Role of basic biological sciences in clinical orthodontics: a case series.

INTRODUCTION: Orthodontic therapy is based on interaction between mechanics and biology. Basic biologic research aims at developing a better understanding of the mechanism of transformation of mechanical energy into biologic reactions, and exposing the reasons for iatrogenic tissue damage in orthodontics. Previous research has shown that inflammation is a major part of the biologic response to orthodontic forces. In inflammation, signal molecules that originate in remote diseased organs can reach strained paradental tissues and exacerbate the inflammatory process, leading to tissue damage. ME…

Neuromuscular diagnosis in orthodontics: effects of TENS on maxillo-mandibular relationship.

AIM: This study was conducted in order to assess the changes in the occlusal position of the mandible after Ultra Low Frequency (ULF)-TENS r … onclusion This study suggests that TENS recorded occlusion is an interesting diagnostic approach in orthodontics since it allows visualising the trends of the neuromuscular system.

A Bibliometric Analysis of 100 Top-cited Articles on Cleft Orthodontics.

ObjectiveThis bibliometric study seeks to provide a comprehensive overview of the 100 most frequently cited articles in the domain of cleft orthodontics. The analysis will reveal key influential publications, collaborative author networks, and identify prevailing research themes within the field.MethodThe studies related to Orthodontics in the realm of cleft lip and palate (CLP) were retrieved from the Scopus database on 30th August 2024 using key terms. The results obtained were sorted in descending order of citations and the 100 top-cited articles were hand-filtered. RStudio software version…

White spot lesions in orthodontics: consensus statements for prevention and management.

OBJECTIVE: To establish consensus recommendations for clinicians to manage white spot lesions (WSLs) during orthodontic treatment. MATERIALS AND METHODS: Three task force members reviewed the literature to identify best practices for minimizing WSLs during orthodontic treatment. Each draft statement was read to the task force members by a facilitator, followed by voting, accepting, or editing if necessary. The statements were then sent electronically by an independent third party (Magellan Medical Technology Consultants Inc, Minneapolis, Minn) to a previously formed content validation panel c

Periodontal soft tissue increase induced by periodontally accelerated osteogenic orthodontics surgery.

OBJECTIVES: To quantitatively assess periodontal soft tissue changes, including gingival thickness and keratinized gingiva width after periodontally accelerated osteogenic orthodontics (PAOO) surgery by digital measurements.  METHODS: This study enrolled 15 maxillaries with 89 anterior teeth and 16 mandibles with 94 anterior teeth from Chinese adult patients with skeletal Angle Class III malocclusion for whom PAOO surgery was proposed during orthodontic treatment. Intraoral scanning and cone beam computed tomography (CBCT) examinations were performed before PAOO surgery and 6 months after the …

A new subgingival sling suture technique for periodontally accelerated osteogenic orthodontics.

This study aimed to design a modified subgingival sling suture for periodontally accelerated osteogenic orthodontics (PAOO) as well as evaluate postoperative effects including gingival recession (GR), alveolar bone crest resorption, dental plague accumulation on sutures and alveolar bone augmentation. Twelve patients with bone defects in the anterior alveolar region of the mandible were included in this study. Subgingival sling suture, developed from traditional sling suture, was applied in modified PAOO operation. Probing depth, bleeding index, and GR were assessed, and cone-beam computerized…

How efficient is customized lingual orthodontics? An assessment of treatment outcome.

OBJECTIVE: To assess the efficacy of lingual orthodontics by comparing setups and post-treatment casts. SETTING AND SAMPLE POPULATION: Thirty-two consecutive patients treated with a customized lingual orthodontic appliance were included in this retrospective study. MATERIALS AND METHODS: Initial casts, post-treatment casts and setups were scanned, and the digital models produced were analysed in terms of overjet; overbite; molar and canine relationships; intercanine, interpremolar and intermolar distances; upper and lower arch lengths; midline deviation; bucco-lingual angulation of all teeth…

Assessment of Maxillary Expansion by Piezocision-Assisted Orthodontics: A Pilot Study.

The correction of transverse malocclusions due to maxillary width deficiency in adults is challenging. Multiple surgical and nonsurgical procedures have been used in conjunction with orthodontics to address this situation, and most common is the surgically assisted rapid maxillary expansion (SA-RME). Although successful, it is quite aggressive. The present investigation assesses the usefulness of Piezocision-assisted orthodontics as a less-invasive option for treatment of transverse maxillary deficiencies in adults. Dental casts were taken before and after Piezocision-assisted palatal expansio…

Occlusion, orthodontics, and temporomandibular disorders: Cutting edge of the current evidence.

The relationship between occlusion and temporomandibular disorders (TMD) represents one of the major controversies reported in the scientific literature. Nevertheless, sound evidence from large population-based surveys point toward weak and inconsistent associations, supporting that the role of the occlusion in the etiology of TMD should not be overstated. Also, despite the type of device, traditional orthodontic treatment seems to play a neutral role with regard to the onset of TMD: in particular, this therapeutic approach has been reported not to prevent or to increase the risk of TMD develo

Paediatric Orthodontics Part 1: Anterior open bite in the mixed dentition.

The correction of anterior open bite in the mixed dentition: treatment or over-treatment? Early treatment of the anterior open bite is a common, widespread treatment, which involves sometimes multiple clinical procedures: active orthodontic treatment and correction of the dysfunctional habits with a large waste of financial and biological costs. Therefore, also considering the significant possibility of self-improvement in the pre-pubertal phase, active early treatment should not aim to the active correction of the AOB, but only to other aspects of the malocclusion and to the interception of d

A new modified bone grafting technique for periodontally accelerated osteogenic orthodontics.

The aim of this study was to introduce an improved surgical technique using a pouch design and tension-free wound closure for periodontally accelerated osteogenic orthodontics (PAOO) in the anterior alveolar region of the mandible.Patients with bone dehiscence and fenestrations on the buccal surfaces of the anterior mandible region underwent the modified PAOO technique (using a pouch design and tension-free closure). Postoperative symptoms were evaluated at 1 and 2 weeks intervals following the procedure. Probing depth (PD), gingival recession depth (GRD), and clinical attachment level (CAL) w…

Influence of clinicians' experience and gender on extraction decision in orthodontics.

OBJECTIVE: To determine whether, in Class I borderline cases, experienced orthodontists choose nonextraction treatment more frequently than … es. The survey was distributed to approximately 2000 clinicians through the American Association of Orthodontics. RESULTS: Of the 253 responses collected, a trend was observed wherein clinicians with more than 15 years of experience preferred an extraction treatment option more frequently than did clinicians with less than 5 years of experience. There was no …

Preprosthetic interceptive orthodontics for missing lateral incisors in late mixed dentition.

BACKGROUND: Different treatment alternatives are possible in the preprosthetic orthodontic management of missing of lateral incisors. We describe an efficient approach in a 12.11-year-old girl with incisors agenesis. CASE REPORT: Treatment started with repositioning of the permanent canines in site 2 and the deciduous canines in site 3. After growth completion the deciduous canines will be extracted and replaced by dental implants. Permanent canines will then undergo reshaping in order to look like lateral incisors. The molar and cuspid relationships were finalised in Class I, with correct ov
