Rate advancement measurement for lung cancer and pleural mesothelioma in asbestos-exposed workers.

INTRODUCTION: Exposure to asbestos increases the risk of lung cancer and mesothelioma. Few studies quantified the premature occurrence of these diseases in asbestos-exposed workers. Focus on premature disease onset (rate advancement or acceleration) can be useful in risk communication and for the evaluation of exposure impact. We estimated rate advancement for total mortality, lung cancer and pleural mesothelioma deaths, by classes of cumulative asbestos exposure in a pooled cohort of asbestos cement (AC) workers in Italy. METHOD: The cohort study included 12 578 workers from 21 cohorts, with

Mesotheliomas and Benign Mesothelial Tumors: Update on Pathologic and Imaging Findings.

A diverse spectrum of benign entities and malignant neoplasms originate from the monotonous mesothelium that lines the serosal membranes of the pleural, pericardial, and peritoneal cavities. The mesothelium of myriad sites shows a common origin from the lateral plate mesoderm; primary mesothelial tumors thus demonstrate similar pathogenesis, imaging findings, and treatment options. Significant changes have been made in the 2021 World Health Organization (WHO) classification schemata of the pleural and pericardial tumors on the basis of recent advances in pathology and genetics. While malignant

Primary ovarian neuroendocrine neoplasia with concurrent large epithelial borderline ovarian tumor, coexistent with non-malignant pleural effusion and multiple uterine fibroids: a case report and review of the literature.

BACKGROUND: Neuroendocrine neoplasms are a group of neoplasms often originating from the neuroendocrine cells in the gastrointestinal tract, pancreas, and respiratory tract. Neuroendocrine neoplasms rarely occur in female reproductive organs and less than a hundred cases of ovarian high-grade lesions have been reported in the literature so far. Fewer still are cases reported in the literature associated with a borderline epithelial tumor in the same ovary. Owing to the rarity of the condition, there is a lack of specific guidelines for staging, and optimal management of these tumors. CASE PRE

A Narrative Review-Management of Malignant Pleural Effusion Related to Malignant Pleural Mesothelioma.

Malignant pleural mesothelioma (MPM) is an aggressive, almost universally fatal cancer with limited therapeutic options. Despite efforts, a real breakthrough in treatment and outcomes has been elusive. Pleural effusion with significant breathlessness and pain is the most typical presentation of individuals with MPM. Although thoracentesis provides relief of breathlessness, most such pleural effusions recur rapidly, and a definitive procedure is often required to prevent a recurrence. Unfortunately, the optimal treatment modality for individuals with recurrent MPM-related effusion is unclear, a

[Functional morphology of the pleura (author's transl)].

Peculiarities of topography, structure and injuries are responsible for the pathomorphological behaviour of the pleura. Its characteristical macro- and microscopic alterations are correlated with the typical cytological findings of effusions. Additionally to the pleural reactions in acute inflammation, the postpleuritic functional disturbances of pleura and lung are reported.

Histologic aspects in pleural diseases revealed by needle biopsy.

In 59 patients, 18 to 82 years old, with pleural exudates, a number of 78 needle biopsies were performed in view of establishing the etiologic diagnosis. The diagnosis was: pleural tuberculosis in 9 cases, acute or chronic non-tuberculous inflammation in 8 cases, postinfectious sequelae in 6 cases, secondary malignant neoplasia in 10 cases; 2 mesotheliomas, and in 24 cases the aspect was normal or the minimal alterations found were not conclusive. The proportion of confirmations was of 81% for tuberculosis and 37% for cancer. The method is recommended as being of particular value in establishi

Primary Burkitt's lymphoma only involving the chest wall at relapse in a child.

Burkitt's lymphoma primarily originating from the soft tissue is a very rare neoplasm. A five-year-old boy was admitted with a mass on the lateral side of the right breast. Radiological examination revealed a mass, which had originated from the soft tissue of the chest wall without any contiguous pleural and lung parenchyma disease. Histopathology showed this to be Burkitt's lymphoma. To our knowledge, this case is the first instance of Burkitt's lymphoma presenting as an isolated chest wall mass which originated from the soft tissue.

T-cell lymphoblastic leukemia/lymphoma presenting in a recurrent thymoma.

A lymphoblastic malignancy presented as the lymphoid component of a recurrent invasive lymphocyte-rich thymoma that had been previously resected and treated with chemotherapy. This high grade lymphoid neoplasm concurrently involved mediastinal nodes and subsequently disseminated to pleural fluid and peripheral blood. Lymphocytes with convoluted nuclei demonstrated a T-lymphoblastic phenotype (UCHL1+, Leu-22+, TdT+) by immunohistochemical studies. T-cell lymphoblastic leukemia/lymphomas in this clinical setting have not been reported, and this case suggests that neoplastic transformation of lym

Massive malignant solitary fibrous tumor of the diaphragm: A case report.

INTRODUCTION: Malignant solitary fibrous tumor (MSFT) of the diaphragm is extremely rare, and to the best of our knowledge, only three cases have been reported in the past two decades. In all these cases, the diaphragms were usually reconstructed with artificial diaphragm patch because of the extensive resection. PATIENT CONCERNS: We reported a male patient with complaints of dyspnea, chest pain and massive pleural effusion in the left chest detected by chest X-ray. A huge mass of 20 × 20 cm was seen in the left lower chest in the computed tomography (CT) scan. DIAGNOSIS: The diagnosis of MS

Efficacy and Safety of Povidone-iodine Pleurodesis in Malignant Pleural Effusions.

INTRODUCTION: Malignant pleural effusion (MPE) is determined by the detection of malignant cells in pleural fluid or pleural tissue. Neoplasm of lung, breast, ovary and lymphoma are the causes of more than 75% of MPE. Pleurodesis is a usual technique in the management of MPE to achieve a symphysis between two layers of the pleura, and various chemical agents have been used in an attempt to produce pleurodesis. With regard to complications and limitations of these sclerosing agents, efficacy and safety of povidone-iodine have been investigated in this study. MATERIALS AND METHODS: Between June

Pleural diseases: multimodality imaging and clinical management.

The parietal and visceral pleura are specialized membranes which are highly efficient in keeping the pleural space essentially dry and free of protein and particulate matter. Radiology has played a pivotal role in the understanding of pleural diseases because radiography and, more recently, computed tomography (CT), sonography, and magnetic resonance imaging (MRI) have allowed in vivo visualization of abnormalities. In addition, these newer modalities have been invaluable in guiding diagnostic and therapeutic measures. Cross-sectional imaging techniques, particularly CT, are frequently of assi

Delayed Solitary Adrenal Metastasis in Malignant Pleural Mesothelioma with Long-Term Survival under Immunotherapy.

Malignant pleural mesothelioma (MPM) is a rare and aggressive neoplasm with limited long-term survival. We report a case of a 72-year-old woman diagnosed with epithelioid MPM and NF2 mutation in 2018, who presented with a solitary adrenal metastasis six years after initial diagnosis. After initial surgery and chemotherapy, disease progressed within one year. Immunotherapy with ipilimumab and nivolumab resulted in long-term disease control. This case highlights an exceptionally delayed metachronous adrenal metastasis and prolonged survival under sustained immunotherapy.

Cytological Profile of a Case of Malignant Mesothelioma of Pleura with Supraclavicular Lymph Node Metastasis.

Malignant mesothelioma of pleura is a locally aggressive rare neoplasm of mesothelial cells, which produces distant metastasis in advanced stages of its course. It is exceptional for patients to present with metastasis prior to the detection of primary tumor in the pleura. Here, we report a case of malignant mesothelioma of pleura in a 65-year-old quarry worker where the initial diagnosis was offered by fine needle aspiration of an enlarged supraclavicular lymph node. Subsequent pleural fine needle aspiration cytology (FNAC), tumor biopsy, and immunohistochemical analysis confirmed the diagnos

[Round atelectasis of the lung: clinicopathological study of 6 cases and review of the literature].

OBJECTIVE: To report about 6 new patients with round atelectasis of the lung, 1 of them professionally exposed to asbestos and another to silicates. RESULTS: The patients, 5 males and 1 female, presented with a peripheral, rounded pulmonary opacity, simulating a neoplasm. The examination of the surgical specimen revealed a pleuritis, with multiple pleural folding: the underlying lung parenchima was compressed, but otherwise unremarkable. CONCLUSIONS: Round atelectasis is relatively unusual for the pathologist. However, the correct diagnosis is potentially important, because the lesion can be

Scintigraphic localization of lymphatic leakage site after oral administration of iodine-123-IPPA.

UNLABELLED: Chylothorax can occur secondary to traumatic lesions of the thoracic duct caused by chest injuries, surgical procedures involving the pleural space, neoplasms or malformations of the lymphatics. METHODS: Lymphatic leakage sites were localized by scintigraphy after oral administration of the 123I-labeled long-chain fatty acid derivative iodophenyl pentadecanoic acid (IPPA). We report on three patients with different lymphatic leakage sites and on one normal control subject. RESULTS: IPPA scintigraphy localized the lymphatic leakage site correctly in all three patients.
