In some cases a gratifying transference fantasy is the subject of progressive analytic work, while in other cases the same type of fantasy eludes investigation, and its enactment causes treatment to become an unproductive endless task. PURPOSE: The purpose of this paper is to outline research which aimed to explore psychotherapists' experience of working with despair, in the UK prison setting, through a qualitative phenomenological approach. Within the forensic psychological literature, despair is considered a pathology, associated with suicide and self-harm, resulting from the prisoners histories and the coercive prison setting. In turn, therapeutic writings outline the importance of therapy in the prison setting with despair in providing coping skills, containment and learning opportunities for the prisoners involved.

The present paper assessed therapeutic alliance over the course of Enhanced Cognitive Behavioural Therapy (CBT-E) in a community-based sample of 112 patients with a diagnosis of bulimia nervosa (BN) or atypical BN. Temporal assessment of alliance was conducted at three time points (the start, middle and end of treatment) and the relationship between alliance and treatment retention and outcome was explored. We studied 814 client laughter events nested within 330 sessions nested within 33 clients nested within 16 therapists at one community clinic in which doctoral student therapists provided psychodynamic psychotherapy to adult community clients. Each laughter event in Sessions 1 to 5 and 16 to 20 was rated for cheerfulness, politeness, reflectiveness, contemptuousness, and nervousness.
