Common Peroneal Nerve and Tarsal Tunnel Release Surgery in an Adolescent Male with Hunter Syndrome: Illustrative Case.

BACKGROUND: Children with Hunter syndrome have a high prevalence of nerve compression syndromes given the buildup of glycosaminoglycans in the tendon sheaths and soft tissue structures. These are often comorbid with orthopedic conditions given joint and tendon contractures due to the same pathology. While carpal tunnel syndrome and surgical treatment has been well-reported in this population, the literature on lower extremity nerve compression syndromes and their treatment in Hunter syndrome is sparse. OBSERVATIONS: We report the case of a 13-year-old male with a history of Hunter syndrome wh

Thread common peroneal nerve release-a cadaveric validation study.

OBJECTIVE: To determine if the thread release technique can be applied to common peroneal nerve entrapment at the fibular neck. METHODS: The thread common peroneal nerve release was performed on 15 fresh frozen cadaveric lower extremity specimens. All procedures were performed under ultrasound guidance and immediately underwent post-procedural gross anatomic inspection for completeness of decompression and presence or absence of iatrogenic neurovascular injury. RESULTS: All 15 specimens demonstrated complete transection of the deep fascia of the peroneus longus overlying the common peroneal

Quantitative assessment of nerve echogenicity as an additional tool for evaluation of common fibular neuropathy.

OBJECTIVE: Quantitative assessment of nerve echogenicity using the MaxEntropy, RenyiEntropy and Yen methods has shown promise in the assessment of ulnar neuropathy at the elbow. The purpose of this study was to determine the clinical usefulness of quantitative echogenicity measurement when using these three automatic thresholding methods in the evaluation of patients with common fibular (CF) neuropathy. METHODS: High-resolution ultrasonography (HRUS) images of the CF nerve from 45 patients with CF neuropathy were compared to 42 healthy controls. RESULTS: With all three methods patients with

Peroneal nerve injury associated with sports-related knee injury.

OBJECT: This study analyzes 84 cases of peroneal nerve injuries associated with sports-related knee injuries and their surgical outcome and management. METHODS: The authors retrospectively reviewed the cases of peroneal nerve injury associated with sports between the years 1970 and 2010. Each patient was evaluated for injury mechanism, preoperative neurological status, electrophysiological studies, lesion type, and operative technique (neurolysis and graft repair). Preoperative status of injury was evaluated by using a grading system published by the senior authors. All lesions in continuity

Modified Lambrinudi Arthrodesis for the Acute Treatment of Neurogenic Clubfoot: A Case Report.

UNLABELLED: Neurogenic contracture often results in spastic, nonreducible equinovarus deformity. Rigid contracture leads to pain, instability, and bracing difficulties. This case report details the utilization of the modified Lambrinudi triple arthrodesis intended to create a plantigrade, functional limb that is amenable to an extremity brace in a case of an acquired neurologic clubfoot. LEVELS OF EVIDENCE: Therapeutic, Level IV: Case Report.

Sudden peroneal nerve palsy in a varus arthritic knee.

Peroneal nerve palsy has been reported in association with traumatic and nontraumatic causes. We encountered a 75-year-old man whose peroneal nerve palsy developed suddenly following varus deformity of the arthritic knee. A review of the literature found 1 other report describing a progressive peroneal nerve palsy associated with a varus deformity of the knee due to arthritis. Our patient had progressive intractable knee pain; 3-compartment, severe degenerative changes in the knees; varus knee malalignment and laxity; right peroneal nerve palsy; and decreased sensation to light touch and pinpr

Thickening of Dorsal Foot Nerves: A Frequent Sonographic Finding in Asymptomatic Volunteers, Potentially Leading to False Positive Results.

Objectives: Compression neuropathies such as Anterior Tarsal Tunnel Syndrome are usually associated with focal thickening at the compression site. This study aimed to determine the frequency and location of thickenings of dorsal foot nerves in asymptomatic, healthy volunteers. We hypothesized that focal nerve thickening of dorsal foot nerves is a frequent finding in asymptomatic individuals and occurs at anatomically plausible locations, potentially limiting the specificity of ultrasound in the diagnosis of anterior tarsal tunnel syndrome. Materials and Methods: In this prospective study, the

Mechanomyography as a Surgical Adjunct for Treatment of Chronic Entrapment Neuropathy: A Case Series.

BACKGROUND: Chronic entrapment neuropathy results in a clinical syndrome ranging from mild pain to debilitating atrophy. There remains a lack of objective metrics that quantify nerve dysfunction and guide surgical decision-making. Mechanomyography (MMG) reflects mechanical motor activity after stimulation of neuromuscular tissue and may indicate underlying nerve dysfunction. OBJECTIVE: To evaluate the role of MMG as a surgical adjunct in treating chronic entrapment neuropathies. METHODS: Patients 18 years or older with cubital tunnel syndrome (n = 8) and common peroneal neuropathy (n = 15) w

Transcranial electric motor evoked potential detection of compressional peroneal nerve injury in the lateral decubitus position.

The peroneal nerve is susceptible to injury due to compression at the fibular head for patients placed in the lithotomy, hemilithotomy or lateral decubitus positions during surgery. Upper extremity somatosensory and transcranial electric motor evoked potential monitoring has proven efficacious for identifying impending positional brachial plexopathy or upper extremity peripheral neuropathy in adult and pediatric patients undergoing spine surgery. We report on two cases to illustrate the usefulness of monitoring transcranial electric motor evoked potentials recorded from tibialis anterior muscl

Acute intermittent porphyria with peripheral neuropathy: a follow-up study after hematin treatment.

We report a patient with acute intermittent porphyria who presented with progressive motor neuropathy, particularly in the upper limbs. The electrophysiological studies showed an asymmetric motor neuropathy with a prominent involvement of both the radial and left peroneal nerves. During the 1-year follow-up period, 6 courses of hematin infusion, with 150 mg daily for 4 consecutive days every month, were administrated. The motor neuropathy showed a steady and gradual improvement following the hematin treatment. Molecular analysis of the porphobilinogen deaminase gene revealed a short segment de

Cerebral infarction presenting with unilateral isolated foot drop.

Weakness of the dorsiflexor muscles of the ankle or toe, referred to as foot drop, is a relatively common presentation. In most cases, foot drop is caused by a lower motor neuron disease such as peroneal peripheral neuropathy, L4-5 radiculopathic sciatic neuropathy, or polyneuropathy. Although upper motor neuron lesions can present as foot drop, the incidence is very rare. Here, we report an extremely rare case in which foot drop was the only presenting symptom of cerebral infarction.

Somatosensory and brainstem auditory evoked potentials in neurodegenerative system disorders.

Brainstem auditory (BAEP) and somatosensory (SEP) evoked potentials to median and peroneal nerve stimulation were investigated in 25 patients with neurodegenerative system disorders: 9 Friedreich's ataxia, 7 hereditary motor sensory neuropathies, 3 familial spastic paraplegia, 3 olivopontocerebellar atrophy, 1 ataxia telangiectasia and 1 abetalipoproteinemia. BAEPs were abnormal in 39%, SEPs to both upper- and lower-limb stimulation were abnormal in 63%. Serial evoked potential testing paralleled the clinical progression. SEPs were more frequently and severely altered than BAEPs suggesting tha

Common peroneal nerve palsy associated with pelvic surgery for cancer. An analysis of 11 cases.

Eleven occurrences of common peroneal palsies following pelvic surgery for malignant conditions are reported. The patients' clinical course and possible mechanisms of nerve injury were reviewed. It was concluded that the current belief that all peripheral neuropathies occurring under general anesthesia are preventable may not be applicable to patients with pelvic cancer who must undergo tedious, lengthy, meticulous, extirpative surgery in the dorsal lithotomy position. In patients with tumors that are seemingly isolated to the pelvis, amenable for surgical resection, the possible risk of a per

Charcot-Marie-Tooth's disease with severe trophic and sensory disorders. Study of a case following along a half a century with anatomical studies.

Plantar ulcers of neurological origin are known to be associated with several disorders, some of which may be familial. We present the case report together with the clinical and post-mortem data of a patient with peroneal muscular atrophy of the Charcot-Marie-Tooth type and plantar ulcers as a prominent feature. At least three other members of her family had Charcot-Marie-Tooth disease. The family pedigree is included. The discussion stresses the difference between patients with Charcot-Marie-Tooth disease with associated manifestations, bony destruction of the feet and plantar ulcers and pati

The nearly invisible intraneural cyst: a new and emerging part of the spectrum.

OBJECTIVE The authors have observed that a subset of patients referred for evaluation of peroneal neuropathy with "negative" findings on MRI of the knee have subtle evidence of a peroneal intraneural ganglion cyst on subsequent closer inspection. The objective of this study was to introduce the nearly invisible peroneal intraneural ganglion cyst and provide illustrative cases. The authors further wanted to identify clues to the presence of a nearly invisible cyst. METHODS Illustrative cases demonstrating nearly invisible peroneal intraneural ganglion cysts were retrospectively reviewed and are

An interesting cause of wrist drop: The crow position in yoga and hereditary neuropathy with liability to pressure palsies.

Hereditary neuropathy with liability pressure palsies (HNPP) is usually caused by compression and is an episodic, painless, recurrent hereditary neuropathy with focal motor and sensory involvement. It begins in adolescence and young adulthood. The most commonly affected nerves in HNPP are the ulnar, peroneal, radial, and median nerves. In this article, we present a 31-year-old female patient with a previously undescribed case of HNPP, which presented with wrist drop due to the trapping of the radial nerve in the spiral groove after the crow position in yoga.

Piriformis syndrome surgery causing severe sciatic nerve injury.

Piriformis syndrome is a controversial entrapment neuropathy in which the sciatic nerve is thought to be compressed by the piriformis muscle. Two patients developed severe left sciatic neuropathy after piriformis muscle release. One had a total sciatic nerve lesion, whereas the second had a predominantly high common peroneal nerve lesion. Follow-up studies showed reinnervation of the hamstrings only. We conclude that piriformis muscle surgery may be hazardous and result in devastating sciatic nerve injury.

Intra-articular Entrapment of the Common Peroneal Nerve in a Multiligamentous Knee Injury: A Case Report.

CASE: A 25-year-old severely obese woman sustained a knee injury with foot drop after an electric scooter crash. Initially diagnosed with a knee sprain and advised to follow-up with her primary care physician, she was later diagnosed with chronic multiligamentous knee instability and complete common peroneal nerve (CPN) palsy. Surgery 7 months postinjury demonstrated a severed intra-articular CPN scarred to the anterior cruciate ligament stump, necessitating arthroscopic-assisted extrication. CONCLUSION: Clinicians should maintain high suspicion for ligament injury and peroneal palsy after lo
