Beyond traditional orthopaedic data analysis: AI, multimodal models and continuous monitoring.

Multimodal artificial intelligence (AI) has the potential to revolutionise healthcare by enabling the simultaneous processing and integration of various data types, including medical imaging, electronic health records, genomic information and real-time data. This review explores the current applications and future potential of multimodal AI across healthcare, with a particular focus on orthopaedic surgery. In presurgical planning, multimodal AI has demonstrated significant improvements in diagnostic accuracy and risk prediction, with studies reporting an Area under the receiving operator curve

Team Approach: Use of Opioids in Orthopaedic Practice.

»: The opioid epidemic represents a serious health burden on patients across the United States. »: This epidemic is particularly pertinent to the field of orthopaedics because it is one of the fields providing the highest volume of opioid prescriptions. »: The use of opioids before orthopaedic surgery has been associated with decreased patient-reported outcomes, increased surgery-related complications, and chronic opioid use. »: Several patient-level factors, such as preoperative opioid consumption and musculoskeletal and mental health conditions, contribute to the prolonged use of opioids

Athletic children: Guidelines and monitoring in pediatric orthopedic surgery.

In day-to-day practice pediatric orthopedic surgeons often come up against the question of sport. The aim of the present study was to analyze the relationship between sport and childhood, with 3 questions: (1) What are the benefits of sport for children? (2) How to manage high-level child athletes? And (3) What sports are possible after major orthopedic surgery? Sports provide many benefits for children, and are to be encouraged. Sixty minutes' moderate to intense physical activity per day benefits motor development and bone mineralization and reduces the risk of obesity. On the other hand, ex

A Global Bibliometric Analysis of Hallux Valgus Research (1999-2019).

Hallux valgus (HV) is a very common deformity among foot disorders, therefore attracting strong interest from foot and ankle surgeons. We investigated publication trends on HV in the literature. The analysis was conducted through an electronic search of the Web of Science database for publications between 1999 and 2019, studying the data of origin through bibliometrics. The following search string was utilized: TI = (hallux valgus* OR hallux abductovalgus*) with filters for the English language and documents in article format. The "Bibliometrix" package of R software was used for the bibliomet

Endoscopic repair of distal biceps tendon using an EndoButton.

The technique of endoscopic repair of an acute rupture of the distal biceps tendon with a single anterior portal incision (1.5 cm) and fixation with an EndoButton (Acufex, Smith & Nephew, Andover, MA) is described. The ruptured biceps tendon is delivered through the portal wound and sutured to the EndoButton with a No. 5 Ethibond suture (Ethicon, Somerville, NJ). Endoscopic visualization of the tract of the avulsed biceps tendon guides the endoscopy cannula to the radial tuberosity. The endoscopy cannula serves to protect the adjacent neural structures while using the guidewire and drills for

The role of traditional growing rods in the era of magnetically controlled growing rods for the treatment of early-onset scoliosis.

PURPOSE: To describe the clinical and radiographic profile of early-onset scoliosis (EOS) patients treated with traditional growing rods (TGR) during the magnetically-controlled growing rod (MCGR) era. METHODS: A US multicenter EOS database was reviewed to identify (1) patients who underwent TGR after MCGR surgery was introduced at their institution, (2) patients who underwent MCGR during the same time period. Of 19 centers, 8 met criteria with all EOS etiologies represented. Clinical notes were reviewed to determine the indication for TGR. Patient demographics and pre-operative radiographs w

Managing the Complex Cavus Foot Deformity.

This article describes approaches to and the management of complex cavus foot deformities. Correction of rigid multiplanar deformities can be very challenging, given the presence of skeletal deformities in multiple planes and combined with a varying degree of muscle imbalance. The complexity of these cases always requires a case-by-case approach. Some of the cases presented here occur in patients who have previously undergone surgical management for their deformity, several of which are complicated by additional deformities. With a firm understanding and application of the principles of deform

Modified McCash Technique for Management of Dupuytren Contracture.

Despite recent advancements in the nonsurgical treatment for Dupuytren contracture, a number of patients remain poor nonsurgical candidates or elect for surgical management. The traditional McCash technique releases contractures while leaving open palmar wounds. Although successful in alleviating contractures, these wounds are traditionally large, transverse incisions across the palm. A modification of this technique has been performed that permits the surgeon to utilize smaller wounds while eliminating debilitating contractures.

Complications of Lateral Epicondylar Release.

Reported complication rates are low for lateral epicondylitis management, but the anatomic complexity of the elbow allows for possible catastrophic complication. This review documents complications associated with lateral epicondylar release: 67 studies reporting outcomes of lateral epicondylar release with open, percutaneous, or arthroscopic methods were reviewed and 6 case reports on specific complications associated with the procedure are included. Overall complication rate was 3.3%. For open procedures it was 4.3%, percutaneous procedures 1.9%, and arthroscopic procedures 1.1%. In higher-l

Evaluation and Surgical Management of the Overcorrected Clubfoot Deformity in the Adult Patient.

Adult patients presenting with an overcorrected clubfoot often have had a posteromedial release. They present later in life and have compensated quite well despite the development of deformity. Minor trauma may lead to the onset of acute symptoms. A spectrum of deformity exists. Key features include a dorsally subluxated navicular, a dorsal bunion from overpull of the tibialis anterior tendon, valgus of the ankle or hindfoot or both, and a flattop talus. This article details the diagnostic approach to the overcorrected clubfoot patient and options for management of the various components of th

Painful flatfoot deformity.

The posterior tibial muscle is the key dynamic support of the medial longitudinal arch of the foot. When it fails--typically in women older than 40 years of age--progressively, the arch slowly collapses, the heel drifts into valgus, the midfoot flattens, and the forefoot gradually abducts and supinates, resulting in painful acquired pes planovalgus abductus et supinatus. Posterior tibial tendon insufficiency (PTTI) is often misdiagnosed as a chronic ankle sprain, osteoarthritis, or collapsed arch as a result of aging or obesity, and it leaves the patient debilitated. Prompt diagnosis prevents

Treatment of distal humerus fractures.

The elbow is a complex joint and is vital in positioning the hand in space. We believe that open reduction internal fixation offers the best chance for return to function following intra-articular fractures of the distal humerus. We advocate the following principles for the effective treatment of these injuries: identification and protection of the ulnar nerve followed by transposition, broad exposure of the fracture utilizing an olecranon osteotomy, anatomic restoration of the articular surface with preservation of all osteochondral fragments, rigid fixation of both columns using pre-contoure

[Emergencies in paediatric orthopaedics].

Emergencies in pediatric orthopaedics surgery comprise a wide spectrum of diseases and injuries which demand fast diagnosis and appropriate treatement. If certain diagnostic and therapeutic measures are not undertaken punctual, in diseases like septic arthritis of the hip, acute haematogenous osteomyelitis, acute slipped capital femoral epiphysis, changes that can give arise to permanent consequences and disabilities will be developed. Well knowing of clinical features, using of ultrasound, diagnostic punctures and if necesarry using other investigations are essential for the fast diagnosis of
