The clinical spectrum of isolated peripheral motor dysfunction.

INTRODUCTION: Isolated peripheral motor dysfunction due to lower motor neuron or peripheral nerve disorders presents an interesting challenge to clinicians because of the diverse differential diagnosis with a broad range of prognoses. METHODS: We retrospectively reviewed clinical data of adults who presented over 12 years with muscle weakness, atrophy, or fasciculations, but without hyperreflexia or sensory involvement. RESULTS: In 119 patients, 52% had a motor neuron disease (MND), 13% had immune neuropathies, 11% had genetic neuronopathies, 10% had residual or post-polio syndrome, 5% had b

New insights into structural determinants of prion protein folding and stability.

Prions are the etiological agent of fatal neurodegenerative diseases called prion diseases or transmissible spongiform encephalopathies. These maladies can be sporadic, genetic or infectious disorders. Prions are due to post-translational modifications of the cellular prion protein leading to the formation of a β-sheet enriched conformer with altered biochemical properties. The molecular events causing prion formation in sporadic prion diseases are still elusive. Recently, we published a research elucidating the contribution of major structural determinants and environmental factors in prion p

[Neurasthenia, yesterday and today].

Neurasthenia was described and explained in very mechanistic terms, at the end of the 19th century, by G.M. Beard to account for physical and mental exhaustion and for varied somatic troubles imputed to failure of too much solicited nervous resources. This concept was then universally adopted and gave rise to diverse interpretations, among which was the Freud's one. Later, in Occident, came a deterioration, the diagnostic of neurasthenia giving way to those of anxious or affective disorders. In the same time, at least for ideological and cultural reasons, the concept remained lively in Russia

Post-irradiation somnolence syndrome in an adult patient following allogeneic bone marrow transplantation.

Neurologic disturbances are common following the intensive chemotherapy and radiotherapy of bone marrow transplantation (BMT) conditioning regimens. The somnolence syndrome, which occurs in most children treated for leukemia with prophylactic cranial irradiation, has previously not been reported following BMT. This syndrome consists of transient lethargy, irritability, headaches, low grade fevers, gastrointestinal disturbances and depression. We report the case of a 38-year-old female with acute non-lymphocytic leukemia who developed symptoms typical of the somnolence syndrome 8 weeks followin

Post-infectious Moyamoya Syndrome: A Review of Existing Scientific Literature From 2000 to 2023.

Moyamoya disease (MMD) is considered a primary disorder of an unknown etiology. In contrast, Moyamoya syndrome (MMS) refers to MMD associated with other underlying diseases, such as meningitis in childhood, neurofibromatosis type II, Down syndrome, cranial irradiation, and different types of anemias, particularly hemoglobinopathies. We aimed to provide a comprehensive clinicopathological overview of MMS. All case reports and case series published from 2000 to 2023 pertaining to MMD were included in the study. Case studies, original articles, editorials, letters to editors, and clinical images

Assessment of the prevalence and associated risk factors of pediatric hydrocephalus in diagnostic centers in Addis Ababa, Ethiopia.

BACKGROUND: Hydrocephalus (HCP) is a common disorder of cerebral spinal fluid (CSF) physiology resulting in abnormal expansion of the cerebral ventricles. Infants commonly present with progressive macrocephaly whereas children older than 2 years generally present with signs and symptoms of intracranial hypertension. Neither qualitatively nor quantitatively are there adequate data to determine the prevalence and incidence of HCP in the developing world. HCP is a treatable condition that when left untreated, has fatal consequences. OBJECTIVE: The objective of this study was to assess the preval

Parainfectious Transverse Myelitis in a Pregnant Woman: A Rare Clinical Entity.

Transverse myelitis (TM) is an inflammatory disorder of the spinal cord characterized by motor, sensory, and autonomic impairments. Parainfectious transverse myelitis (PITM) represents a post-infectious, immune-mediated entity. Its occurrence during pregnancy is rare and presents with unique diagnostic and therapeutic challenges. We report a 31-year-old primigravida at 32 weeks of gestation who presented with acute-onset bilateral lower limb weakness, sensory loss, and bladder involvement following a febrile illness. MRI spine revealed longitudinally extensive transverse myelitis (C3-T2). The

Glossopharyngeal neuralgia after SARS-CoV-2 infection: A case report.

Glossopharyngeal neuralgia (GN) is a rare neuropathic disorder characterized by sudden, unilateral, electric shock-like pain in the areas innervated by the glossopharyngeal nerve. Its diagnosis is frequently delayed because of its clinical overlap with odontogenic and otorhinolaryngological conditions. In the context of the COVID-19 pandemic, different cranial neuropathies have been reported, suggesting possible post-infectious mechanisms. We describe the case of a 54-year-old male dentist, without relevant medical history, who developed recurrent episodes of intense pain in the right pharynx

Intimomedial mucoid arterial degeneration, a rare arterial disorder of forensic significance.

The body of a 43-year-old African woman with a history of aortic aneurysm and hypertension was forensically investigated after her sudden death. The cause of death was related to a cardiac tamponade due to a ruptured aneurysm of the ascending aorta. Post-mortem gross examination showed an abnormal whitish discoloration of the intima with fibrous thickening of the aortic wall. Several arteries (left main and circumflex coronaries, carotid, renal and iliac arteries) showed similar features. Upon histological examination, the aortic aneurysm as well as the other arteries sampled showed mucoid deg

Swyer-James-MacLeod Syndrome: A Rare Cause of Unilateral Hyperlucent Lung in a Two-Year-Old Male Child.

Swyer-James-MacLeod syndrome (SJMS) is an uncommon post-infectious bronchiolitis obliterans-related pulmonary disorder characterized by a unilateral hyperlucent lung with associated hypovascularity on chest radiography and CT, and markedly reduced perfusion on ventilation-perfusion scintigraphy. Although typically identified in older children or adolescents due to its insidious progression, diagnosis in early childhood remains rare and diagnostically challenging. This report describes a two-year-old male child with recurrent lower respiratory tract infections who was ultimately diagnosed with

Multiple intracranial space-occupying lesions in a renal transplant recipient from an area endemic for tuberculosis (TB): TB vs. toxoplasmosis.

Renal transplant recipients may present with intracranial space-occupying lesions (SOLs) due to infections as well as a post-transplant lymphoproliferative disorder (PTLD). Here, we discuss a renal transplant recipient who presented with neurologic symptoms and magnetic resonance imaging (MRI) of the brain showed multiple focal SOLs. Tuberculosis (TB), toxoplasmosis, nocardiosis, fungal infections, and PTLD were considered in the differential diagnosis. MRI spectroscopy was suggestive of an infectious cause, such as toxoplasmosis or TB. Serologic tests using Toxoplasma were negative. A brain b

Progressive Pulmonary Alveolar Proteinosis Diagnosis Masking Pneumonic Lung Carcinoma: Anchoring Bias in Critical Care.

Pulmonary alveolar proteinosis (PAP) is a rare disorder characterized by impaired surfactant clearance and intra-alveolar accumulation of proteinaceous material, resulting in progressive respiratory insufficiency. Its clinical, radiologic and cytologic features are nonspecific and overlap with a variety of diffuse pulmonary diseases. Similarly, poorly cohesive pneumonic-type lung carcinomas can present with diffuse pulmonary infiltrates that mimic infectious, inflammatory or metabolic lung disorders. On bronchoalveolar lavage (BAL), malignant cells may closely resemble atypical reactive pneumo

Etiology of Acute Leukemia: A Review.

Acute leukemias constitute some of the most common malignant disorders. Despite significant progress made in the treatment of these disorders, their etiology remains unknown. A large and diverse group of genetic and environmental variables have been proposed. The role of a variety of factors, including pre-existing and acquired genetic mutations, exposure to radiation and various chemicals during preconception, pregnancy and throughout life, have been explored. The effects of inherited genetic variations and disorders, pre-existing diseases, infectious agents, hobbies, occupations, prior treat

Post-tuberculosis pulmonary function and noninfectious pulmonary disorders.

Tuberculosis (TB) is among the top infectious causes of morbidity and mortality worldwide and is associated with frequent pulmonary damage despite microbiological cure. Patients with treated TB may remain lifelong sufferers of disabling structural and functional sequelae of the disease, which subsequently impair quality of life. Long-term follow-up studies have revealed that many patients with treated pulmonary TB show signs of permanent impairment of their lung function. Impairment is variable in pattern and severity, ranging from none to severe, and shows restrictive, obstructive, or mixed p

[Benign encephalitis of the brain stem].

INTRODUCTION: The secondary encephalitis disorders are due to an immunological mechanism which causes demyelinating lesions of the central and peripheral nervous systems, with very variable clinical features. The pathogenesis and localization of benign encephalitis of the brain stem and the Miller Fisher syndrome (MFS) are still subject to debate. It is suggested that they may both belong to different extremes of the same nosological spectrum known as the ophthalmoplegia-ataxia-areflexia syndrome. CLINICAL CASE: We report the case of an 11 year old boy with encephalitis of the brain stem who

Somatic mutation, a cause of biliary atresia: A hypothesis.

Despite many years of research, the causes of biliary atresia still remain elusive. Infection, immune disorder, toxins or maternal microchimerism have been cited as potential triggers of biliary atresia. This is a rare disease with a stable incidence over the years although with sizeable ethnic variations. This stability suggests that environmental factors have in fact only a slight influence. During the search for etiologies, twin studies have often helped disentangle the genetic from the environmental. For this condition, twin studies have mainly demonstrated a lack of concordance between tw

Prediction of neutrophil nadir and recovery following paediatric haematopoietic cell transplantation with busulfan conditioning.

AIMS: In haematopoietic cell transplantation (HCT), neutropenia resulting from myelosuppression is an expected endpoint following busulfan-based conditioning. However, if prolonged, neutropenia can lead to complications like serious infection and death. The routine use of pharmacokinetic (PK)-guided busulfan dosing has decreased toxicities; however, patients still have serious, sometimes fatal, complications of drug-induced neutropenia. We sought to investigate whether the time-course of neutropenia after HCT could be predicted for paediatric patients following busulfan-based conditioning. Suc

Neuroinflammatory syndromes in children.

PURPOSE OF REVIEW: Neuropsychiatric symptoms due to paediatric neuroinflammatory diseases are increasingly recognized and reported. Psychiatrists are crucial in front-lines identification, diagnosis and care of individuals with disorders such as autoimmune encephalitis and management of long-term neurobehavioral sequelae. This review summarizes recent literature on autoimmune and post-infectious encephalitis, discusses special considerations in children with neurodevelopmental conditions and presents a paradigm for evaluation and management. RECENT FINDINGS: There is a growing body of evidenc

IL-23/IL-17 immune axis in Guillain Barré Syndrome: Exploring newer vistas for understanding pathobiology and therapeutic implications.

Guillain Barré Syndrome (GBS) is a severe disorder of the peripheral nervous system with an inadequately known etiopathology. It is a post infectious immune mediated disorder, characterized by autoantibody production, complement activation as well as T reactivity against gangliosides. However, the precise etiopathogenesis remains poorly understood in a majority of the patients. Th17 cells, a recently identified lineage of Th cells have emerged as a predominant inducer of autoimmunity and inflammation in various immunological disorders. Pathobiological role of Th17 pathway is also becoming incr
