Radiographic Parameters to Predict Union After Volar Percutaneous Fixation of Herbert Type B1 and B2 Scaphoid Fractures.

PURPOSE: To study the angle of screw placement in relation to the scaphoid fracture plane and its effect on union after percutaneous fixation of scaphoid waist fractures. METHODS: Twenty-four consecutive scaphoid waist fractures were retrospectively evaluated for the orientation of screws in relation to the fracture plane using a method in which the sum-of-smaller angles (SSA) in 3 different radiographs were used to correlate with time to fracture union. RESULTS: All but one patient achieved union after percutaneous fixation of the scaphoid. Another patient required revision surgery within t

Biomechanical comparison of volar locked plate constructs using smooth and threaded locking pegs.

The goal of this study was to determine whether there is any biomechanical difference in terms of construct strength with axial loading between volar fixed-angle locking plates with threaded locking vs smooth locking pegs. The control group comprised 7 cadaveric specimens with threaded locking pegs, and the test group comprised 7 cadaveric specimens from the same donor with smooth locking pegs. The DVR plate (Biomet, Warsaw, Indiana) was applied to the volar surface. A 15-mm dorsal wedge osteotomy was created near the level of Lister's tubercle. The radii were potted in polymethylmethacrylate

[Biomechanics of distal radius fractures : Basics principles and GPS treatment strategy for locking plate osteosynthesis].

Fractures of the distal radius are most commonly caused by hyperextension injuries of the wrist. Tensile forces and force vectors, strength of impact, bone strength and soft tissue tension create individually different fracture patterns. Metaphyseal comminution, loss of cortical support, ligament avulsion and shear fragments are defining parameters for fracture instability. The dislocation of the articular fragment follows the force vectors of the extrinsic forearm muscles bridging the joint. The goal-plan-standardized (GPS) treatment strategy has proven to be helpful in choosing the ideal ind

Open reduction and plate fixation of dorsally displaced fractures of the distal radius: surgical technique, clinical and radiological outcome.

Thirty-five unstable dorsally displaced fractures of the distal radius in 34 patients (mean age 39) were studied; 28 fractures were intra-articular. All fractures had open reduction and fixation, through a dorsal approach, with mini-fragment plates placed between the first and second dorsal tendon compartments and deep to the fourth compartment. A congruous reduction of the articular surface was obtained in all cases. Twenty-four patients were available for follow-up (median 38 months). The Modified Mayo wrist score was excellent in 12 cases, good in four, and fair in eight. The median Patient

Treatment of Malassezia pachydermatis-associated seborrhoeic dermatitis in Devon Rex cats with itraconazole--a pilot study.

Treatment of Malassezia pachydermatis-associated seborrhoeic dermatitis with oral itraconazole was investigated in six Devon Rex cats (DRC). The cutaneous populations of Malassezia were determined using contact plates and a swab-wash method before and after 21 days of pulse treatment with itraconazole (5 mg kg-1 once daily, 7 days on, 7 days off, 7 days on). Before treatment, all cats had greasy seborrhoeic dermatitis involving the axillae, groin, claw folds and palmar and plantar interdigital skin, and two had similar lesions on the ventral neck. After treatment, there was a significant (P<0.

The proximal interphalangeal joint in Dupuytren's disease.

A precise excision of the diseased fascia corrects proximal interphalangeal joint deformity in many instances. When excision of the fascia results in a contracture of greater than 30 degrees that has not been corrected to this level by gentle manipulation, then a systematic release of the causative structures is occasionally indicated, paying particular attention to the check rein ligaments of the palmar plate and adherence of shortened collateral and accessory collateral ligaments. Postoperative physical therapy and splinting are necessary, particularly in those cases in which a joint release…

Locked metacarpo-phalangeal joint of the thumb.

16 patients developed locking of the M.P. joint of the thumb as the result of a hyperextension injury. Four were successfully treated by manipulation under an intra-articular injection of a local anaesthetic, while the remaining twelve required open reduction. All were examined 4 to 48 months after treatment (average 12 months). The torn palmar plate and the accessory collateral ligament, which acted as a constrictive band, were trapped on the prominence of the radial condyle of the metacarpal. Four of the twelve surgically-treated patients had a deformity which we believe was congential: a tr…

Volar locking plate osteosynthesis for distal radial fractures.

PURPOSE: To review the one-year outcome after volar locking plate fixation for distal radial fractures. METHODS: Records of 22 men and 40 women aged 17 to 86 (mean, 52.5) years who underwent volar locking plate fixation for distal radial fractures were reviewed. According to the Fernandez classification, the distal radial fractures were classified as type 1 (n=20), type 2 (n=24), type 3 (n=6), type 4 (n=6), or type 5 (n=6). Three types of plate were used: Stryker Variax (n=33), Synthes LCP (n=20), and Smith & Nephew Peri-Loc (n=9). Wrist function was assessed at one year using the validated p

A Brachioradialis Splitting Approach Sparing the Pronator Quadratus for Volar Plating of the Distal Radius.

Fractures of the distal radius account for up to 15% of all extremity fractures. Volar plating has become more popular, as it allows locking plate technology to be applied. Traditionally, access to the volar radius has been achieved through the approach of Henry using the interval between flexor carpi radialis and the radial artery, involving incising the radial border of the pronator quadratus (PQ). With this approach, PQ repair is difficult, and when attempted is often incomplete or tenuous, as it is a direct muscle-to-muscle repair. Theoretical advantages of repairing the PQ include the pro

Volar Rim Morphology and Plate Congruence in Fragment-Specific Distal Radius Fixation.

BACKGROUND: Fragment-specific fixation of distal radius fractures often requires implants positioned at or beyond the watershed line, which may increase the risk of flexor ultrasound-detected flexor tendon abnormalities. However, tendon conflict may depend not only on plate position but also on how the implant adapts to individual volar rim morphology. This study assessed whether volar rim morphology, classified according to the Dolomites concept, was associated with plate application and ultrasound-detected flexor tendon abnormalities after fragment-specific fixation. We hypothesized that Dol

The metaphyseal bone defect in distal radius fractures and its implication on trabecular remodeling-a histomorphometric study (case series).

BACKGROUND: The invention of the locking plate technology leads to alterations of treatment strategies at metaphyseal fracture sites with the concept of spontaneous remodeling of trabecular bone voids. Whereas trabecular regeneration has been proven in experimental animal studies, no histologic data exist on human fracture healing with special emphasis on bone voids. METHODS: In order to qualify the trabecular bone remodeling capacity in vivo, bone specimens from the metaphyseal bone void were analyzed 14 months after trauma using quantitative histomorphometry. Twenty-five patients with an un
