classification of Diseases, 9th revision pro

Metal-on-metal hip resurfacing in osteonecrosis of the femoral head.

BACKGROUND: The treatment of end-stage osteonecrosis of the femoral head remains a challenge to the orthopaedic surgeon. Historically, total hip arthroplasty for this condition has been associated with poor rates of survival and function when compared with total hip arthroplasty for the treatment of osteoarthritis. The purpose of this study was to determine the medium-term clinical and radiographic results of metal-on-metal hip resurfacing arthroplasty in patients with end-stage osteonecrosis of the femoral head. METHODS: From June 1994 to March 2004, a consecutive single-surgeon series of se

Dual Mobility for Monoblock Metal-on-Metal Revision-Is It Safe?

BACKGROUND: Revision of monoblock metal-on-metal (MoM) total hip arthroplasty (THA) is associated with high complication rates. Limited revision by conversion to a dual mobility (DM) without acetabular component extraction may mitigate these complications. However, the concern for polyethylene wear and osteolysis remains unsettled. This study investigates the results of DM conversion of monoblock MoM THA compared to formal acetabular revision. METHODS: One hundred forty-three revisions of monoblock MoM THA were reviewed. Twenty-nine were revisions to a DM construct, and 114 were complete revi

A Misdiagnosed Complication of a Metal-on-Metal Total Hip Arthroplasty.

INTRODUCTION: The enterobacterial genus Yersinia includes a number of human pathogens. Large-diameter, metal-on-metal prostheses are no longer used because of their high failure rate. Here, we describe the first case of Yersinia enterocolitica infection of a metal-on-metal total hip arthroplasty. CLINICAL EXAMINATION: A metal-on-metal prosthesis failed ten years after implantation. After surgical revision, bacteriological testing revealed the presence of a pathogenic strain of Yersinia enterocolitica. Combination antibiotic therapy resulted in a favorable clinical outcome. DISCUSSION: Three

Metallosis: A diagnosis not only in patients with metal-on-metal prostheses.

Although the real actual incidence of metallosis is unknown, it is described as a rare diagnosis with a 5% estimated incidence in the hip prosthetic replacements. The adoption of non-metallic articular prosthetic devices, made of polyethylene and ceramic, is the main reason to the diminishing number of reported cases. We present a case of metallosis with a clinical systemic presentation in a patient with a non-metallic hip prosthesis, which occurred due to a fracture of the acetabular liner component, leading to abnormal metal-metal contact. The metallic debris leads to a massive local and sys

Modes of failure in metal-on-metal total hip arthroplasty.

Use of large-head metal-on-metal (MoM) bearing surfaces in total hip arthroplasty (THA) has created new and unique modes of failure for this type of articulation. These unique modes are in addition to the traditional modes of failure seen in conventional THA, which include instability, osteolysis, infection, iliopsoas tendinitis, aseptic loosening, and periprosthetic fracture. Ion levels and cross-sectional imaging are helpful when evaluating a MoM patient in the identification of adverse local tissue reactions. Unique modes of failure in MoM THA include tissue necrosis, metallosis-induced ost

[Cobalt poisoning due to metal-on-metal hip implants].

Since 2011, cobalt and chromium blood levels are measured in patients with a metal-on-metal hip implant (MoM prosthesis). In this article we discuss the health risks that are related to chronically elevated blood cobalt concentrations induced by abnormal wear and corrosion of the MoM prosthesis. Only a few patients who have systemic symptoms of poisoning, besides local symptoms around the failing MoM prosthesis, have been described in the literature. Toxic blood cobalt concentrations may be accompanied by hypothyroidism, polyneuropathy, impairment of cranial nerves II and VIII and cardiomyopat

MRI findings in painful metal-on-metal hip arthroplasty.

OBJECTIVE: The objective of our study was to compare the frequency of osseous and soft-tissue abnormalities in patients presenting with hip pain after resurfacing arthroplasty and after total hip arthroplasty (THA), correlate the MRI findings with histologic results, and determine which MRI findings are predictive of aseptic lymphocytic vasculitis-associated lesions. MATERIALS AND METHODS: The MRI examinations of patients with metal-on-metal hip prostheses placed at resurfacing arthroplasty (n=31) or THA (n=29) were reviewed for osteolysis, synovitis, extracapsular disease, synovial pattern,

Metal-on-metal bearings, inflammatory pseudotumours and their neurological manifestations.

Metal-on-metal bearings have become more popular for hip arthroplasty in younger patients in the last 15 years. We present a review of the neurological consequences relating to such bearing surfaces illustrated by an appropriate case report. The mechanisms of increased metal ion production and pseudotumour formation are discussed. If unexplained neurology, hip/groin pain or a localised mass around the hip are present with a metal bearing hip then urgent referral to a specialist orthopaedic surgeon is warranted for consideration of exchange arthroplasty.

Perivascular lymphocytic infiltration is not limited to metal-on-metal bearings.

BACKGROUND: Perivascular lymphocytic infiltration (PVLI) suggests an adaptive immune response. Metal hypersensitivity after THA is presumed associated with idiopathic pain and aseptic loosening, but its incidence and relationship to metallic wear leading to revision are unclear as are its presence and relevance in non-metal-on-metal arthroplasty. QUESTIONS/PURPOSES: We compared (1) incidence and severity of PVLI in failed hip metal-on-metal (MoM) to non-MoM implants and TKA; (2) PVLI in MoM and non-MoM hip arthroplasty based on reason for revision; and (3) PVLI grade to diffuse lymphocytic in

Tissue reaction to metal on metal total hip prostheses.

The periprosthetic tissue reaction to polyethylene wear debris in metal on polyethylene total hip replacements is strongly implicated as the cause of osteolysis. This has led to a renewed interest in metal on metal total hip replacements. However, little is known about the role of wear debris in failures of these prostheses. Capsular and interface tissues from 9 long and short term metal on metal total hip replacement retrievals were studied to assess the tissue reaction around these prostheses. As compared with metal on polyethylene cases, the extent of the granulomatous inflammatory reaction

Bioremediation of organic and metal contaminants with dissimilatory metal reduction.

Dissimilatory metal reduction has the potential to be a helpful mechanism for both intrinsic and engineered bioremediation of contaminated environments. Dissimilatory Fe(III) reduction is an important intrinsic process for removing organic contaminants from aquifers contaminated with petroleum or landfill leachate. Stimulation of microbial Fe(III) reduction can enhance the degradation of organic contaminants in ground water. Dissimilatory reduction of uranium, selenium, chromium, technetium, and possibly other metals, can convert soluble metal species to insoluble forms that can readily be rem…

Technique to disguise the metal margin of the metal/ceramic crown.

When it is necessary to place casting margins subgingivally, it is important that they not violate the biologic width. A technique utilizing nonmedicated retraction cord during preparation is described which has several advantages. It results in bloodless, trauma-free tooth preparation and concealment of the metal margin without biologic width impingement or gingival recession. A resilient, unattached gingival margin free of inflammation is critical for the success of this technique. The physical and chemical properties of healthy gingival collagen and ground substance are briefly discussed al
