INTRODUCTION: The ability to examine the posterior segment of the eye accurately in patients with opaque media is essential to good surgical care of the cataract patient. In dense cataracts, the posterior segment is inaccessible to direct and indirect ophthalmoscopy and so adequate evaluation of posterior segment to exclude abnormalities become difficult. OBJECTIVE: To describe the phenotype and genotype of patients with early-onset Stargardt disease.
DESIGN: Retrospective cohort study.
PARTICIPANTS: Fifty-one Stargardt patients with age at onset ≤10 years.

CASE REPORT: The ophthalmic examination and results of fluorescein angiography using Retcam II are described in a patient with Incontinentia Pigmenti (IP).
DISCUSSION: Angiography fluorescein is extremely valuable in detecting vascular lesions that were invisible with ordinary ophthalmoscopy. Retcam II allows documentation of these lesions which is very useful for diagnosis, treatment and follow-up of this disease (Arch Soc Esp Oftalmol 2009; 84: 529-532).

Thirty-two patients with presumed choroidal nevi were examined and photographed in both white and green light. Twenty-one lesions disappeared or became markedly attenuated when viewed with green light. The remaining 11 lesions did not disappear when viewed with green light. Many of these nevi demonstrated visual field defects and fluorescein angiographic abnormalities. The clinical applications of green light ophthalmoscopy and photography are discussed.

Binocular diplopia is usually due to misalignment from ocular motor dysfunction. Three patients with chronic binocular vertical diplopia and small comitant hypotropic deviations due to macular displacement (heterotopia) associated with monocular retinal wrinkling are described. This maculopathy was idiopathic in two and secondary to treated retinal detachment in the third. Displacement of the macula by vitreoretinal traction is an unusual cause of binocular diplopia that requires careful ophthalmoscopy to diagnose.

In this case report a patient with bilateral optic nerve hypoplasia, schizencephaly and a pigmented skin lesion is described. The diagnosis of de Morsier syndrome or septo-optic dysplasia is put forward on the basis of the diagnosis of optic nerve hypoplasia. The differential diagnosis with Jadassohn's naevus phakomatosis is discussed. The importance of direct ophthalmoscopy of optic nerve abnormalities is stressed, as well as of magnetic resonance imaging, which has become a guideline in the classification of this syndrome.

A 53-year-old man presented with an acute headache and mental status changes due to rupture of an anterior choroidal artery aneurysm. A preoperative CT scan demonstrated subarachnoid hemorrhage, bilateral optic nerve sheath hemorrhage, and bilateral intraocular hemorrhage. Ophthalmoscopy and B-scan ocular ultrasound disclosed vitreous hemorrhages, features consistent with Terson syndrome. This is the first CT report of Terson syndrome showing bilateral optic nerve sheath hemorrhage.

Differentiation of a primary uveal melanoma from a uveal metastasis can be difficult. Review of the records of 627 patients with choroidal or ciliary body melanoma seen at our institution between September 1977 and November 1987 showed that 53 (8%) had a second, systemic malignant disorder. A complete ophthalmologic evaluation, including indirect ophthalmoscopy, fundus photography, ultrasonography and general medical studies, should minimize incorrect ocular diagnosis and inappropriate therapy.

This paper presents the clinical findings in 12 patients with a peculiar vascular mass of the sensory retina. On the basis of ophthalmoscopy, fluorescein angiography, ultrasonography and other studies, the authors presume that the described lesion is a primary acquired benign vascular tumor, most likely a variant of capillary hemangioma. The features that seem to differentiate clinically this lesion from von Hippel's retinal angiomatosis, other ocular tumors, and pseudotumors are discussed and therapeutic recommendations are presented.

A 15-year-old white boy had bilateral vitreochorioretinal degeneration associated with trichomegaly. Findings on electroretinography and fluorescein angiography provided additional information on the retinal changes not noted in 3 previously reported cases having these associated features. Although similar on ophthalmoscopy, fluourescein angiography, and by electroretinography, the retinal disease in our patient differed from cases of gyrate atrophy of the choroid and retina in having normal plasma ornithine levels.

A 37-year-old man presented with an optic nerve pit and serous macular detachment of the left eye. Scanning laser ophthalmoscopy revealed a cyst-like structure terminating at the pit in the premacular vitreous. During ocular movement, this structure moved vigorously and seemed to exert traction on the pit. We believe that it is part of an anomalous Cloquet's canal, and that traction on the pit may be a significant factor in the development of serous macular detachment in this patient.
