Tuberculous otitis media.

Tuberculosis is one of the most common infectious diseases of developing countries including Bangladesh. It rarely affects the middle ear. The objective is to review the literature on tuberculous otitis media and know the facts, incidence, etiology, clinical presentation, investigations and treatment of tuberculous otitis media. It is characterized by painless otorrhoea which fails to respond to the usual antimicrobial treatment, in a patient with evidence of tubercle infection elsewhere followed by multiple tympanic membrane perforations, abundant granulation tissue, and bone necrosis, preaur

Temporal bone fractures: a clinical diagnosis.

Temporal bone fractures may be complicated by intracranial haemorrhage, C.S.F. leakage and infection, damage to the middle and inner ear and damage to the seventh and eighth cranial nerves. Accurate early diagnosis is important to enable adequate investigation and prompt treatment of any complications. We present eight cases seen in a 12 month period in which a temporal bone fracture was not diagnosed at presentation in spite of a full clinical examination and standard skull radiographs. Five of these cases developed complications which resulted in their referral. The absence of a visible frac

Canal-wall up cholesteatoma surgery with mastoid obliteration leads to lower rates of disease recurrence without affecting hearing outcomes.

OBJECTIVES: The primary objective was to determine whether obliteration of the epitympanic area and mastoid cavity during canal wall up (CWU) cholesteatoma surgery reduces the rate of recurrent and residual cholesteatoma compared to not obliterating the same area. The secondary objective was to compare postoperative hearing outcomes between both techniques. METHODS: A retrospective cohort study was conducted in a tertiary referral center. One-hundred-fourty-three ears were included of patients (≥18y) who underwent a CWU tympanomastoidectomy for cholesteatoma with or without bony obliteration

Prevalence of chronic ear disease among HIV+ children in Sub-Saharan Africa.

OBJECTIVES: To determine the prevalence of chronic ear disease in HIV+, highly active anti-retroviral therapy (HAART)-treated children and compare this to the prevalence in healthy children of similar age living in a similar setting. INTRODUCTION: From previous clinical work in Ethiopia, we suspected that chronic middle ear disease was common both in the general pediatric population and especially among children with HIV/AIDS. Few studies have examined the prevalence of chronic ear disease in HIV + children, particularly in those treated with HAART. METHODS: Full examination of the head and

Silent obstruction: The hidden role of aberrant internal carotid artery in middle ear pathology.

PURPOSE: Aberrant Internal carotid artery (ICA) within the middle ear is a rare congenital vascular anomaly, occurring in approximately 1% of the population [1]. It often remains clinically silent or presents with nonspecific otological symptoms, making preoperative diagnosis challenging. This study aims to highlight the importance of early identification and safe management of aberrant ICA through a clinical case and literature review. METHODS: We report a case of 36 year old female presenting with bilateral chronic ear discharge and tympanic membrane retraction. Detailed otoscopic examinatio

Middle Ear Aspiration Cytology via Otoendoscope-Guided Myringotomy Reliably Confirms Tympanokeratoma in Dogs.

The purpose of this study was to determine the correlation between cytology and histology in the diagnosis of canine tympanokeratoma. Dogs that had a middle ear aspiration cytology via otoendoscope-guided myringotomy and a histology of the tympanic bulla content sampled following total ear canal ablation with lateral bulla osteotomy between 2010 and 2024 in one hospital were retrospectively examined. Cytological diagnosis of tympanokeratoma was considered when the sample contained a majority of corneocytes. Thirty-seven ears with a pathological diagnosis of either tympanokeratoma (31/37) or no

Juvenile otosclerosis: a case presentation and review of the literature.

Otosclerosis in childhood and adolescence or juvenile otosclerosis is a rare disorder resulting in conductive hearing loss. A 9-year-old boy presented to our clinic, suffering from moderate hearing loss. According to his parents, his hearing acuity had progressively deteriorated over the past 2 years. Otoscopy and tympanometry revealed bilateral secretory otitis media and the patient underwent bilateral grommet insertion. However, he continued to report of hearing loss and a right exploratory tympanotomy was performed. Stapedial fixation was confirmed, being compatible with juvenile otoscleros

Clinical aspects of middle ear tympanokeratoma in dogs diagnosed through advanced imaging, otoendoscopy and histopathological evaluation.

BACKGROUND: Canine aural cholesteatoma (more appropriately named tympanokeratoma) is an epidermoid cyst whose aetiopathogenesis remains poorly recognised in veterinary medicine. There are a few reports published, possibly because it may be underdiagnosed. OBJECTIVES: To characterise the clinical aspects of dogs with tympanokeratoma, to describe the otoendoscopic, advanced imaging and histopathological findings of tympanokeratoma and to report the best approach to diagnose canine auricular tympanokeratoma in a retrospective study. MATERIAL AND METHODS: Of 890 dogs with suspected tympanokerato

Diagnosis and management of a tympanic membrane hemangioma.

Vascular tumors arising in the temporal bone represent 0.7% of all lesions in this area. Hemangiomas confined to the tympanic membrane are exceptionally rare. We report a new case of tympanic membrane hemangioma that arose in 59-year-old man who presented with a 2-month history of constant right-sided pulsatile tinnitus and associated neck discomfort. The lesion and a cuff of healthy tympanic membrane were excised, and the resulting defect was repaired with a temporalis fascia graft. At 3 months, the neotympanum was well healed and the patient's symptoms had resolved. We also review the limite

[Otoacoustic emissions in hearing screening in children].

BACKGROUND/AIM: Prevention of the consequences of hearing loss can be accomplished by early hearing screening of the cochlear function in newborus, but also with continuous hearing screening in the early childhood. The aim of this study was to investigate the reliability and feasibility of otoacoustic emissions (OAE) in hearing screening in children, evaluate the frequency and type of hearing loss and determine etiological factors of these losses. METHODS: In the study, 133 children, aged 2 to 7.5 years, were tested on both ears with transient evoked otoacoustic emissions (TEOAE) and distorti

Foramen Tympanicum (Foramen of Huschke) as a Cause of Unexplained Spontaneous Otorrhea: A Case Report.

The foramen of Huschke or foramen tympanicum is a developmental defect in the bony part of the external auditory meatus. It occurs due to incomplete obliteration of the tympanic canal and the persistence of openings along the canal. This leads to the presence of a salivary fistula and the most common symptoms of the condition are otalgia, otorrhea, and hearing loss. We report a case of a 69-year-old female patient who presented to the ENT with symptoms of watery discharge from the left ear for 12 years. In the last two weeks, pain and itching appeared. Otoscopy showed the presence of a clear f

Secondary Otalgia: Referred Pain Pathways and Pathologies.

Otalgia is very common, and when the cause of ear pain is not identified on otoscopy and physical examination, cross-sectional imaging is routinely used to evaluate for potential sources of referred ear pain (secondary otalgia). Innervation of the ear structures is complex, involving multiple upper cervical, lower cranial, and peripheral nerves, which transit and innervate a large anatomic territory involving the brain, spine, skull base, aerodigestive tract, salivary glands, paranasal sinuses, face, orbits, deep spaces of the neck, skin, and viscera. Interpreting radiologists must be familiar

Neuroendocrine adenoma of the middle ear presented as paragaglioma: a case report.

BACKGROUND: Neuroendocrine adenoma of the middle ear (NAME) is a rare benign glandular tumor with neuroendocrine differentiation arising from the middle ear mucosa. The common symptoms and clinical findings of this tumor are usually non-specific. When the tumor appears as a reddish retrotympanic mass, it can be misdiagnosed to be a paraganglioma. CASE DESCRIPTION: A 38-year-old male presented with a 12-month history of left ear fullness and tinnitus. Otoscopy revealed a reddish retrotympanic mass with an intact tympanic membrane. The imaging demonstrated a soft tissue mass adjacent to the e

Schneiderian papilloma of the temporal bone.

Temporal bone Schneiderian papilloma may present as a primary tumour originating from the middle ear and mastoid process, or an extension from sinonasal disease. Both forms are rare, this being only the 18th case of primary temporal bone Schneiderian papilloma described to date. Although the current patient has remained disease free after excision of the papilloma, the reported recurrence rate is high, comparable to sinonasal Schneiderian papilloma with extrasinus extension. Malignant progression of primary Schneiderian papillomas is significantly reduced as compared to Schneiderian papillomas
