Prison - no place for older people.

Prisons and probation ombudsman Nigel Newcomen has called for a national strategy to address the healthcare needs of older people in prison. His report should be welcomed - it focuses attention on a group of people who are all too easily overlooked in our society. The bad news is, the report is a depressing read. Prisons designed for young men are struggling to cope with people in their seventies and eighties who may have dementia or need palliative care.

[Prisoners in units for difficult patients].

Prisoners, on remand or convicted, can be placed for a variable amount of time in a unit for difficult patients if their pathological mental state so requires. For the most part, their therapeutic care does not depend on their status as prisoner. The treatments provided are those indicated for their psychological pathologies and their potential or known dangerousness. However, some administrative measures make a distinction between their treatment and that of non-prisoner patients placed in these secure psychiatric units.

Intellectual disability and the prison setting.

INTRODUCTION: The prevalence of intellectual disability (ID) in the prison setting has scarcely been studied. Although some approximations or estimates regarding people with intellectual disabilities have been performed in Spain, there is little in the way of reliable data. OBJECTIVES: 1) To determine the prevalence of ID in a sample population in the residential modules of a Spanish prison, 2) Obtain data on the prevalence of ID in prison psychiatric units and hospitals. METHODS: 1) A TONI II test was performed on a sub-sample (n = 398) of a prevalence study in Spanish prisons to identify i…

Stress and Hardship after Prison.

The historic increase in U.S. incarceration rates made the transition from prison to community common for poor, prime-age men and women. Leaving prison presents the challenge of social integration--of connecting with family and finding housing and a means of subsistence. The authors study variation in social integration in the first months after prison release with data from the Boston Reentry Study, a unique panel survey of 122 newly released prisoners. The data indicate severe material hardship immediately after incarceration. Over half of sample respondents were unemployed, two-thirds recei

The health of female prisoners in Indonesia.

PURPOSE: Indonesian law provides prisoners with basic rights, including access to education, health care and nutrition. Yet, structural and institutional limitations, notably overcrowding and under-resourcing, prohibits penal institutions from fulfilling these commitments for female prisoners. The purpose of this paper is to explore their health concerns. DESIGN/METHODOLOGY/APPROACH: Six prisons and one detention centre were researched, comprising: female prisoners (n=69); clinical officers (six); clinic heads (seven); wardens (seven); heads of prisons (seven); and a Directorate representativ…

A prisoner with chronic myeloid leukemia.

The health care received by prisoners in the area of tissue and organ transplantation is not well discussed or documented. We encountered a prisoner with newly diagnosed chronic myelogenous leukemia who was a candidate for allogeneic bone marrow transplantation and had two HLA-identical siblings who were willing to donate bone marrow. Based on humanitarian, constitutional, and ethical considerations, we suggest that the prisoner (patient) should receive the same health care as individuals who are not incarcerated and that the costs of care should play no greater role for prisoners than for oth

Tuberculosis in a prison population in Malawi.

BACKGROUND: Much concern has been expressed about the high prevalence of tuberculosis in prisons in industrialised countries. Since there is almost no information from developing countries, we investigated the rate of pulmonary tuberculosis in a large prison in Malawi. METHODS: Between May and July, 1996; we carried out an active case-finding survey in Zomba Central Prison, Malawi, through the National Tuberculosis Control Programme. We interviewed prisoners, and those with a cough of at least 1 week's duration were screened by sputum-smear microscopy. If microscopy was negative, prisoners un…

Admissions to a prison psychiatric unit.

Fifty consecutive admissions to the psychiatric division in a central metropolitan goal were given DSM III diagnoses and their psychiatric, social and criminal histories analysed. Seventy-two per cent suffered from schizophrenic, affective or organic mental illness, 56% had chronic physical disabilities and 84% had had previous in-patient treatment in either hospital or prison or both. Fifty-four per cent exhibited psychopathology that most clinicians would have thought required urgent attention. Of the current offences committed by this group 54% were against the person (murder, assaults, sex

Respiratory mortality in prisons during the 1918-1919 influenza pandemic.

Prisons are among the most controlled human environments, and prisoners often suffer epidemic disease when influenza is introduced into these restricted populations. A review of contemporary records of five prison systems during the 1918-1919 influenza pandemic showed that most prison populations did remarkably well under lockdown, with few resulting respiratory deaths. Exceptional prisons with high respiratory mortality included the Andaman Islands prison colony (17.3/1000) and the US Army Disciplinary Barracks (42/1000) at Fort Leavenworth, Kansas. Factors that likely resulted in high respir…

Burn Care in Prisons: Ethical Dilemmas in Correctional Health Care.

Burn injuries among incarcerated individuals represent an underexplored intersection of medical ethics, correctional policy, and health equity. Although scholarship increasingly examines vulnerable groups such as refugees and undocumented patients, incarcerated burn patients remain largely absent. Dual loyalty-balancing duty to the patient with obligations to the institution or state-creates distinctive challenges that are intensified in burn care, where timely intervention, pain control, rehabilitation, and psychosocial support are critical but often constrained by security protocols and syst
