Speech based transmission index for all: An intelligibility metric for variable hearing ability.

A method to measure the speech intelligibility in public address systems for normal hearing and hearing impaired persons is presented. The proposed metric is an extension of the speech based Speech Transmission Index to account for accurate perceptual masking and variable hearing ability: The sound excitation pattern generated at the ear is accurately computed using an auditory filter model, and its shapes depend on frequency, sound level, and hearing impairment. This extension yields a better prediction of the intensity of auditory masking which is used to rectify the modulation transfer func

Quantitative assessment of CBCT image quality metrics and their translation to clinical Art workflow performance in breast cancer patients.

BACKGROUND: Online adaptive radiotherapy (ART) relies on cone-beam computed tomography (CBCT) imaging for daily treatment adaptation, but CBCT's physical limitations can compromise contour delineation and dose calculation accuracy. The relationship between image quality metrics and clinical ART workflow accuracy remains unclear. PURPOSE: To investigate the clinical impact of CBCT image quality on ART workflow accuracy in breast cancer patients by comparing contour agreement and dose calculation accuracy between Halcyon and TrueBeam systems, establishing how quantitative image quality metrics

Acoustic Frequency Comb-Based QPSK Demodulation Using Nonlinear MEMS Resonators.

This work demonstrates a microelectrome-chanical system (MEMS)-based phase shift keying (PSK) demodulator that leverages nonlinear resonators. In particular, this work employs an internal-resonating MEMS resonator that generates acoustic frequency combs. The spacing of these combs is dynamically modulated by a primary driving signal and an auxiliary signal that carries the phase-encoded information. The discrete variations in comb spacing serve as the demodulation metric, enabling accurate retrieval of the original phase-encoded data. This approach offers a feedforward demodulation structure,

Nuclear Packing Sets Fluidity Along the Epithelial to Mesenchymal Spectrum.

Investigations of jamming in cancer have produced numerous phase diagrams intending to map fluidity across the epithelial-to-mesenchymal transition (EMT). Here, we use coalescence of homotypic and heterotypic multicellular spheroids to examine and carefully probe these phase spaces. Small changes in cellular EMT status result in full traversal of the solid-to-fluid continuum. We propose that stiff nuclei impede cell motion and spheroid coalescence and find that by softening nuclei, fluidization of an otherwise solid-like system occurs. Changes in fluidity during coalescence is fully captured b

Insole-Derived Plantar Pressure Variability Reveals Dual-Task Gait Differences in Early-Stage Parkinson's Disease.

OBJECTIVE: Gait disturbances in Parkinson's disease (PD) indicate impaired motor automaticity, particularly under cognitively demanding conditions. Although spatiotemporal parameters are commonly used to assess dual-task cost (DTC), their sensitivity in PD is limited. This study aimed to characterize the plantar pressure alterations during dual-task walking using a sensor-based insole system and compare them with conventional gait metrics. METHOD: We performed an on-site validation of the insole-derived parameters against a 3D motion analysis system in healthy adults. Subsequently, we conduct

Oscillometry Measures the Response to Acute Asthma Therapy in the Pediatric Emergency Department.

Rationale: Oscillometry is a feasible and safe method to measure pulmonary function in children with asthma exacerbations in the emergency department (ED), but its utility to measure respiratory impedance as an objective marker of response to initial acute asthma treatments is unknown. Objectives: We sought to determine the associations between respiratory impedance-derived metrics and asthma exacerbation severity and treatment response in the pediatric ED. Methods: We conducted a prospective study of children, ages 4-18 years, who presented to a tertiary-care pediatric ED for asthma exacerbat

Association between continuous glucose monitoring metrics and cardiovascular autonomic neuropathy in diabetic patients: a systematic review.

Cardiovascular autonomic neuropathy (CAN) is a serious and prevalent complication of diabetes, linked to significant morbidity and mortality. Continuous glucose monitoring systems (CGM) provide a comprehensive and continuous glucose profile, enabling precise assessment of glycemic variability, hypoglycemia, and other key glucose metrics. Despite the increasing use of CGM, the relationship between CGM-derived metrics and CAN risk has yet to be rigorously evaluated. A systematic search of PubMed, Cochrane Library, Web of Science, Medline, and Embase was conducted up to 30 September 2024. Ventilator-Assisted Inspiratory and Expiratory Breath-Hold Thoracic Computed Tomographic Scans Can Detect Dynamic and Static Airway Collapse in Dogs with Limited Agreement with Tracheobronchoscopy.

Airway collapse (AC) in dogs includes a tracheal collapse, mainstem and lobar bronchial collapse, and bronchomalacia (i.e., segmental/subsegmental bronchial collapse). The clinical presentation of AC may overlap with non-collapsible airway disease (NCAD) or another non-lower airway respiratory disease (NLARD). This study determined whether paired inspiratory (I)/expiratory (E)-breath-hold computed tomography (I/E-BH CT) can detect a static and dynamic AC in dogs with spontaneous respiratory disease and it compared the CT-derived metrics of the AC to the tracheobronchoscopy metrics. The CT-acqu

A Single-Number Value Metric Integrating Quality and Cost in Outpatient Physical Therapy.

IMPORTANCE: As health care transitions toward value-based care, outpatient physical therapy lacks a practical metric that objectively integrates quality of care and cost into a single measure of clinical value. OBJECTIVE: The objective was to develop a single-number value metric (VM) integrating clinical effectiveness, patient experience, and cost efficiency in outpatient physical therapy and to determine its concordance with blinded manager assessments of therapist value. DESIGN: This was a retrospective observational cohort study. SETTING: The study was conducted at 8 outpatient physical

A novel methodology for localizing pallidal deep brain stimulation leads.

INTRODUCTION: Positioning of deep brain stimulation (DBS) leads is paramount for optimizing therapeutic efficacy in Parkinson's disease (PD) and dystonia. Quantitative determination of lead position remains essential; however, current atlas-based targeting and stereotactic coordinate methods, while informative, limit patient specificity afforded by high-resolution reconstruction and introduce subjective variability. METHODS: We developed a novel pipeline in Unity™ to ingest 7T MRI-based reconstructions of DBS leads within the globus pallidus internus (GPi). Using anatomical landmarks and stru

A confidence-based, artificial intelligence pathology model for diagnosis of intrahepatic cholangiocarcinoma.

BACKGROUND: Intrahepatic cholangiocarcinoma (ICCA) is a rare but highly lethal adenocarcinoma arising within the hepatic parenchyma. Diagnosis presents a significant clinical challenge as the histological features of ICCA substantially overlap with those of metastatic liver cancers. This diagnostic ambiguity often necessitates extensive and costly exclusionary investigations, such as upper and lower gastrointestinal endoscopy, to rule out an occult primary site. Consequently, this process results in treatment delays and an increased financial burden on health care systems. PATIENTS AND METHOD

The Ratio of Immunogenicity: A quantitative metric to identify highly immunogenic protein antigens to improve tissue-engineered biomaterials.

The ability to a-priori identify antigenic components responsible for driving recipient graft-specific adaptive immune destruction of tissue-engineered biomaterials is critical for their safe clinical translation and long-term function. However, despite our in-depth understanding of cellular responses toward antigenic stimuli, the question of whether individual antigens are equally stimulatory to the immune system remains largely unanswered. This lack of knowledge limits our capacity to rationally design immunologically acceptable biomaterials. Previously published decellularization/antigen re

A Dual-Polarized Narrow-Beam Antenna for Microwave Interrogation of Back-Surface Flaws in Polyethylene Slabs.

In view of the advantage of cross-polarized inspection (CrPI) in microwave nondestructive testing (MNT), in this paper a dual-polarized narrow-beam antenna as the pivotal reflectometric sensor is systematically designed and realized particularly for enhancement of detection and imaging of subsurface defects in dielectric structures. The antenna is equipped with a compact asymmetric waveguide orthomode transducer, with Teflon used as the internal filling material, in an effort to reduce its size and narrow the beamwidth. The internal dimensions of the realized dual-polarized narrow-beam antenna

A metric for quantifying adaptation to successive disruptions.

Complex systems face a myriad of stresses and shocks. The ability of complex systems to deal with such shocks is often expressed in terms of the system's resilience. Although resilience is conventionally conceptualised as recovery from a single shock or disruption, in reality, systems often face a sequence of successive shocks, as is the case for earthquake aftershocks or epidemic waves. A system's ability to handle such a sequence of disruptions depends on its capacity to learn from preceding events. To capture adaptation and learning as transformative aspects of resilience, new approaches to

Normalized internalization predicts potency of carcinoembryonic antigen-related cell adhesion molecule 6-targeting antibody-drug conjugates.

Antibody-drug conjugate (ADC) efficacy in solid tumors frequently fails to track with target abundance or binding affinity, reflecting delivery bottlenecks that remain difficult to quantify during lead selection. Here, we develop a discovery-to-function workflow for carcinoembryonic antigen-related cell adhesion molecule 6 (CEACAM6), a glycosylphosphatidylinositol (GPI)-anchored antigen with context-dependent trafficking and show that intracellular delivery-related parameters can better organize ADC potency than antigen abundance or binding affinity alone in the CEACAM6 model system. Evaluation of periodontal status in gastritis patients with clinical and microbiological parameters.

BACKGROUND: Gastritis is characterized by alterations in the gastric microbiota. This dysbiosis in the stomach may affect immune system regulation as well as influence the oral microbiota. Periodontal disease results from dysbiosis of subgingival microbial communities that cause inflammatory responses in periodontal tissues. The aim of our study is to examine the oral flora profile and dysbiosis status of gastritis patients along with their periodontal status. METHODS: A total of 30 patients were included in our study, divided into two groups 15 patients diagnosed with gastritis and 15 system
