Histiocytosis X, a clinical entity which was not defined and classified exactly so far, is manifested by a varied range of clinical manifestations. A 35-year-old man with a history of repeated previous facial injuries presented with acute onset of a right orbital mass less than 2 weeks after blunt trauma to the right eye. Orbital B-scan ultrasonography and computed tomography showed an intraconal cystic lesion. The tumor was excised via lateral orbitotomy. A case of orbital tuberculoma presenting with proptosis and gross diminution of vision in a young girl 20 years is reported. The tuberculoma disappeared within 3 months of antitubercular therapy and there was marked improvement in visual acuity. Because of its rarity in the younger age group, the case is being reported.

Gunshot wounds to the globe and orbit require careful evaluation and management. This article highlights five cases of penetrating gunshot injuries, each demonstrating different points to consider when managing the severely traumatized patient. Immediate and late complications are described. Missile velocities of different weapons are detailed, and the mechanism of injury is discussed. Careful preoperative evaluation is stressed as a means of avoiding complications.

Immunological responses in the eyelid and the orbit are reviewed: (1) A local immune response is dependent on the presence of lymphoid tissue in an organ. Lymphoid tissue is found in the conjunctival fornices and in the lacrimal gland but not in the orbit. The eyelids also have lymphatic drainage into the local lymph nodes. Seven patients with orbital plexiform neurofibromas (PNFs) were evaluated by computed tomography (CT). These lesions are pathognomonic of von Recklinghausen disease and manifest a spectrum of radiologic abnormalities ranging from thickening of the eyelid and periorbital soft tissues to enlargement of the bony orbit and extensive infiltration of the orbital soft tissues. With the advent of the antibiotic era, the incidence of osteomyelitis has decreased remarkably. However, cases of trauma-induced orbital cellulitis, abscess, fistula formation, and osteomyelitis may still occur. The following case is illustrative of the chronicity of problems (8-year course) caused by retained orbital wooden foreign bodies.

Proptosis of the left eye developed in a 51-year-old man with a long history of nasal polyposis. Displacement of the orbit was associated with a palpable mass superior and lateral to the orbit. Enophthalmos with an associated superior sulcus syndrome is a common finding in the anophthalmic socket. Two common causes are: (1) progressive relaxation of the lower eyelid leading to downward migration of the prosthesis and associated ectropion formation; and (2) decreased volume of the orbit. Relaxation of the lower eyelid is corrected with repair and support using autogenous fascia lata. Decreased volume of the orbit is corrected with implantation of RTV silicone. The surgical techniques, surgical complications, ocular prosthetic modifications and follow-up on 20 patients are discussed.
