Controversy continues over the mode of delivery in severe pre-eclampsia remote from term. The aim of this study was to assess prospectively the mode of delivery in severe pre-eclampsia remote from term. The clinical data of 108 women who presented consecutively with severe pre-eclampsia over a 1-year period was used for the study material. Sixty-eight (63%) patients were delivered by elective caesarean section and 40 (37%) underwent induction of labour. INTRODUCTION: The differential diagnosis between pre-eclampsia and chronic hypertension is not easy, but is essential to proper management of a pregnancy. Patients presenting pre-pregnancy hypertension can be treated conservatively, if not a superimposed pre-eclampsia occurs, controlling pressure pharmacologically and completing the pregnancy with a natural delivery. In pre-eclampsia, hypertension is merely the visible sign of a process of endothelial damage and coagulation cascade activation which is often destined to emerge clinically on a dramatic scale.

We report a case of geographic or mosaic pattern yellowish opaque foci in the left eye of a 36-year-old woman who suffered from severe pre-eclampsia. Though the geographic lesions resulted in chorioretinal atrophy, the mosaic lesion led to no significant atrophy. These two findings were both diagnosed as manifestations of choroidal ischaemia, the former due to choroidal artery occlusion, the latter to transient insufficiency of choroidal circulation, which reflects the lobular pattern of the choriocapillaris.

Pregnancies of fourteen women who gave birth to babies with trisomy 13 were studied retrospectively for evidence of pre-eclampsia, with twenty-eight controls matched for age and parity. Of the five nulliparous women who subsequently gave birth to a baby with trisomy 13 all had had severe pre-eclampsia, compared with none of the control group. The response of maternal lymphocytes to phytohaemagglutinin was determined during and after pregnancy in patients with severe pre-eclampsia and in controls. The patients with pre-eclampsia showed a greater degree of immunosuppression during pregnancy than the normotensive controls. This was apparent in the presence and absence of maternal serum. In pre-eclampsia there may be an abnormality affecting the T cell subpopulation of lymphocytes and also a decreased immunosuppressive effect of maternal serum.
