A fragile assemblage: Mutant bird flu and the limits of risk assessment.

This paper examines the recent public controversy sparked by the laboratory creation of a strain of highly pathogenic avian influenza transmissible among mammals. The contours of the controversy can be understood by tracking the assemblage of actors, institutions and devices gathered together in response to the governmental problem of how to manage emerging diseases. The grouping is tenuously held together by a shared commitment to the project of 'pandemic preparedness'. However, as the controversy unfolds, it becomes clear that the main actors involved do not share a common understanding of t…

Transmissibility of the 1918 pandemic influenza in Montreal and Winnipeg of Canada.

BACKGROUND: The threat of 2009 pandemic influenza A (H1N1) is still causing widespread public concern. A comprehensive understanding of the epidemiology of 1918 pandemic influenza commonly referred to as the Spanish flu may be helpful in offering insight into control strategies for the new pandemic. OBJECTIVE: We explore how the preparedness for a pandemic at the community and individual level impacts the spread of the virus by comparing the transmissibility of the 1918 Spanish flu in two Canadian cities: Montreal and Winnipeg, bearing in mind that each pandemic is unique and the current one

Healthcare workers' knowledge, attitude and practices on infection prevention and control in the context of the COVID-19 pandemic at the Faranah regional hospital and associated healthcare centers, Guinea.

BACKGROUND: In response to the COVID-19 pandemic, WHO launched a strategic preparedness and response plan, outlining public health measures to support countries worldwide. Healthcare workers have an increased risk of becoming infected and their behaviour regarding infection prevention and control (IPC) influences infection dynamics. IPC strategies are important across the globe, but even more in low-resource settings where capacities for testing and treatment are limited. Our study aimed to assess and implement COVID-19 pandemic preparedness and response measures in Faranah, Guinea, primarily …

Best practices for after-action review: turning lessons observed into lessons learned for preparedness policy.

All-hazards preparedness and response planning requires ongoing individual, organisational and multi-jurisdictional learning. Disaster after … security crises. After-action reviews in a One Health security context are essential to improve the pandemic preparedness of public health and Veterinary Services. After-action reviews can also provide the evidence-based 'feedback loop' needed to galvanise public policy and political will to translate lessons observed into sustained and applied lessons learned.

Exploring the relationships between food insecurity, maternal stress, and maternal-infant health outcomes during the COVID-19 pandemic.

The COVID-19 pandemic intensified food insecurity (FI) and stress for many pregnant individuals, which may have contributed to adverse fetal developmental programming. This study aimed to identify key social determinants of health associated with pandemic-related FI and stress, and their association with gestational weight gain (GWG) and newborn birth weight in a Canadian pregnant cohort. Data were collected retrospectively from 273 pregnant individuals who delivered infants in Canada during the pandemic (March 2020-March 2023). Validated questionnaires were used to assess FI and pandemic-rela

A stochastic programming approach for dynamic allocation of bed capacity and assignment of patients to collaborating hospitals during pandemic outbreaks.

Sustaining regular and infectious care during an infectious outbreak requires adequate management support for patient capacity allocation. During the COVID-19 pandemic, hospitals faced severe challenges, including uncertainty surrounding the number of infectious patients needing hospitalization and too little regional cooperation. This led to inefficient usage of healthcare capacity. To better prepare for future pandemics, we have developed a decision support system for central regional decision-making on opening and closing hospital rooms for infectious patients and assigning new infectious p

Opportunities and lessons from the COVID-19 pandemic: a qualitative study of health program managers' perspectives in Ethiopia.

INTRODUCTION: Globally, the COVID-19 pandemic has created opportunities to enhance accessibility and strengthen health system beyond the immediate response to the crisis. Irrespective of its impact on human life and the economy, COVID-19 has contributed to some healthcare improvements. Therefore, this study aimed to explore the experiences and perspectives of health programs managers regarding the opportunities and lessons learned following the COVID-19 pandemic in Ethiopia. METHODS: A phenomenological qualitative study design was employed focusing on experts working in different programs and

Disaster preparedness among nurses caring for older adults: a systematic review.

BACKGROUND: Older adults are disproportionately affected during disasters because of multimorbidity, frailty, functional dependency, and reliance on continuous healthcare and social support services. Nurses play a central role in maintaining safety, continuity of care, communication, and clinical coordination during emergencies. However, disaster preparedness in geriatric care settings remains inconsistently developed and has often been examined through a narrow pandemic-related infection prevention and control lens rather than through comprehensive disaster nursing competency frameworks. OBJ

Adaptation of H9N2 Influenza Viruses to Mammalian Hosts: A Review of Molecular Markers.

As the number of human infections with avian and swine influenza viruses continues to rise, the pandemic risk posed by zoonotic influenza viruses cannot be underestimated. Implementation of global pandemic preparedness efforts has largely focused on H5 and H7 avian influenza viruses; however, the pandemic threat posed by other subtypes of avian influenza viruses, especially the H9 subtype, should not be overlooked. In this review, we summarize the literature pertaining to the emergence, prevalence and risk assessment of H9N2 viruses, and add new molecular analyses of key mammalian adaptation m…

The vaccination paradox: how East Africa's success in Ebola vaccine preparedness may be creating new public expectations for future outbreaks.

This perspective synthesizes published evidence on Ebola vaccination and outbreak preparedness in Africa. Successful vaccination against Zaire ebolavirus has strengthened vaccine-based outbreak control but may also create a preparedness risk when public expectations exceed species-specific vaccine availability. We propose the Vaccine Expectation Gap, Vaccine Anticipation and Species-Specific Vaccine Literacy as linked, hypothesis-generating constructs, supported by a Preparedness Trust Contract for practical implementation. The framework draws on expectation-disconfirmation and crisis and emer

[The strategic role of primary care in emerging diseases : surveillance, research, and preparedness].

The role of primary care in monitoring and under-standing emerging diseases goes far beyond the management of common conditions. Through its proximity to the population and its ability to detect weak signals early on, primary care is an essential pillar of public health. Primary care is an indispensable arena for early detection, population data collection, and conducting integrated research as close as possible to communities. Recent experiences in Belgium, particularly in population surveillance during the COVID-19 pandemic and in the fight against antimicrobial resistance, illustrate its sc

The impact of school-based mitigation measures on the transmission of respiratory pathogens: the case of SARS-CoV-2.

BACKGROUND: School-based mitigation measures can curb transmission of severe respiratory pathogens but may also impose high societal costs. Understanding their impact at the population level is key for pandemic preparedness evaluations. METHODS: We developed an individual-based model of SARS-CoV-2 transmission in households, schools, and the general community, calibrated to epidemiological estimates from complete contact tracing data collected in the province of Reggio Emilia, Italy, during March-April 2021 (a period characterized by the dominance of the Alpha variant and strict community res…

Exploring the Potential Contribution of Climate-Informed Research to Future Ebola Preparedness in Central Africa.

The ongoing 2026 Bundibugyo ebolavirus outbreak in eastern Democratic Republic of the Congo highlights the continued vulnerability of Central Africa to recurrent Ebola emergence. This outbreak emerged less than six months after the previous one ended, appears to represent one of the shortest documented inter-epidemic intervals in the Democratic Republic of the Congo based on currently available outbreak reports. Current surveillance systems remain largely reactive, focusing on the detection of human cases after zoonotic spillover has occurred. While strengthening health systems, diagnostic cap

Gabon's Experience: Factors Enabling and Impeding the COVID-19 Response.

This case study describes the country-level experience and response to the COVID-19 pandemic in Gabon between December 2020 and December 2022. We structured the presentation of COVID-19 response as an action cycle with five elements: (1) engagement, (2) assessment and surveillance measures used to track new cases, (3) planning, (4) action/implementation, and (5) evaluation. We describe the participatory monitoring and evaluation (M&E) process implemented in collaboration with the WHO Regional Office for Africa M&E team. The M&E system was used to organize and make sense of emerging data regard

Multifaceted epidemic preparedness against cholera outbreak by One Health approach and genomic surveillance.

Many developing countries in Africa and Asia are hotspots and endemic for cholera. Over the past five years, these countries have had steady rises in the incidence of cholera, with case fatality ratios continuously exceeding the WHO recommendation of 1 %. Despite this, there aren't many non-outbreak investigations that try to check for Vibrio cholerae in asymptomatic individuals who could act as reservoirs and vectors of transmission. We address the fundamental causes of global cholera transmission following a systematic review of available literature with a focus on the ecological and epidemi

The dynamic preparedness metric: results from a global and regional analysis of health emergency preparedness.

BACKGROUND: The COVID-19 pandemic has underscored limitations in current methods for assessing country-level health emergency preparedness, which often overlook essential factors like ongoing epidemics, natural disasters, conflicts, or community trust. Addressing this, the World Health Organization (WHO) developed the Dynamic Preparedness Metric (DPM), a composite measure that assesses epidemic risk by accounting for hazards, vulnerabilities, and capacity, offering insights for improving country-level health emergency preparedness. METHODS: Our analysis tested the DPM's effectiveness in suppo

Health system capacity for infectious disease pandemic response in low- and lower-middle-income countries: Insights from national health facility surveys.

OBJECTIVE: Despite growing recognition of pandemic preparedness gaps in low- and lower-middle-income countries (LLMICs), empirical evidence on facility-level capacity remains limited. This study assesses the capacity of health facilities in LLMICs to respond to infectious disease pandemics and identifies structural determinants influencing that capacity. METHODS: We used a secondary analysis of pooled Service Provision Assessment (SPA) survey data conducted in nine LLMICs between 2013 and 2022. Thirty-one facility-level indicators were grouped into four domains (infection prevention, personal…

[New Pandemic Plan in Italy: the central role of epidemiology].

The experience of the COVID-19 pandemic highlighted the need for a radical rethink of pandemic preparedness in Italy. The new National Pandemic Plan, currently under discussion at the interregional level, represents a crucial opportunity to translate lessons learned into operational tools. In emphasizing the centrality of epidemiology in prevention, early surveillance, and timely response to health emergencies, structural and organizational criticalities of the current system are highlighted. Among the main recommendations: overcoming regional fragmentation, ensuring interoperable information …
