Assessing the quality and completeness of reporting in health systems guidance for pandemics using the AGREE-HS tool.

BACKGROUND: During health emergencies, leading healthcare organisations, such as the World Health Organization (WHO), the European Centre for Disease Control and Prevention (ECDC), and the United States Centers for Disease Control and Prevention (CDC), provide guidance for public health response. Previous studies have evaluated clinical practice guidelines (CPGs) produced in response to epidemics or pandemics, yet few have focused on public health guidelines and recommendations. To address this gap, we assessed health systems guidance (HSG) produced by the WHO, the ECDC, and the CDC for the 20…

COVID-19 positive cases in a pediatric surgery department from Romania: Case series from 2 years of pandemics.

RATIONALE: The COVID-19 pandemic had a dramatic effect on various health systems in terms of admissions and outcomes, including pediatric surgery activity. The aim of this paper was to analyze the outcome of SARS-CoV-2-positive patients admitted to our department during the regional COVID-19 pandemic in North-Eastern Romania. We also evaluated the changes generated in our daily practice by the COVID-19 pandemic and the dynamic response to this major challenge. PATIENT CONCERNS: The patients presented with symptoms related to their primary diagnosis: local pain and deformity in case of fractur

Sexual and reproductive health services during outbreaks, epidemics, and pandemics in sub-Saharan Africa: a literature scoping review.

BACKGROUND: The COVID-19 pandemic could worsen adolescent sexual and reproductive health (ASRH). We sought evidence on the indirect impacts … y planning (n = 8 [38.1%]) outcomes. Although we sought out to assess all outbreaks, epidemics, and pandemics, the only relevant studies took place during the West African Ebola pandemic (n = 17 [80.9%]) and COVID-19 pandemic (n = 4 [19.0%]). One study (4.8%) highlighted adolescent-specific outcomes and condom use. Most studies found declined access to and …

A scoping review on psychosocial consequences of pandemics on parents and children: Planning for today and the future.

The COVID-19 worldwide pandemic has forced individuals into an unnatural way of life. Families with children experience unique stressors, such as school closures, disrupted childcare arrangements, requirement of parents to uptake additional responsibilities such as homeschooling, possible financial strain, and lack of breathing space between family members. The adjustments required of parents and children during a pandemic presumably impacts the psychosocial wellbeing of parents and children in different ways. To better under the psychosocial consequences of pandemics on parents, children, and…

Clinical Practice: Should we Radically Alter our Sedation of Critical Care Patients, Especially Given the COVID-19 Pandemics?

The high number of patients infected with the SARS-CoV-2 virus requiring care for ARDS puts sedation in the critical care unit (CCU) to the edge. Depth of sedation has evolved over the last 40 years (no-sedation, deep sedation, daily emergence, minimal sedation, etc.). Most guidelines now recommend determining the depth of sedation and minimizing the use of benzodiazepines and opioids. The broader use of alpha-2 adrenergic agonists ('alpha-2 agonists') led to sedation regimens beginning at admission to the CCU that contrast with hypnotics+opioids ("conventional" sedation), with major consequen

Microorganisms as Shapers of Human Civilization, from Pandemics to Even Our Genomes: Villains or Friends? A Historical Approach.

Universal history is characterized by continuous evolution, in which civilizations are born and die. This evolution is associated with multiple factors, among which the role of microorganisms is often overlooked. Viruses and bacteria have written or decisively contributed to terrible episodes of history, such as the Black Death in 14th century Europe, the annihilation of pre-Columbian American civilizations, and pandemics such as the 1918 Spanish flu or the current COVID-19 pandemic caused by the coronavirus SARS-CoV-2. Nevertheless, it is clear that we could not live in a world without these …

Historical Y. pestis Genomes Reveal the European Black Death as the Source of Ancient and Modern Plague Pandemics.

Ancient DNA analysis has revealed an involvement of the bacterial pathogen Yersinia pestis in several historical pandemics, including the second plague pandemic (Europe, mid-14(th) century Black Death until the mid-18(th) century AD). Here we present reconstructed Y. pestis genomes from plague victims of the Black Death and two subsequent historical outbreaks spanning Europe and its vicinity, namely Barcelona, Spain (1300-1420 cal AD), Bolgar City, Russia (1362-1400 AD), and Ellwangen, Germany (1485-1627 cal AD). Our results provide support for (1) a single entry of Y. pestis in Europe during …

The pandemic gap of respiratory viruses during the COVID-19 pandemic.

UNLABELLED: Respiratory viruses typically exhibit seasonal patterns, posing ongoing public health challenges. The coronavirus disease 2019 pandemic altered these patterns dramatically, with many common respiratory viruses disappearing from circulation for extended periods. Here we analyzed over three million diagnostic tests from a metropolitan healthcare system in New York City over 7 years, tracking severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and eight common respiratory viruses before and during the pandemic. Following the initial SARS-CoV-2 wave in the spring of 2020, infl

The pandemic body: the lived body during the COVID-19 pandemic.

In this study, we conduct a detailed analysis of qualitative survey data focusing on adult populations in the UK, Japan and Mexico to address the following question: How has the COVID-19 pandemic changed people's lived experience of their bodies, other people's bodies and the world? We identify five themes: (i) fear and danger, (ii) bodily doubt and hypervigilance, (iii) risk and trust, (iv) adapting and enduring and (v) changes in perspective. We use two theoretical frameworks: first, Mary Douglas' anthropological work on purity, risk, danger and symbolism is applied to understand how social

Insights into the monkeypox virus: Making of another pandemic within the pandemic?

Just when the world started to adapt to the 'new normal' amid the coronavirus disease 19 (COVID-19) pandemic, the world is witnessing the wrath of another viral disease, the monkeypox virus (MPXV). The virus is endemic to African countries, where several outbreaks have been reported in the past. However, the present cases have been reported in non-endemic countries worldwide. Although MPX is considered to be a self-limiting disease, recent reports on its incidence have proved otherwise. The 2022 multi-country MPX outbreak has drawn the attention of global surveillance organizations and epidemi

Preparing for Pandemics with Large Language Models: An Evaluation of Sensitivity Across COVID-19, Zika, and Monkeypox Case Reports.

Large language models (LLMs) have emerged as potential tools for early disease characterization and pandemic preparedness due to their ability to interpret complex textual data. This study evaluated the sensitivity of three LLMs: GPT-5, Claude Sonnet 4, and Gemini 2.5 Pro on early case reports of COVID-19, Mpox, and Zika. Each case report was modified to remove explicit diagnostic terms, and models were prompted to identify whether the presentation represented a disease of pandemic potential. Claude Sonnet 4 achieved the highest sensitivity overall across all three diseases. GPT-5 demonstrated

Delivering physical rehabilitation services during the COVID-19 pandemic in Iran: Common challenges and potential solutions for future pandemics.

The COVID-19 pandemic affected people with disabilities in different aspects, including their access to physical rehabilitation (PR). Despite a significant global surge in the need for PR, it has not been prioritized and is under-resourced, even in the non-COVID era, in many countries. This study aimed to explore the challenges of delivering PR services during the COVID-19 pandemic in Iran and potential solutions to these challenges. This qualitative study was done using a thematic approach from November 2023 to March 2024 in Iran. 45 PR professionals and faculty members from three fields-phys

Integrating Western Medicine With Japanese Traditional Medicine for Primary Health Care During Natural Disasters and Pandemics: Lessons From Japan.

Natural disasters and pandemics create major challenges for public health systems, requiring efficient medical responses under constrained conditions. Ensuring comprehensive healthcare in disaster-stricken areas is critical, as conventional medical resources may be insufficient in some settings. Japanese traditional (Kampo) medicine (JTM) offers a potential solution for enhancing healthcare delivery in these scenarios. This work was motivated by the desire to facilitate the international dissemination of the integration of Western medicine and traditional medicine. Japan has integrated JTM, in…

An Unwanted but Long-Known Company: Post-Viral Symptoms in the Context of Past Pandemics in Switzerland (and Beyond).

Objectives: Some people do not fully recover from an acute viral infection and experience persistent symptoms or incomplete recovery for months or even years. This is not unique to the SARS-CoV-2 virus and history shows that post-viral conditions like post COVID-19 condition, also referred to as Long Covid, are not new. In particular, during and after pandemics caused by respiratory viruses in which large parts of the population were infected or exposed, professional and public attention was increased, not least because of the large number of people affected. Methods: Given the current relevan…

Call for a Global Vaccine Plan to Combat Current and Future Pandemics: One for ALL and ALL for One.

The emergence of SARS-CoV-2 in late December 2019 has taken the world by storm. In March 2020, the World Health Organization (WHO) named this virus COVID-19. To date, it has infected approximately 186 million people worldwide and is attributed as the cause of death of more than 5 million people (and this number is only increasing.) The global effort to develop vaccines and therapeutics occurred at the fastest pace yet, with several vaccines' approval under emergency authorization use. There are also several post-marketing side effects, including myocarditis, cerebral venous embolism, and Guill

Possible Roles of Permafrost Melting, Atmospheric Transport, and Solar Irradiance in the Development of Major Coronavirus and Influenza Pandemics.

Major pandemics involving respiratory viruses develop semi-regularly and require a large flux of novel viruses, yet their origination is equivocal. This paper explores how natural processes could give rise to this puzzling combination of characteristics. Our model is based on available data regarding the emergence of historic influenzas, early COVID-19 cases and spreading, the microbiome of permafrost, long-distance airborne transport of viruses reaching stratospheric levels, ultraviolet immunosuppression, sunlight variations, weather patterns, Arctic thawing, and global warming. Atmospheric c…
