Neurolept anaesthesia for the renal transplant operation.

Between October 1973 and December 1976, neurolept anesthesia was used in 176 consecutive renal transplantations carried out in 155 patients. For premedication, the combination either of droperidol, diazepam and atropine or of oxicon and scopolamine was used; the latter was found to be more effective. The induction agent was droperidol or thiomebumal sodium, the barbiturate being found to be preferable. For maintenance, nitrous oxide and fentanyl were employed. Pancuronium was given as a muscle relaxant. The fact that the patients belonged to a high-risk category as regards anesthesia was appar

Anesthesia for craniotomy in a patient with previous paralytic polio.

Poliomyelitis remains endemic in many developing nations. Patients may develop residual muscle weakness in one or more limbs after an attack of poliomyelitis in childhood. We report an adult patient who presented for right temporal cortical grid placement. He had childhood poliomyelitis and, while showing no evidence of postpolio syndrome, demonstrated excessive sensitivity to nondepolarizing muscle relaxants and developed prolonged muscle weakness during the postoperative period.

Vecuronium kinetics and dynamics in anesthetized infants and children.

Vecuronium kinetics and dynamics were determined in five infants (3 to 11 months old) and five children (1 to 5 years old) during anesthesia with 70% nitrous oxide and 0.9 MAC halothane. Vecuronium was infused intravenously at a rate of 2.5 micrograms/kg/min while twitch tension of the adductor pollicis muscle was recorded and venous blood samples were drawn for determination of vecuronium concentrations by mass spectrometry. The elimination t1/2 was determined by linear regression of the log postdistribution concentration-time data; these values and noncompartmental techniques were used to ca

Parasympathetic mediated pupillary dilation elicited by lingual nerve stimulation in cats.

PURPOSE: To determine the autonomic efferent nerve pathways for the reflex pupillary dilation elicited by somatic stimulation in cats. METHODS: Cats anesthetized with a mixture of alpha-chloralose (50 mg/kg) and urethane (100 mg/kg) were intubated and paralyzed by intravenous injection of pancuronium bromide. The central cut end of the lingual nerve (LN) was stimulated electrically to simulate somatic stimulation, and 1 microL of lidocaine (2%) was microinjected into the Vsp or the EW nucleus to determine its effect on the pupillary dilation induced by LN stimulation. The effect of electrical

Systemic administration of serotonin 2A/2C agonist improves upper airway stability in Zucker rats.

The effects of [+/-]-2,5-dimethoxy-4-iodoaminophentamine, a serotonin(2A/2C) receptor agonist, on pharyngeal airflow mechanics were examined in isoflurane-anesthetized lean and obese Zucker rats. The pharyngeal pressure associated with flow limitation, maximum inspiratory flow, oronasal resistance, genioglossus muscle activity, and arterial blood pressure (BP) were measured before and after the intravenous administration of the agonist. A robust activation of the genioglossus muscle in all lean and obese rats was associated with decreased upper airway (UA) collapsibility (p < 0.05), unchanged

Prevention of hypertension during trans-sphenoidal surgery--the effect of bilateral maxillary nerve block with local anaesthetics.

BACKGROUND: Severe cardiovascular responses are known to occur during trans-sphenoidal excision of the pituitary gland despite adequate depth of anaesthesia. This study was undertaken to evaluate the effects of bilateral maxillary nerve block with local anaesthetics on the cardiovascular responses to various stimuli during this procedure. METHODS: In a group of 32 patients, 5-10 ml of a mixture of bupivacaine 0.5% and lignocaine 2% (1:1) was injected in the pterygopalatine fossa after induction of general anaesthesia. Seven control group patients did not receive any nerve block. In all the pa

Transfer function analysis of ventilatory influence on systemic arterial pressure in the rat.

We evaluated the hypothesis that fluctuations in systemic arterial pressure (SAP) are under the influence of the respiratory pumping mechanism subjected to a modulatory action by the autonomic nervous system that is exerted primarily on the heart. Computer-generated broad-band mechanical ventilation (0-3 Hz) was applied to Sprague-Dawley rats that were anesthetized with ketamine and paralyzed with pancuronium. We observed excellent coherence between lung volume and SAP signals at ventilatory rates between 0.5 and 2.5 Hz; this coherence was unaffected by phentolamine, propranolol, atropine, bil

Treatment of delirium in the critically ill patient.

The clinical use of neuroleptics, benzodiazepines, narcotic analgesics, barbiturates, and neuromuscular blockers to manage delirium and agitation in the intensive-care setting is reviewed. Delirium is the most commonly encountered mental disturbance in critically ill patients and may be precipitated by factors such as physical illness, medications, pain, and emotional stress. If agitation cannot be controlled through nonpharmacologic means, pharmacologic intervention may be necessary. Haloperidol is the neuroleptic of choice for rapid control of delirium and agitation in the critically ill pat

[Lack of prognostic value of the determination of 3 serum proteins during the acute phase of brain injury].

In has been suggested that monitoring of blood levels of serum proteins could be of value in order to assess the prognosis of trauma patients. A study was set up to assess the value of albuminaemia, prealbuminaemia, transferrin and ceruleoplasmin levels in intensive care unit patients admitted after multiple trauma including head injury. This study involved 43 patients (31 men and 12 women), with a mean age of 26 years (range: 15 and 71 years). Two patients had extradural haematoma. The other patients had brain contusion associated with extradural haematoma (5 cases), subdural haematoma (1 cas

Ivermectin effects on motor coordination and contractions of isolated rat diaphragm.

Ivermectin, the antiparasitic drug from the macrocyclic lactones class raises attention due to its high efficiency against nematodes and arthropods and very specific toxic and side effects that it may produce in host. Dominant clinical symptoms of adverse effects and toxicity of ivermectin in animals are tremor, ataxia, CNS depression and coma which often results in mortality. In our study increasing intravenous doses of ivermectin, (6 or more times higher than therapeutic dose: 1.25, 2.5, 3.75, 5.0, 6.25 and 7.5 mg/kg), caused dose-dependent disturbance of motor coordination in treated rats.

[Anesthesia for repair of tetralogy of Fallot in an adult patient: case report.].

BACKGROUND AND OBJECTIVES: Congenital cardiopathy is one of the most common problems at birth, with an incidence of 1 to 1.5 in 10,000 live births. Currently, more than 85% of children with congenital cardiopathy survive to adulthood, partly due to the development of new therapies, but especially because of the tendency for earlier surgical corrections. On the other hand, adult patients with non-corrected tetralogy of Fallot (TOF) are hard to find. The objective of the present report was to describe a case of repair of TOF in an adult patient. CASE REPORT: A female patient, 45 years old, with

Characterization of an esophagocardiovascular reflex in the rat.

A cardiovascular reflex evoked by esophageal distension (ECR) in urethan-anesthetized male Sprague-Dawley rats was studied to 1) determine whether the relevant sensory input from the esophagus is conveyed by vagal and/or spinal afferents and 2) evaluate the effects and sites of action of antinociceptive agents. Esophageal distension evoked a rise in arterial blood pressure and heart rate that increased linearly with the log of inflation pressure (25-150 mmHg). Distension (100 mmHg for 20 s) of the lower esophagus was a more effective stimulus than distension of the upper esophagus. The ECR was

Cross-reactivity of rocuronium with other neuromuscular blocking agents.

The cross-reactivity of rocuronium with other neuromuscular blocking agents (NMBAs) was studied in 31 patients known to be allergic to a muscle relaxant. Tests for diagnosing cross-reactivity were skin tests (prick tests and intradermal tests: IDTs), detection by RAST assay of IgEs against the quaternary ammonium group (QAS-RIA: quaternary ammonium sepharose radio-immuno-assay), QAS-RIA inhibition test to detect IgE specificity, and leucocyte histamine release test (LHRT). Skin tests were performed with rocuronium, suxamethonium, gallamine, vecuronium, pancuronium, atracurium. The threshold fo

Cardiovascular and abdominal motor responses evoked by intravenous neurotensin in guinea pigs.

The intravenous (IV) infusion of neurotensin (NT) in anesthetized guinea pigs was found to elicit dose-dependent increases of systemic blood pressure (BP) and of heart rate (HR), accompanied by abdominal motor responses consisting in transient, twitch-like contractions of the abdominal wall (TAWC), and a slowly developing, relatively sustained increase of the basal abdominal wall tension (AWT). The TAWC responses were inhibited in animals pretreated with pancuronium, morphine, clonidine, and CP-96,345 [a neurokinin (NK) antagonist], were potentiated by naloxone, but were not modified by atropi

A pharmacokinetically designed etomidate infusion regimen for hypnosis.

An etomidate infusion regimen for hypnosis as part of balanced, totally intravenous anesthesia was designed to maintain plasma etomidate concentrations above the awakening concentration of 300 ng/ml while avoiding dose-related side effects. The etomidate infusion regimen of 0.1 mg/kg/min for 3 min, 0.02 mg/kg/min for 27 min, and 0.01 mg/kg/min for the remainder of the anesthesia was used together with intravenous bolus doses of fentanyl, droperidol, and pancuronium. This was evaluated in 11 patients and the kinetics of etomidate were reexamined. The anesthetic technique seemed clinically satis

Nicotine-induced C-shape movements in planarians are reduced by antinociceptive drugs: Implications for pain in planarian paroxysm etiology?

C-shapes are stereotyped movements in planarians that are elicited by diverse stimuli (e.g. acidity, excitatory neurotransmitters, psychostimulants, and pro-convulsants). Muscle contraction and seizure contribute to the expression of C-shape movements, but a causative role for pain is understudied and unclear. Here, using nicotine-induced C-shapes as the endpoint, we tested the efficacy of three classes of antinociceptive compounds - an opioid, NSAID (non-steroidal anti-inflammatory drug), and transient receptor potential ankyrin 1 (TRPA1) channel antagonist. For comparison we also tested effe
