This is a case report of a 19-year-old woman with cannabinoid hyperemesis syndrome (CHS), which is characterised by cyclic vomiting and nausea, relieved by hot showers and caused by chronic cannabis usage. Many diagnoses must be ruled out, but the suspicion of cannabis usage must be present. Early suspicion of CHS can prevent overtreatment and unnecessary investigations. Lack of evidence makes treatment difficult and is primarily symptomatic. Attention to cannabis usage is needed when admitting patients with emesis.

Renal lymphangiomatosis is an unusual disorder. It may develop due to the abnormality of the intrarenal, peripelvic and perirenal lymphatics. The differential diagnosis contains renal lymphoma, polycystic kidney disease, multicystic dysplasia and renal tumors. We report a case of renal lymphangiomatosis, previously diagnosed as autosomal dominant polycystic kidney disease, to emphasize that these two diseases can be easily confused. It should be kept in mind that RL is in the differential diagnosis of polycystic renal disease to prevent overtreatment.

Non-alcoholic Wernicke Encephalopathy (WE) is an overlooked complication of malnourishment in all its forms including undernutrition, inadequate vitamine intake and endocrinological diseases. Both delay in treatment and overtreatment can lead to severe neurological life-long consequences. Inadequate management in patients with chronic malnutrition may cause abrupt osmolytes unbalance and subsequent osmotic demyelination syndromes. We describe a 65-year old man with apathetic encephalopathy caused by thyreotoxicosis and associated with malnutrition and severe thiamine depletion.

Porokeratosis (PK) consists of a group of mucocutaneous lesions, including some with a significant rate of malignant transformation. Oral PK has been rarely reported. Awareness of its clinical presentation, histopathological features, and appropriate management are necessary to avoid misdiagnosis and overtreatment. While the potential of oral PK to undergo malignant transformation remains unanswered, this report highlights an unfamiliar condition affecting the oral cavity. We present three new cases of oral PK and review the pertinent literature.

Littoral cell angioma (LCA) is a rare benign tumor originating exclusively from the venous sinus lining cells of the splenic red pulp. These cells are unique in having a distinctive hybrid endothelial/histiocytic phenotype. Also, there are reports of the association of LCA with internal malignancies. We present a case report highlighting an unusual association of LCA with conventional renal cell carcinoma (RCC), masquerading as a metastatic lesion. Knowledge of such an association is necessary to avoid misdiagnosis and prevent potential overtreatment.

Human orf is usually considered a rare disease caused by a virus belonging to the paravaccinia subgroup of pox viruses and transmitted to man from sheep and goats. This paper presents 119 new human cases with epidemiological, clinical, histopathological and ultrastructural findings. Erythema multiforme was found to be a common complication of human orf. Other complications tended to be caused by overtreatment. Electron microscopy of negatively stained suspensions from lesions was found to be the best and most rapid diagnostic method available.

Using Germany as a case example, a comparison between organized mammography screening and contemporary, risk-adapted screening for prostate cancer suggests that prostate-specific antigen-based early detection with incorporation of magnetic resonance imaging and active surveillance can achieve benefits comparable to those of mammography while addressing previous concerns about overdiagnosis and overtreatment. Findings from the PROBASE trial support the potential of such an approach to provide a favorable balance between benefit and harm when initiated at younger ages.

We report the case of an 81-year-old male presenting with lower urinary tract symptoms and a PSA of 39 ng/mL, initially attributed to Escherichia coli prostatitis. Despite PSA normalization with antibiotics, MRI demonstrated a PIRADS 5 lesion in the left transition zone. Histopathological evaluation following transperineal biopsy revealed prostatic malakoplakia. This rare granulomatous condition can mimic prostate cancer both clinically and radiologically. Our case highlights the importance of histological confirmation in atypical presentations to avoid misdiagnosis and overtreatment.

Longus colli tendinitis (LCT) has been described in several case reports, and is likely underdiagnosed due to its non-specific symptoms and self-limiting nature. It usually presents as acute neck pain with retropharyngeal swelling seen on nasoendoscopy and imaging studies. This may lead to unnecessary invasive interventions if physicians are unaware of the disease course. We discuss a case of LCT in a young woman who was treated at our institution, with recommendations on how to identify and treat such cases accurately without overtreatment.
