COVID-19: A perspective on Africa's capacity and response.

Global powerhouses with tried and tested health systems have struggled to contain the COVID-19 pandemic. One is left to wonder what will be left of Africa, the second most populous continent after Asia, which is torn by civil wars, hunger, and diseases like AIDS and TB and, in recent years, the Ebola Virus Disease (EVD). The majority of countries' health systems, already dependent on donor aid, are ill-equipped and under-resourced to deal with the raging pandemic. There is a lack of isolation and intensive care infrastructure, ventilators, and financial resources to bankroll the fight against

Increasing Immigration Enforcement in the Hospital Setting: Public Health Risks and Guidance for Hospitals and Healthcare Professionals.

Healthcare settings, dedicated to patient care and healing, have traditionally been somewhat insulated from broader political and cultural controversies. However, on January 21, 2025, the Department of Homeland Security rescinded the "sensitive location" designation for hospitals and other healthcare facilities, permitting Immigration and Customs Enforcement to conduct more enforcement actions in these settings. The previous sensitive location designation effectively meant that immigration enforcement actions, such as arrests or surveillance, were to be avoided absent exigent circumstances (i.

What would it really take to solve the overdose epidemic in the United States?

The high overdose mortality rates in the United States poses several questions: Why have they been increasing exponentially since 1979? Why are they so high? And how can they be greatly reduced? Building on past research, the causes of the increase seem to be deeply rooted in US social and economic structures and processes, rather than due only to opioid prescription patterns or the advent of synthetic opioids. Given this, we consider what changes might be needed to reverse the exponentially-increasing overdose mortality. We use a path dependency argument to argue that the United States politi

Neurorehabilitation for people with disorders of consciousness: an international survey of health-care structures and access to treatment, (Part 1).

AIMS: The provision of rehabilitation services for people with disorders of consciousness (DoC) may vary due to geographical, financial, and political factors. The extent of this variability and the implementation of treatment standards across countries is unknown. This study explored international neurorehabilitation systems for people with DoC. METHODS: An online survey (SurveyMonkey®) was disseminated to all members of the International Brain Injury Association (IBIA) DoC Special Interest Group (SIG) examining existing rehabilitation systems and access to them. RESULTS: Respondents (n = 3

Establishing a path to unity: Recommendations for the Biden/Harris administration.

The Biden/Harris Administration faces many challenges, from systems and policies that do not work for or benefit all Americans to stark social and political divisions. Multiple courses of action will be necessary, and there must be commitment and investment for the "long haul." When considering the nation's challenges, overarching themes emerge that must be addressed. For instance, recommendations for justice reform cannot be followed without significant focus on race and equity. This focus will also be needed in considering solutions to affordable housing shortages, economic crises, and socia

Jails, prisons, and the health of urban populations: a review of the impact of the correctional system on community health.

This review examined the interactions between the correctional system and the health of urban populations. Cities have more poor people, more people of color, and higher crime rates than suburban and rural areas; thus, urban populations are overrepresented in the nation's jails and prisons. As a result, US incarceration policies and programs have a disproportionate impact on urban communities, especially black and Latino ones. Health conditions that are overrepresented in incarcerated populations include substance abuse, human immunodeficiency virus (HIV) and other infectious diseases, perpetr

Integration of Cardiovascular Disease Services within Primary Health Care Systems: Insights from South Asia.

PURPOSE OF REVIEW: This review assesses progress towards integrating cardiovascular disease (CVD) services within primary health care (PHC) systems in South Asia. The review examines the policy landscape, existing implementation efforts, barriers and enablers to integrated care, and the impacts of these approaches on health and health service outcomes. RECENT FINDINGS: National policies widely prioritise integration in response to rising non-communicable disease burdens, but implementation remains uneven across countries. Our findings illustrate that both national programmes and smaller pilot

COVID-19 and the malaria elimination agenda in Africa: Re-shifting the focus.

The global Coronavirus disease 2019 (COVID-19) pandemic has resulted in public health, political, scientific and private sector response at an unprecedented scale. However, this shift in focus has caused widespread disruption to global health services and has the potential to reverse gains made in efforts to control malaria. If health systems are not able to maintain malaria control interventions while managing the response to the COVID-19 pandemic, malaria cases will increase, thereby placing even more strain on already overtaxed systems. Using a Narrative Review Approach, this commentary exp

From complexity to parsimony: A systems thinking validation of the multiple streams framework in abortion policy agenda setting.

BACKGROUND: Abortion policy is a highly contested area of health policy. Despite international recognition of abortion as a human right, legal restrictions persist in many countries, and recent decades have seen both liberalization and retrenchment of abortion laws. While much research has examined the politics and outcomes of abortion policy reform, less attention has been paid to the upstream process of how abortion emerges on the policy agenda. This study addresses this gap by focusing on agenda setting for abortion policy, using Kingdon's multiple streams framework (MSF) and systems thinki

A socio-ecological approach to the determinants of animal health management: A scoping review.

With approximately 60% of human infectious diseases originating from zoonotic sources, an integrated approach to animal health management is critical. Significant barriers persist in optimizing disease control strategies, particularly regarding diagnostics, surveillance, biosecurity, and vaccination systems. Beyond technical and health-related aspects, socio-economic factors substantially influence both adoption and efficacy of intervention measures. While these determinants have been increasingly explored through integration of social sciences, such as economics, sociology, political science,

One Health in Mesoamerica: historical legacies, current challenges, and strategic directions.

BACKGROUND: Mesoamerica, a biodiversity hotspot extending from Mexico to Panama, faces intersecting health challenges rooted in colonial-era transformations and amplified by contemporary environmental change. This perspective paper demonstrates how historical legacies interact with current ecological and social drivers to shape zoonotic disease vulnerability across the region. APPROACH: We integrate historical analysis of colonial disease introduction with an examination of current environmental, climatic, and social determinants of disease transmission. Findings from a structured multisector

'At a tipping point': a comparative analysis of oral health coverage for children across six European countries: Denmark, Germany, Hungary, Ireland, Scotland, and Spain.

BACKGROUND: Dental caries remains a significant public health problem for children with continuing calls to incorporate oral health under the Universal Healthcare domain (UHC). However, there is little knowledge on the variations in the coverage, financing, and access to child oral healthcare across Europe. METHODS: This inter-country comparative analysis provides a detailed description and mapping of publicly funded child oral health coverage across six European countries: Denmark, Germany, Hungary, Ireland, Scotland, and Spain. Enablers and barriers for climate resilient health systems: Lessons from a qualitative study in Nepal.

BACKGROUND: Climate change poses escalating threats to health systems, particularly in low-resource settings like Nepal, where extreme weather events and chronic stressors challenge service delivery. Despite Nepal's high vulnerability, little is known about the enablers and barriers that underpin its health system's resilience to climate change. METHODS: We conducted a qualitative study using semi-structured interviews with 26 key informants across Nepal's health system, including government officials, researchers, provincial workers and non-governmental organization (NGO) representatives. A checklist to improve health system resilience to infectious disease outbreaks and natural hazards.

Recent infectious disease outbreaks, including the ongoing global COVID-19 pandemic and Ebola in the Democratic Republic of the Congo, have demonstrated the critical importance of resilient health systems in safeguarding global health security. Importantly, the human, economic and political tolls of these crises are being amplified by health systems' inabilities to respond quickly and effectively. Improving resilience within health systems can build on pre-existing strengths to enhance the readiness of health system actors to respond to crises, while also maintaining core functions. Using data

Reproductive health, and child health and nutrition in India: meeting the challenge.

India, with a population of more than 1 billion people, has many challenges in improving the health and nutrition of its citizens. Steady declines have been noted in fertility, maternal, infant and child mortalities, and the prevalence of severe manifestations of nutritional deficiencies, but the pace has been slow and falls short of national and Millennium Development Goal targets. The likely explanations include social inequities, disparities in health systems between and within states, and consequences of urbanisation and demographic transition. In 2005, India embarked on the National Rural

[The Other, the object and the social connection].

The Occupy Abay movement was formed in Moscow in a context of anomie. This Russian opposition movement consisted in gatherings of strangers, without any predefined ideology, the tacit objective possibly being a civilizing project. The experience of the Scratch orchestra brought together nonprofessional musicians whose performances could be linked to political acts. The psychiatric institution brings people together; it must be able to offer a response to patients' suffering. The choice of working together through multi-person practice is an option. It is based on the fact of accepting, like th

Development and validation of a PEST-based instrument for assessing macro-environmental factors in Olympic athletes' preparation under crisis conditions.

INTRODUCTION: Understanding how external crises influence elite sport requires context-specific research tools. This study aimed to develop and validate two structured questionnaires based on the PEST framework (Political, Economic, Socio-cultural, and Technological) to evaluate macro-environmental factors affecting the preparation of Ukraine's Olympic team in times of instability. MATERIALS AND METHODS: A panel of seven experts in sport science from four countries assessed 55 items (27 for Tokyo 2020 and 28 for Paris 2024) across three dimensions: clarity, relevance, and appropriateness. Health policies in Brazil (2016-2022): neoliberalism, pandemic, and threats to the Brazilian Unified National Health System.

The article analyzes health policies in Brazil from 2016 to 2022, a period marked by a shift in national politics and the COVID-19 pandemic. The study examines federal health policy management under the administrations of Michel Temer (2016-2018) and Jair Bolsonaro (2019-2022), considering the context, the political process, and policy content, as well as the different phases of the pandemic response. Drawing on historical institutionalism and the concept of critical juncture, the analysis explores continuities, changes, and disputes surrounding the Brazilian Unified National Health System (SU
