The latent difference score model is a viable alternative to arithmetic difference score-based anchor-driven minimal important change calculation.

BACKGROUND: Traditional MIC estimates contain measurement error from PROMs. The LDSM offers a viable alternative to estimate MIC controlling for measurement error. We also propose a conceptual framework for considering the multiple steps necessary to calculate anchor-based MICs. METHODOLOGY: Data from KASTPain, a no-effect multi-center randomized clinical trial of 364 participants with knee osteoarthritis and who underwent knee arthroplasty, were used to illustrate the LDSM to estimate MIC. Based on a structural equation modeling framework, we used LDSMs to estimate MICs of three commonly use

Health State Values Should Not Be Used as Minimal Important Differences.

This article critically examines the application of minimal important differences (MIDs) to health state values or utilities. The concept of MIDs aims to guide clinical and research decisions by identifying important changes in health-related quality-of-life (HRQoL) indicators. However, this cannot be used without additional information not contained within the indicator itself, so that the MID cannot be regarded as a property of the indicator. First, MIDs defined at the individual patient level cannot be meaningfully aggregated for groups without additional context. Second, any improvement in

Enhancing Interpretability of Patient-Reported Outcomes Measurement Information System (PROMIS) and Related Measures in Rehabilitation Populations: A Systematic Review of Clinical and Research Applications.

OBJECTIVE: To evaluate and synthesize interpretability metrics, including minimal important change (MIC), minimal important difference (MID), and minimal detectable change (MDC), across Patient-Reported Outcomes Measurement Information System (PROMIS) and related systems (Quality of Life in Neurological Disorders [Neuro-QoL], Quality of Life in Traumatic Brain Injury [TBI-QoL], Quality of Life in Spinal Cord Injury [SCI-QoL]) in rehabilitation populations. Guide For Selection of Appropriate Patient-Reported Outcome Measures and Clinically Significant Outcome Thresholds in Knee Arthroscopy.

Patient-reported outcome measures (PROMs) are vital tools to evaluate patient health status. Although a plethora of validated tools are avai … rch settings. Notably, statistical significance does not always translate to clinical significance. Minimal clinically important difference, patient-acceptable symptomatic state, and substantial clinical benefit are clinically significant outcomes that represent the spectrum of meaningful clinical impact for a patient. However, these metrics are population- and study-specific and should …

Minimal Clinically Important Change of Movement Pain in Musculoskeletal Pain Conditions.

Movement pain, which is distinct from resting pain, is frequently reported by individuals with musculoskeletal pain. There is growing interest in measuring movement pain as a primary outcome in clinical trials, but no minimally clinically important change (MCIC) has been established, limiting interpretations. We analyzed data from 315 participants who participated in previous clinical trials (65 with chronic Achilles tendinopathy; 250 with fibromyalgia) to establish an MCIC for movement pain. A composite movement pain score was defined as the average pain (Numeric Rating Scale: 0-10) during 2

Development of prediction models for clinically meaningful improvement in PROMIS scores after lumbar decompression.

BACKGROUND: The ability to preoperatively predict which patients will achieve a minimal clinically important difference (MCID) after lumbar spine decompression surgery can help determine the appropriateness and timing of surgery. Patient-Reported Outcome Measurement Information System (PROMIS) scores are an increasingly popular outcome instrument. PURPOSE: The purpose of this study was to develop algorithms predictive of achieving MCID after primary lumbar decompression surgery. PATIENT SAMPLE: This was a retrospective study at two academic medical centers and three community medical centers…

Interpretation of statistical findings in randomised trials: a survey of statisticians using thematic analysis of open-ended questions.

BACKGROUND: Dichotomisation of statistical significance, rather than interpretation of effect sizes supported by confidence intervals, is a long-standing problem. METHODS: We distributed an online survey to clinical trial statisticians across the UK, Australia and Canada asking about their experiences, perspectives and practices with respect to interpretation of statistical findings from randomised trials. We report a descriptive analysis of the closed-ended questions and a thematic analysis of the open-ended questions. RESULTS: We obtained 101 responses across a broad range of career stages

What Is the Clinical Benefit of Common Orthopaedic Procedures as Assessed by the PROMIS Versus Other Validated Outcomes Tools?

BACKGROUND: Patient-reported outcome measures (PROMs), including the Patient-reported Outcomes Measurement Information System (PROMIS), are increasingly used to measure healthcare value. The minimum clinically important difference (MCID) is a metric that helps clinicians determine whether a statistically detectable improvement in a PROM after surgical care is likely to be large enough to be important to a patient or to justify an intervention that carries risk and cost. There are two major categories of MCID calculation methods, anchor-based and distribution-based. This variability, coupled wi

How Do PROMIS Scores Correspond to Common Physical Abilities?

BACKGROUND: The Patient-Report Outcomes Measurement Information System (PROMIS) is increasingly used as a general-purpose tool for measuring orthopaedic surgery outcomes. This set of questionnaires is efficient, precise, and correlates well with specialty-specific measures, but impactful implementation of patient-specific data, especially at the point of care, remains a challenge. Although clinicians may have substantial experience with established patient-reported outcome measures in their fields, PROMIS is relatively new, and the real-life meaning of PROMIS numerical summary scores may be un

Determining Clinically Meaningful Improvement in Children and Adolescents with Tourette Syndrome Receiving Pharmacotherapy.

Introduction: Accurate assessment of treatment outcomes in patients with Tourette syndrome (TS) is essential for evidence-based clinical care. The minimally important difference for the Xerostomia Inventory among Sjögren's disease patients.

OBJECTIVE: Until now, the clinically relevant improvement for the Xerostomia Inventory (XI) has not been defined. Therefore, our aim was to determine the Minimally Important Difference (MID) of the XI for improvement in dry-mouth symptoms in SjD patients. METHOD: The study recruited 34 SjD patients who underwent sialendoscopy of major salivary glands and 15 SjD patients in a nonintervention control group. XI scores were assessed at several time points. The MID was determined from the mean difference in XI scores between the groups with and without improvement. RESULTS: In the control group,

Predictors of achieving Neck Disability Index minimum clinically important difference following cervical deformity surgery.

OBJECTIVE: The aim of this study was to determine predictors of the minimum clinically important difference (MCID) in the Neck Disability Index (NDI) following cervical spinal deformity surgery. METHODS: A retrospective review was performed of a prospective, multicenter adult cervical spinal deformity database. All patients had baseline and 1-year NDI scores. Patients met MCID with an improvement of NDI by 7 points between baseline and 1 year, as previously established. Baseline demographics, comorbidities, and both baseline and 1-year spinopelvic parameters were evaluated for statistical sig
