[Primary peritonitis caused by Streptococcus pneumoniae].

Primary peritonitis caused by Streptococcus pneumoniae is a rare but serious complication of childbirth. We present here three cases of young women who developed abdominal pain after childbirth. All of the patients had fever with abdominal pain, diarrhea and clinical signs of peritonitis. In two cases a laparotomy was performed to remove pus. Cultures taken were positive for Streptococcus pneumoniae. Culture of vaginal swabs and blood cultures were also positive for the same pathogen. For the third patient, both vaginal swabs and blood cultures were positive for Streptococcus pneumoniae, antib…

Tuberculous peritonitis--do not miss it.

UNLABELLED: Incidence of tuberculosis is sharply rising in the United States, and tuberculous peritonitis is often diagnosed late in the course of the disease, resulting in undue patient morbidity and mortality. PURPOSE: Purpose of this study was to better identify which clinical, laboratory, radiologic, and invasive procedures were most useful in diagnosing tuberculous peritonitis. METHODS: All cases of tuberculous peritonitis diagnosed between 1982 and 1994 were reviewed retrospectively to discern which laboratory, radiographic, and procedural tests were helpful in diagnosing the condition…

Small bowel myoelectric activity in peritonitis.

Peritonitis is associated clinically with paralytic ileus, but the physiologic mechanisms of the effects of peritonitis on bowel myoelectric activity have not been explored. Bipolar electrodes were inserted into the rats, and myoelectric control recordings were obtained for 4 h/d for 5 consecutive days. Peritonitis was then induced, and myoelectric recordings were again obtained. Each animal served as its own control. Prior to induction of peritonitis (control), phase I, II, and III myoelectric activity was present in all recordings. The cycle duration of the migrating myoelectric complex was …

Tuberculous peritonitis presenting as coloenteric fistula.

Abdominal tuberculosis can present as tuberculous peritonitis with generalised or loculated ascites, lymphadenitis of the mesenteric or retroperitoneal nodes, or as intestinal tuberculosis. In the last case it could be annular mucosal ulceration, single or multiple strictures or hyperplastic tuberculosis of the bowel, usually involving the terminal ileum or ileocaecal region. Disease higher in the ileum is considered common but involvement of the appendix, stomach, duodenum, jejunum, colon and rectum is unusual. Tuberculous peritonitis presenting as a coloenteric fistula is not a common clinic…

Cryptococcal peritonitis in a CAPD patient.

A 50-year-old diabetic woman with end-stage renal disease, who had been on continuous ambulatory peritoneal dialysis for 8 months, developed peritonitis caused by Cryptococcus neoformans var. neoformans. The patient was completely asymptomatic and infection was confirmed by detection of budding yeast cells in Gram-stained smears of turbid peritoneal fluid. The infection was cleared after intravenous fluconazole with delayed removal of the catheter. Fluconazole may be a suitable alternative drug in treating cryptococcal peritonitis.

Novel approach to iatrogenic bile peritonitis.

Bile peritonitis after injury to the biliary tree is a serious complication that requires exploratory laparotomy. Our patient had an obstructing ampullary carcinoma, and generalized bile peritonitis developed from attempted percutaneous transhepatic cholangiography. The patient's condition was managed by peritoneal lavage and endoscopic transampullary stenting, with immediate relief of pain and toxicity. Exploratory laparotomy was avoided, and an eventual pylorus-sparing Whipple resection was the definitive treatment. We believe this to be the first report of successful nonoperative treatment …

Listeria monocytogenes peritonitis associated with CAPD.

Listeria monocytogenes peritonitis developed in a 67-year-old man on continuous ambulatory peritoneal dialysis following a catered luncheon. Alcoholic liver disease was a predisposing factor. L. monocytogenes multiplies at 4 degrees C. It is often present in imported soft cheese, less often in chicken and other refrigerated food. Listeria peritonitis has not been previously reported in Australia.

Idiopathic sclerosing peritonitis in a man.

Idiopathic sclerosing peritonitis is a rare disease described in young adolescent women, characterized by fibrosis and adhesions of the peritoneum to loops of the small bowel. Here we describe a 35-year-old man who underwent exploratory laparotomy for repeated small bowel obstruction. Only partial resection of the terminal ileum was possible because of adhesions; recurrent abdominal infections and leakage from anastomosis required further resection, which ultimately resulted in short bowel syndrome and malabsorption. The clinical and pathological findings were characteristic for idiopathic scl…

[Biliary peritonitis caused hy hydatid disease].

After specifying that rupture of a hydatid cyst in the peritoneal cavity is one of the most serious and rarest complications of hepatic hydatidosis, attention is paid to the peritoneal hydatid (o chole-hydatid). The aetiopathogenetic mechanisms and the anatomo-pathological features of this type of lesion are described in brief. Finally, the personally employed diagnostic protocol and therapeutic strategy in the face of this feared complication are reported.

Primary peritonitis. An unusual operative diagnosis.

Primary peritonitis, or spontaneous bacterial peritonitis, is a highly morbid and often fatal complication of cirrhosis and other conditions associated with ascites. Prompt antibiotic therapy may be lifesaving, as may early surgical intervention in patients who have signs and symptoms of an acute abdomen. During a 5-year period, 12 patients had 14 episodes of primary peritonitis diagnosed in our hospital. Three patients had exploratory laparotomy, and gram-positive organisms were obtained from peritoneal fluid in two patients. The clinical features, patho-physiology, and natural courses of the…

[Therapy of postoperative ileus (including peritonitis)].

In each case of postoperative ileus initial decompression of the intestine by an endoscopically placed tube is indicated (success rate 80%). With clear mechanical obstruction or diffuse peritonitis immediate relaparotomy is indicated. With prolonged gastrointestinal dilatation or paralytic ileus conservative therapy (tube-decompression, substitution of fluids and electrolytes, peristaltics, sympathicolysis with following stimulation) and close observation by an experienced surgeon is recommended. With local peritonitis (intraabdominal abscess) percutaneous puncture and drainage, sometimes just…

[Local noncircumscribed peritonitis in acute appendicitis].

An analysis of 280 clinical observations of acute appendicitis complicated by local noncircumscribed peritonitis was performed. It was shown that the pathology was characterized by more acute clinical course, later time of operative interventions, greater amount of postoperative complications and longer staying at the hospital. In the treatment of appendicitis complicated by local peritonitis main attention must be given to sanitation of the abdominal cavity and complex postoperative management after appendectomy.

[Role of ileostomy in typhoid peritonitis].

Diffuse peritonitis secondary to ileal perforation due to typhoid fever are serious and frequent in our country. 42 cases are reported. In 85% the perforation is on the end of the ileum between 10 to 50 cm from the Bauhin's valve. In 32 cases there is only one perforation. 8 patients underwent 50 cm resection of the end of the ileum with terminal ileostomy: one died. One case underwent closure. The washing out of peritoneum is done in 15 cases. The covery of the continuity is usually executed 3 months later by an iliac surgical approach in preference.
