Chylothorax is a pleural effusion caused by leakage of chyle into the pleural cavity. The commonest causes of pleural effusion are trauma to the ductus thoracicus or neoplasms in the mediastinum. The trauma can be iatrogenic or accidental. Chylothorax caused by tension pneumothorax has not previously been reported. We describe a case where this etiology is the most likely explanation of the chylous effusion in our patient.

Blood pressure measurements made in the physician's office have been the standard for diagnosing hypertension and determining the efficacy of antihypertensive drug therapy. However, these measurements do not necessarily reflect a patient's usual blood pressures in other circumstances, such as at work or in the home. Accurate blood pressure measurements performed by the patient or a family member in an out-of-office setting can be integrated with office readings to assist in the management of hypertension.

The technique of micturitional urethral pressure profilometry using a trilumen catheter provides a method of assessing the dynamic behavior of the lower urinary tract during voiding. This method of evaluation is simple to perform, highly reproducible, accurate, and clinically useful not only in diagnosing the presence of outlet obstruction, but also in identifying its location and assessing its severity. The rationale for the use of this technique, the interpretation of the results, and various pressure profile configurations are discussed.

Carcinomas arising from chronic pressure ulcers, Marjolin's ulcers, are an uncommon occurrence. They are virulent cancers that require aggressive surgical treatment. We present 3 such cancers including the first report of a squamous cell carcinoma arising in a distal extremity pressure ulcer.
