A great deal of information has been accumulated on the synthesis and release of AVP, oxytocin, and their associated neurophysins under normal circumstances. In 1957, Schwartz and Bartter first described SIAD in patients with lung cancer and postulated that the clinical findings were the results of excessive vasopressin secretion. Tumors have been known since 1964 to produce vasopressin, and small cell (oat cell) carcinoma of the lung is by far the most frequent malignant cause of SIAD. The biosynthetic pathway for the synthesis of AVP and its associated neurophysin (and to a lesser extent, ox

One autopsy case with small cell lung cancer and the syndrome of inappropriate antidiuretic hormone secretion (SIADH) is reported. Both plasma atrial natriuretic peptide (ANP) and arginine vasopressin (AVP) levels were high, and the presence of significantly high levels of ANP and AVP in tumor tissue was confirmed by gel chromatography and radioimmunoassay techniques. To the best of the authors' knowledge, this is the first case in which ectopic ANP production and its secretion into blood (leading to SIADH) were proved.

An autopsy case of disseminated, military-type, composite adenocarcinoma and carcinoid tumor associated with adrenocorticotropic syndrome is discussed. Prostatic origin and functional activity were demonstrated by immunohistochemical techniques. Of interest in this case is that the tumor had the characteristics of amine precursor uptake and decarboxylation (APUD) cells and also kept the antigenicity of its prostatic origin. The APUD system perhaps should be considered as one form of functional differentiation of cells of different origin rather than an exclusive derivative of the neural crest.

Tumor metastasis to the posterior pituitary-hypothalamic area rarely is expressed by either clinical or radiographic evidence. A case is reported of widespread metastic ACTH secreting bronchogenic carcinoma presenting to the clinician as diabetes insipidus. This type of tumor associated with diabetes insipidus is apparently rare, as is antemortem radiographic documentation of metastasis to the pituitary--hypothalamic region. A review of the ten-year experience of Loyola University Medical Center and Hines VA Hospital with the association of bronchogenic carcinoma and diabetes insipidus is pres

Excessive production of adrenocorticotrophic hormone (ACTH) by oat cell carcinomas and carcinoid tumours of the lung has been well documented. Florid cases of Cushing's syndrome secondary to the excessive production of ACTH by the tumour are rare. In this article 2 cases of florid Cushing's syndrome associated with oat cell carcinoma are described.

Endocrinological symptoms of, on the one hand, hypothalamic-hypophyseal Cushing's syndrome and, on the other, ectopic ACTH syndrome (usually caused by bronchial carcinoma) can often not be distinguished. Reliable differentiation of these two forms of secondary hypercortisolism is necessary, because in hypothalamic-hypophyseal Cushing's syndrome therapy is more and more frequently directed at the anterior pituitary and hypothalamus rather than attempted by bilateral adrenalectomy. The following method was used for distinguishing between the two in six patients: blood was withdrawn by catheter f

A young man presented with rapidly developing Cushing's syndrome which was due to the ectopic secretion of ACTH and beta-MSH-like material from hepatic tumour deposits, possibly originating from biliary radicals. This association of the ectopic ACTH syndrome has not previously been described. During the 22 month course of the illness the plasma immunoreactive ACTH and 'beta-MSH' concentrations rose by logarithmic progression. The plasma calcitonin concentration was also raised but did not change during the last 12 months. At any stage of the illness the plasma concentration of the ecotopically

An unusual case of bihormonal activity by squamous cell carcinoma of the esophagus is presented. It is a rare presentation of esophagus carcinoma because of the following reasons: 1. Synthesis of cortisteroids by esophageal carcinoma. 2. Pseudohyperparathyroidism caused by the same tumor. 3. Presentation of hypokalemic alkalosis as the initial manifestation and lack of specific gastrointestinal symptomatology of esophageal carcinoma.

Three patients with Cushing syndrome secondary to ectopic adrenocorticotropic hormone (ACTH) production underwent direct 22-gauge needle aspiration of bronchial (two cases) and mediastinal (one case) carcinoid tumors. High levels of ACTH were measured in all three tumors. This technique permits absolute identification of an ectopic source of ACTH before surgery.

In a 53-year-old female patient with typical clinical Cushing's syndrome increased blood sugar levels, hypokalaemia, increased non-dexamethasone suppressable plasma cortisol levels and increased ACTH values in serum were observed. CAT scanning revealed bilateral adrenocortical hyperplasia. The endocrine anterior pituitary function was normal. Radiographically and bronchoscopically a rightsided parahilar tumour was found. Histology showed an atypical metastatic bronchial carcinoid with ectopic ACTH production. Conservative treatment with metyrapone suppressed the Cushing syndrome. The ACTH prod

Twenty-three patients with adrenocorticotrophic hormone-(ACTH)-dependent Cushing's syndrome were subjected to selective venous catheterisation and sampling for ACTH on a total of 26 occasions. Out of 10 patients with pituitary-dependent disease, nine had raised ACTH concentrations in one or both high internal jugular vein samples. Eight patients had 11 proved sites of ectopic hormone production: of these, six were correctly identified by the sampling technique, and in four of them this was the only accurate method of localisation. The results of one catheterisation were misleading, and on 10 o

This report documents the simultaneous occurrence of Cushing's and Zollinger-Ellison syndromes in a patient with islet cell carcinoma. The clinical concurrence of these two syndromes has been recorded in only three previous case studies, while three other case reports record evidence suggesting the presence of both gastrin and ACTH within the tumor but without clinical sequelae. In the present report, evidence based on multiple clinical and biochemical data supports the concept that both syndromes are a result of "ectopic" hormone production by the tumor.

DNA hybridization techniques are now commonly used in studies of endocrine disease. Although solution and filter hybridization have been more widely used in endocrine studies, ISH offers many advantages to the pathologist. Individual cells can be visualized directly with the microscope to study the expression of specific gene products. ISH is somewhat similar to immunohistochemistry and can be set up rapidly in most histology laboratories, especially when nonradioactive biotinylated probes are used. The ability to combine ISH with immunochemical techniques offers to the pathologist the use of

BACKGROUND: Small-cell prostate cancer is the most common type of treatment-related neuroendocrine prostate cancer. It typically originates from prostate adenocarcinoma following prolonged endocrine therapy, presenting as desmoplasia-resistant. At this stage, indicators such as prostate-specific antigen (PSA) and androgen receptor (AR) remain within normal ranges, conflicting with widespread metastatic lesions. Simultaneously, neuroendocrine indicators exhibit a significant elevation. Small-cell prostate cancer can secrete hormones, leading to ectopic endocrine syndrome.

Metastatic pheochromocytomas and paragangliomas (MPPGs) are rare neuroendocrine tumors. Most patients present with advanced disease that is associated with manifestations of catecholamine release. Surgical resection of the primary tumor and ablative therapies of metastases-whenever possible-may improve clinical outcomes and, perhaps, lengthen the patient's overall survival. Significant steps in understanding the genetic alterations linked to MPPGs and scientific progress made on cancers that share a similar pathogenesis are leading to the recognition of potential systemic therapeutic options.
