Phantom pain in a patient with brachial plexus avulsion injury.

OBJECTIVE: Phantom limb pain is a painful sensation perceived in the absent limb following surgical or traumatic amputation. Phantom limb sensations, which are nonpainful, occur in nearly all amputees. Deafferentation can also produce similar symptoms. Here we report the presence of phantom pain in a deafferented limb. DESIGN: Case report. SETTING: Hospital-based outpatient clinic. PATIENT: A 65-year-old man was referred to the pain clinic for management of upper extremity pain secondary to brachial plexus avulsion (BPA) following a motor vehicle accident. Initially he noticed a feeling of …

Pain in traumatic upper limb amputees in Sierra Leone.

Data on 40 upper limb amputees (11 bilateral) with regard to stump pain, phantom sensation and phantom pain is presented. All the patients lost their limbs as a result of violent injuries intended to terrorise the population and were assessed 10-48 months after the injury. All amputees reported stump pain in the month prior to interview and ten of the 11 bilateral amputees had bilateral pain. Phantom sensation was common (92.5%), but phantom pain was only present in 32.5% of amputees. Problems in translation and explanation may have influenced the low incidence of phantom pain and high inciden

[Sympathetic reflex dystrophy and phantom pain. Diagnosis, therapy and prognosis].

The incidence of phantom limb pain has been significantly underestimated for many years. However, studies published during the recent decade indicate that the real incidence of phantom limb pain may be between 60% and 90%. Reflex sympathetic dystrophy (RSD) occurs with an incidence of about 15.000 new cases every year in Germany. Both diseases show early centralisation and chronification. Hence, only early diagnosis and onset of correct therapy in time provide significant pain reduction. When therapy is started too late, prognosis in regard to sufficient pain reduction is poor. Phantom limb pa…

Phantom Sensations Following Brachial Plexus Nerve Block: A Case Report.

Following the administration of brachial plexus anesthesia for right thumb carpometacarpal arthroplasty with ligament reconstruction, a 54-year-old woman with all limbs intact developed phantom limb sensations, including the misperception of the placement of her right arm and frozen limb sensations in her fingers. Immobility of her fingers in a stacked position was experienced for ~3.5 days after surgery, and she described her phantom sensations as the hand experiencing "tingling" and feeling "heavy." While the onset of these phantom sensations occurred almost immediately after administration …

Classification of Phantom Finger, Hand, Wrist, and Elbow Voluntary Gestures in Transhumeral Amputees With sEMG.

Decoding finger and hand movements from sEMG electrodes placed on the forearm of transradial amputees has been commonly studied by many rese … tiple electrodes placed on the residual upper arm of five transhumeral amputees with a controllable phantom limb who did not undergo any reinnervation surgery. We showed that with a state-of-the-art classification architecture, it is possible to correctly classify phantom limb activity (up to 14 movements) with a rather important average success (over 80% if c …

[Technological advances in amputation medicine].

Amputation medicine has evolved into a specialized, technology-driven discipline. With increasing global amputation rates, advanced surgical techniques, innovative prosthetic solutions and digital rehabilitation are gaining importance. Transcutaneous osseointegrated prosthetic systems (TOPS) can improve mobility, proprioception and quality of life but carry risks such as stomal infections or periprosthetic fractures. Neuroreconstructive procedures including targeted muscle reinnervation (TMR), targeted sensory reinnervation (TSR), regenerative peripheral nerve interface (RPNI), agonist-antagon

[Postoperative treatment of phantom pain and causalgias with calcitonin].

Results of IV calcitonin treatment in patients suffering from postoperative phantom limb pain (n = 12) or causalgia following peripheral nerve lesions (n = 4) are reported. All patients were complained of severe pain after a traumatic event or amputation, with disturbed sleep in many cases. After only 1-2 infusions 10 patients with phantom limb pain (83%) were discharged from hospital pain-free. Pain was effectively reduced by up to 5 infusions in 2 patients (17%). A follow-up for maximally 24 months showed a recurrence of pain in only 4 patients with obvious stump problems or reamputations. T…

Assessment of reorganization in the sensorimotor cortex after upper limb amputation.

OBJECTIVE: We wanted to investigate plastic changes occurring in the motor and somatosensory cortex after upper limb amputation, and their possible relationship to phantom pain. METHOD: To assess these plastic changes, we used transcranial magnetic stimulation (TMS) and source localization of somatosensory evoked potentials (SEP). Eleven patients with upper limb amputation were investigated. The phantom pain intensity was assessed by visual analogue scaling (VAS). RESULTS: Using TMS mapping, we found a significant lateralization of the amplitude-weighted centre of gravity (P<0.01) and an enl

Hand-to-Face Remapping But No Differences in Temporal Discrimination Observed on the Intact Hand Following Unilateral Upper Limb Amputation.

Unilateral major limb amputation causes changes in sensory perception. Changes may occur within not only the residual limb but also the inta … ithin the brain that occur due to an amputation. Eight of 19 amputees (42.1%) experienced RS in the phantom limb with manual tactile mapping on various regions of the face. There was no correlation between whether someone had phantom sensations or phantom limb pain and where RS was found. Six of the amputees had either phantom sensation or pain in addition to …

Graded motor imagery for pathologic pain: a randomized controlled trial.

BACKGROUND: Phantom limb and complex regional pain syndrome type 1 (CRPS1) are characterized by changes in cortical processing and organization, perceptual disturbances, and poor response to conventional treatments. Graded motor imagery is effective for a small subset of patients with CRPS1. OBJECTIVE: To investigate whether graded motor imagery would reduce pain and disability for a more general CRPS1 population and for people with phantom limb pain. METHODS: Fifty-one patients with phantom limb pain or CRPS1 were randomly allocated to motor imagery, consisting of 2 weeks each of limb later…

PURPOSE: The effects of scanning parameters such as CT system performance, CT bed geometry, and upper limb position on effective diameter (ED) and water equivalent diameter (WED) have not been assessed. The purpose of this study was to compare both ED and WED obtained with various CT systems with theoretical values and to assess their accuracy. METHODS: Jaszczak cylindrical phantom (Data Spectrum, Durham, NC, USA), NEMA IEC body phantom (AcroBio, Tokyo), and thoracic bone phantom were used in this study with and without upper limb phantom. Voluntary phantom hand and finger movements in transhumerai amputees could be used to naturally control polydigital prostheses.

An arm amputation is extremely invalidating since many of our daily tasks require bi-manual and precise control of hand movements. Perfect h … prosthesis by using surface myoelectric activities of residual limb muscles (sEMG) associated with phantom limb movements, even if these residual arm muscles on which the phantom activity is measured were not naturally associated with hand movements before amputation. [Phantom perception and phantom pain].

"Phantom limb" is a well-known phenomenon following loss or amputation of a limb. The limb still seems to be present, although it is no longer there, but the sensation is not associated with pain. Less well-known are the phenomena of autosomatognosis and pain after the loss of other parts of the body such as the genital parts or after partial or complete deafferentiations as in a traumatic transverse syndrome, lesion of the brachial plexus, or lesions of the CNS. Although many hypotheses have been advanced to explain the pathophysiological mechanisms resulting in these phenomena, the pathogene…

Primary care incidence and treatment of four neuropathic pain conditions: a descriptive study, 2002-2005.

BACKGROUND: Between 1992 and 2001 the UK general practice incidence of post-herpetic neuralgia and trigeminal neuralgia declined, whilst the incidence of painful diabetic neuropathy increased. The most common first line treatments were compound analgesics. As therapeutic options have subsequently changed, this study presents updated data on incidence and prescribing patterns in neuropathic pain. METHODS: A descriptive analysis of the epidemiology and prescription treatment at diagnosis of incident post-herpetic neuralgia (n = 1,923); trigeminal neuralgia (1,862); phantom limb pain (57) and pa…

Artificial referred sensation in upper and lower limb prosthesis users: a systematic review.

Objective.Electrical stimulation can induce sensation in the phantom limb of individuals with amputation. It is difficult to generalize existing findings as there are many approaches to delivering stimulation and to assessing the characteristics and benefits of sensation. Therefore, the goal of this systematic review was to explore the stimulation parameters that effectively elicited referred sensation, the qualities of elicited sensation, and how the utility of referred sensation was assessed.Approach.We searched PubMed, Web of Science, and Engineering Village through January of 2022 to ident…

[Perioperative measures for prevention of phantom pain: an evidence-based approach to risk reduction].

Prevention of phantom limb pain is one of the biggest and still largely unsolved challenges in perioperative medicine. Despite many study efforts and optimization of postoperative pain treatment over the last 30 years, a significant reduction in the incidence of phantom limb pain has not been achieved. Current studies have also shown that at least 50% of patients develop phantom pain after 6 months. A possible approach could be to combine multiple synergistic interventions and implement them as a perioperative phantom pain management strategy bundle. In addition to regional anesthesia, NMDA an…

Hand prostheses with somatosensory feedback: functional and clinical benefits.

The loss of a hand can disrupt body movements, perception of self and the surrounding world, and the emotional fabric of touch. Advances in neuroprosthetics now make restoration of tactile, proprioceptive, and thermal sensations feasible through interfaces with peripheral nerves, the spinal cord, or skin. These somatosensory neuroprostheses not only improve the manipulation of objects and motor learning after amputation, but can also reduce phantom limb pain and enhance embodiment of the artificial limb. The combination of multiple sensory channels-such as touch and temperature-can resemble a …

Phantom pain: natural history and association with rehabilitation.

All 338 lower limb amputees under the care of one disablement services centre were asked to assess phantom pain severity at different times after amputation. Of 212 (63%) replies, 13 had died, 22 were non-limb wearers and 22 were unable to complete the questionnaire. In all, 176 useful replies were received--96 below-knee, 74 above-knee and 6 through-knee. Of these, 98 amputations were performed for trauma and 78 for vascular disease. Below-knee amputees rehabilitated better than above-knee amputees (P < 0.05) and traumatic better than vascular amputees (P < 0.0001). Preoperative pain was wors
