The history of deep brain stimulation.

Deep brain stimulation (DBS) surgery is an established and effective treatment for several movement disorders (tremor, Parkinson's disease, and dystonia), and is under investigation in numerous other neurological and psychiatric disorders. However, the origins and development of this neurofunctional technique are not always well understood and recognized. In this mini-review, we review the history of DBS, highlighting important milestones and the most remarkable protagonists (neurosurgeons, neurologists, and neurophysiologists) who pioneered and fostered this therapy throughout the 20th and ea…

Operations Desert Shield and Desert Storm: neurosurgical experience and transformative legacy for operational medicine.

Operation Desert Storm (ODS) was an astounding success for combat arms and logistical units of the US Military. In contrast, Department of Defense (DOD) medical units struggled to keep pace with combat operations and were fortunate that casualty estimates for a Cold War-era battle failed to materialize. The medical support plan included a large contingent of active-duty and reserve neurosurgeons in anticipation of care requirements for more than 500,000 deploying service members engaged in a large-scale combat operation. Here, the authors review the clinical experience and operational challeng…

Lessons from failure: neurosurgical outreach in Managua, Nicaragua.

With respect to the tremendous deficit in surgical care plaguing developing nations, it is critical that medical outreach models be organized in such a fashion that sustainable advancements can be durably imparted beyond the duration of targeted missions. Using a didactic framework focused on empowering host neurosurgeons with an enhanced surgical skillset, a mission was launched in Managua, Nicaragua, after previous success in Kiev, Ukraine, and Lima, Peru. Unfortunately, the failure to critically assess the internal and external state of affairs of the region's medical center compromised the…

Chronic or recurrent meningitis. Neurosurgical perspectives.

Chronic meningitis is not a commonly encountered clinical problem in the day-to-day patient care responsibilities of neurosurgeons. It is, however, important for the neurosurgeon to be familiar with the clinical problems, diagnostic challenges, and therapeutic options required to establish a definitive diagnosis. This article concentrates on specific entities causing chronic or recurrent meningitis that may require neurosurgical participation.

The role of ambulatory surgical centers in current neurosurgical practice.

BACKGROUND: The recent focus on improving quality and reducing cost within the US healthcare system has increased care being performed in the outpatient setting. The impact on neurosurgeons' practice patterns has not yet been fully elucidated. In addition, how this transition may affect neurosurgery resident training is unclear. To better understand these issues, we surveyed neurosurgeons. METHODS: A 13-question survey was sent to Council of State Neurosurgical Societies email subscribers. The survey focused on training or practice level, location, practice setting, ambulatory surgical center…

Prevalence of carpal tunnel syndrome symptoms among neurosurgical spine surgeons.

BACKGROUND: Neurosurgeons, particularly spine surgeons, have high exposure to vibrations via electric or pneumatic drills and repetitive motion. Although no data exist for the prevalence of carpal tunnel syndrome (CTS) among these surgeons, anecdotal evidence suggests the rate of symptoms is higher than in the general population. METHODS: An anonymous questionnaire was developed to assess demographics, practice patterns, presence of CTS symptoms, and treatment (time off, bracing, medication, injections, surgery). The survey was sent via anonymous email link to members of the AANS/CNS Joint Se…

[Prevalences of Trigeminal Neuralgia and Hemifacial Spasm].

Neurosurgeons must have knowledge about the epidemiology of trigeminal neuralgia and facial spasm. The annual incidence of trigeminal neuralgia is 4.3-28.9 per 100,000 persons, with a prevalence of 76.8 per 100,000 persons, increasing with age. It is more common in women and on the right side, with SCA being the most common causative vessel. The long-term efficacy of MVD for trigeminal neuralgia is 80% with complete resolution of pain and 5.2% with complications, which is safe and highly effective when performed by an expert surgeon. Hemifacial spasm has an annual incidence of 0.78/100,000 wit…

[New Preoperative Simulations for the Future of Hydrocephalus Surgery].

Ventricular puncture is a basic procedure that neurosurgeons learn in the early stages of their careers and is also performed in ventricular drainage and neuroendoscopic surgery. However, few neurosurgeons are confident in their ability to insert and place a ventricular catheter in the optimal position for ventriculoperitoneal(VP)shunting in a single pass. Even experienced neurosurgical consultants confident in difficult microsurgical procedures are uncomfortable with ventricular catheter placement in VP shunting. Moreover, many neurosurgeons believe that they will never perform a ventricular …

Intraventricular Tumors: Surgical Considerations in Lateral and Third Ventricular Tumors.

Management of lateral and third ventricular tumors has been a challenge for neurosurgeons. Advances in imaging and pathology have helped in a better understanding of the treatment options. Technical refinement of microsurgical technique and addition of endoscopy has enabled more radical excision of tumors, when indicated, and added more safety.A proper understanding of the pathology at various ages and treatment options is continuously evolving. Many pediatric tumors are amenable to conservative surgical methods with effective complementary treatments. However, radical surgery is required in m…
