Bacterial flora of the parturient vagina--implications for neonatal sepsis.

A bacteriological study of vaginal swabs taken from 78 parturient women at Lagos University Teaching Hospital (L.U.T.H.) is presented. 26.83% of the vaginal cultures were sterile. The most frequently isolated bacteria were Staphylococcus spp. 21.4%, followed by beta hemolytic streptococci 16.65%, of which approximately a half belonged to Lancefield group B. Escherichia coli was isolated in 11.1% of the cases. The implications for neonatal and puerperal sepsis are discussed.

Sepsis in neonates with necrotizing enterocolitis: outcomes and predictive model.

Sepsis after neonatal necrotizing enterocolitis (NEC) onset was associated with substantially adverse outcomes. However, effective tools for predicting sepsis risk in this special population remain lacking. This study aimed to investigate the influence of sepsis after NEC onset on the outcomes and develop a prediction model for early identification of sepsis. Retrospective review of 530 patients diagnosed with NEC was reviewed with clinical data. In this study, the overall proportion of sepsis was 56.2% (298/530). The rates of surgical intervention (63.09% vs. 18.10%) and mortality (17.11% vs.

Hyperthermia in Term Neonates: Profile and Factors Associated With Sepsis.

Aim of the study The aim of this study was to distinguish between hyperthermia (HT) due to sepsis and environmental hyperthermia (EH) or dehydration fever (DF). Hyperthermia (HT) in neonates may be due to many reasons, common among them being environmental hyperthermia (EH), dehydration fever (DF), and sepsis. It is important to make a provisional diagnosis at presentation for better outcomes and prevent the indiscriminate use of antimicrobials. We therefore decided to study the profile of term neonates with HT and analyze which clinical or laboratory parameters are associated with the diagnos

The Sensitivity and Specificity of Procalcitonin in Diagnosing Bacterial Sepsis in Neonates.

CONTEXT AND OBJECTIVES: Neonatal sepsis accounts for 15% of all neonatal deaths. Early detection enables prompt administration of antibiotic treatment, reducing morbidity and mortality. This study aims to review the sensitivity and specificity of procalcitonin in diagnosing microbiologically-proven sepsis in neonates to determine the optimal procalcitonin cut-off value for use in clinical practice. DATA SOURCES, STUDY SELECTION, AND DATA EXTRACTION: Medline, EMBASE and PubMed were searched on 3 May 2023 for original studies in symptomatic neonates in whom both blood culture and procalcitonin

Predictors of Death in Patients with Neonatal Sepsis in a Peruvian Hospital.

Reducing neonatal mortality is a global challenge. This study’s objective was to determine the predictors of mortality in patients with neonatal sepsis. The study was a retrospective cohort study in a Peruvian hospital from January 2014 to April 2022. Neonates diagnosed with sepsis were included. To find predictors of mortality, we used Cox proportional regression models. We evaluated 288 neonates with sepsis; the median birth weight and hospitalization time were 3270 g and seven days, respectively. During follow-up, 18.4% did not survive, and the most common complications were jaundice (35.42

Role of guidelines on length of therapy in chorioamnionitis and neonatal sepsis.

BACKGROUND AND OBJECTIVE: Chorioamnionitis (CAM) is a major risk factor for neonatal sepsis. At our institution, neonates exposed to CAM and intrapartum antibiotics are treated with prolonged antimicrobial therapy if laboratory values are abnormal despite a sterile blood culture. Recently, the Committee on the Fetus and Newborn (COFN) recommended a similar strategy for treating neonates exposed to CAM. Our objective was to determine the frequency of abnormal laboratory parameters in term and late-preterm neonates exposed to CAM and evaluate the implication of recent COFN guidelines. METHODS:

The effect of topical ointment on neonatal sepsis in preterm infants.

OBJECTIVE: To investigate the effects of topical ointment therapy on neonatal sepsis in premature infants. METHODS: A total of 197 premature infants≤34 weeks gestation were randomized to receive topical ointment (Aquaphor Original Emollient) or routine skin care group. Skin cultures were obtained on 3th, 7th and 14th day and blood cultures were obtained if sepsis was suspected clinically. Data included the maternal and neonatal characteristics, factors affecting the risk of sepsis and neonatal outcomes of both groups were collected. RESULTS: There were no significant differences in terms of

Acute osteomyelitis as cause of late sepsis in a nigerian neonate.

Neonatal osteomyelitis (NO) is said to be rare and uncommonly reported in the sub-region. Although this may be true, either because the cases are under-diagnosed, unreported or under-reported. The diagnosis is clinical and supported with blood culture or the aspiration of pus with radiological findings. We report a case of a neonate with acute osteomyelitis of the right femur, diagnosed at 10(th) day of life, which was confirmed with blood culture and radiogram findings of the right upper thigh of the baby. To the best of our knowledge, this is the first documented reported case of NO from the

Late-onset Leclercia adecarboxylata sepsis in a premature neonate.

The epidemiology, etiology and outcome of neonatal sepsis are changing over time. While monitoring longitudinal trends in neonatal sepsis in our institution, we encountered a case of late-onset neonatal sepsis due to Leclercia adecarboxylata. A Gram-negative rod previously not encountered in the clinical setting, L. adecarboxylata has recently emerged as a human pathogen, primarily in immunosuppressed patients. This report describes the clinical and laboratory features of this case of late-onset L. adecarboxylata sepsis, and reviews significant features of infection associated with this emergi

Umbilical Myiasis Associated with Staphylococcus Aureus Sepsis in a neonate.

Myiasis is an infection of live mammalian tissue by the larvae of dipteran flies and commonly found in the tropics and subtropics. It usually infects domestic and wild animals, but sometimes also affects human. Umbilical cord myiasis in the neonatal period associated with sepsis is a rare occurrence with few reported cases in the literature. In this article, authors report a 7-day-old neonate from a rural area presented with neonatal sepsis due to umbilical myiasis caused by fly larval form of blow fly (Chrysomya megacephala).

Oral lactoferrin for the treatment of sepsis and necrotizing enterocolitis in neonates.

BACKGROUND: Neonatal sepsis and necrotizing enterocolitis (NEC) cause significant neonatal mortality and morbidity in spite of appropriate antibiotic therapy. Enhancing host defence and modulating inflammation by using lactoferrin as an adjunct to antibiotics in the treatment of sepsis and/or NEC may improve clinical outcomes. OBJECTIVES: The primary objective is to assess safety and efficacy of oral lactoferrin as an adjunct to antibiotics in the treatment of neonates with suspected or confirmed sepsis and/or NEC. SEARCH STRATEGY: Relevant trials in any language were searched in July 2011 i

Reliability of serum procalcitonin concentrations for the diagnosis of sepsis in neonates.

The evaluation of tests for neonatal sepsis is important because the infection may present a very serious threat to the baby. Extensive literature exists on single laboratory test or combinations of tests, as well as tests used together with risk factors and/or clinical signs, to diagnose neonatal sepsis. In many instances, the results of the evaluations have been conflicting. It has recently been suggested that serum procalcitonin (PCT) is of value: in the diagnosis of neonatal sepsis, with varying results. This study was designed to determine the reliability of PCT concentrations as a new ma

Granulocyte transfusions for neonates with confirmed or suspected sepsis and neutropaenia.

BACKGROUND: Neonatal sepsis causes significant neonatal mortality and morbidity. Neonates, especially preterm infants, have an immaturity of granulopoeisis and have a limited capacity for progenitor cell proliferation. This results in the frequent occurrence of neutropaenia in septic neonates. Neutropaenic septic neonates have a higher mortality than neonates who are septic but not neutropaenic. Transfusion of granulocytes to septic neutropaenic neonates, therefore, may help reduce mortality and morbidity. OBJECTIVES: The primary objective of this review was to determine the efficacy and safe

Serum cortisol and thyroid hormone levels in neonates with sepsis.

OBJECTIVE: To evaluate the thyroid hormone and cortisol levels in neonates with sepsis in relation to the final outcome. It was hypothesized that the hormonal level could act as some prognostic guideline. METHODS: Forty nine neonates, aged 8- 28 days, diagnosed as neonatal sepsis were selected for the study. Neonates below 8 days of age, 35 weeks of gestation and 2000 g of birth weight were excluded from the study. Twenty FT-AGA neonates beyond day 7 of life served as control for the study. The hormones were estimated by radioimmunoassay. RESULTS: The neonates with sepsis had significantly h

Group B beta-hemolytic streptococcal sepsis in the neonate.

In the past 40 years, the predominant organisms responsible for neonatal sepsis have changed. Whereas Group A betahemolytic streptococci were originally most common and staphylococci later became the major pathogens, Group B beta-hemolytic streptococci have become increasingly important in recent years. Predisposing factors include premature rupture of membranes, prolonged difficult labor, prematurity and congenital anomalies. The causes and clinical features of early-onset infection (in the first week of life) and late-onset infection are different.
