A model of hydrocephalus due to windkessel impairment caused by subarachnoid space obstruction.

OBJECTIVE: The traditional view that hydrocephalus due to obstruction in the subarachnoid space (SAS) is caused by malabsorption of CSF does not account for many experimental and clinical aspects of the disorder. Flow MRI reveals that nearly all CSF motion is pulsatile, and that hydrocephalus is linked to significant redistribution of pulsatility within the CSF pathways. Pulsatility in the cranium is governed by the cerebral windkessel system that buffers the energy of arterial pulsations via CSF to ensure smooth capillary blood flow. This study proposes a new model of intracranial thermodynam

Mechanistic insights on age-related changes in heart-aorta-brain hemodynamic coupling using a pulse wave model of the entire circulatory system.

Age-related changes in aortic biomechanics can impact the brain by reducing blood flow and increasing pulsatile energy transmission. Clinical studies have shown that impaired cardiac function in patients with heart failure is associated with cognitive impairment. Although previous studies have attempted to elucidate the complex relationship between age-associated aortic stiffening and pulsatility transmission to the cerebral network, they have not adequately addressed the effect of interactions between aortic stiffness and left ventricle (LV) contractility (neither on energy transmission nor o

Impact of Respiratory Mechanics on Venous Return in Single Ventricle Physiology: A Clinical Imaging Study.

In participants with single-ventricle physiology, it is unclear how respiratory mechanics affects venous return. The aim of this study was to evaluate the impact of respiratory effort and rate on venous return. We used magnetic resonance imaging to quantify flow in the inferior vena cava (IVC) in 20 participants with total cavopulmonary connection during a breathing study (shallow, baseline, deep, and slow, baseline, fast). Retrograde IVC flow rate, forward IVC flow rate, retrograde flow ratio, forward IVC flow volume, retrograde IVC flow volume, and pulsatility index were extracted. Increases

Multiple Venous Malformations as a Cause of Pulsatile Tinnitus.

INTRODUCTION: Pulsatile tinnitus is a relatively common presentation in otolaryngology clinics, most cases of which have a treatable cause. This presentation warrants a thorough workup to identify treatable, and rule out life-threatening, etiologies. We present a case of a patient with pulsatile tinnitus arising from multiple dilated venous channels in the head and neck. Case Presentation. We present the case of a 65-year-old Caucasian female with a two-year history of progressive, bilateral pulsatile tinnitus, which had become debilitating. Computed-tomographic angiography (CTA) studies ruled

The role of altered impedance in the pathophysiology of normal pressure hydrocephalus, Alzheimer's disease and syringomyelia.

Normal pressure hydrocephalus, Alzheimer's disease and syringomyelia appear to be completely unrelated diseases, however, they share a reduction in subarachnoid space compliance as part of their pathophysiology. This paper discusses the physiology of pulsatile fluid flow and its relationship to compliance/impedance. Unlike continuous or non-pulsatile flow where the vessel resistance and pressure gradient are the major determinants of the volume of fluid flowing, when the fluid flow in a vessel pulsates then the vessel compliance/impedance becomes important. A reduction in compliance in the cra…

Anti-neuroinflammatory therapy for non-pulsatile tinnitus in patients with sinus vascular anomalies: preliminary result on two cases.

BACKGROUND: Vascular anomalies are commonly associated with pulsatile tinnitus, but their potential role in triggering non-pulsatile tinnitus through neuroinflammation is not well established. Furthermore, few studies have investigated non-pulsatile tinnitus associated with altered cerebrovascular supply and whether anti-neuroinflammatory therapy might alleviate symptoms. MATERIALS AND METHODS: We describe two patients presenting with non-pulsatile tinnitus and sigmoid sinus stenosis confirmed by imaging, with other causes systematically excluded. RESULTS: Magnetic resonance venography demon

MR Venous Flow in Sigmoid Sinus Diverticulum.

BACKGROUND AND PURPOSE: Case reports demonstrate that coiling of a sigmoid sinus diverticulum can treat pulsatile tinnitus. We hypothesized that MR imaging 4D flow and computational fluid dynamics would reveal distinct blood flow patterns in the venous outflow tract in these patients. MATERIALS AND METHODS: Patients with pulsatile tinnitus of suspected venous etiology underwent MR imaging at 3T, using venous phase contrast-enhanced MR angiography, 4D flow, and 2D phase contrast. The contrast-enhanced MRA contours were evaluated to determine the presence and extent of a sigmoid sinus diverticu

Displacement forces in iliac landing zones and stent graft interconnections in endovascular aortic repair: an experimental study.

OBJECTIVES: Stent graft migration influences the long-term durability of endovascular aortic repair. Flow-induced displacement forces acting on the attachment zones may contribute to migration. Proximal fixation of aortic stent grafts has been improved by using hooks, while distal fixation and stent graft interconnections depend on self-expansion forces only. We hypothesized that flow-induced displacement forces would be significant at the distal end, and would correlate with graft movements. METHODS: As part of an experimental study, an iliac limb stent graft was inserted in a pulsatile flow…

Amplitude and phase of cerebrospinal fluid pulsations: experimental studies and review of the literature.

OBJECT: A recently developed model of communicating hydrocephalus suggests that ventricular dilation may be related to the redistribution of pulsations in the cranium from the subarachnoid spaces (SASs) into the ventricles. Based on this model, the authors have developed a method for analyzing flow pulsatility in the brain by using the ratio of aqueductal to cervical subarachnoid stroke volume and the phase of cerebrospinal fluid (CSF) flow, which is obtained at multiple locations throughout the cranium, relative to the phase of arterial flow. METHODS: Flow data were collected in a group of 1

Pulsatile albumin transport in large arteries: a numerical simulation study.

Albumin transport in a stenosed artery configuration is analyzed numerically under steady and pulsatile flow conditions. The flow dynamics is described applying the incompressible Navier-Stokes equations for Newtonian fluids, the mass transport is modelled using the convection diffusion equation. The boundary conditions describing the solute wall flux take into account the concept of endothelial resistance to albumin flux by means of a shear dependent permeability model based on experimental data. The study concentrates on the influence of steady and pulsatile flow patterns and of regional var…

Effect of pulsatile and nonpulsatile assist on heart and kidney microcirculation with cardiogenic shock.

To estimate microcirculation of the heart and kidney in pulsatile and nonpulsatile-assisted circulation, a comparison study was done using a swine model. Acute myocardial infarction was made by ligation of the left coronary artery branches. After cardiogenic shock, animals were divided into 3 groups as follows: Group C (n = 6), no assist provided; Group NP (n = 6), assisted by a nonpulsatile pump (Bio-Medicus BP-80); Group P (n = 6), supported by a pulsatile pump (Nippon Zeon). left coronary artery flow, endocardial and epicardial regional flows, and renal cortex and medulla tissue blood flows

Heart beat but not respiration is the main driving force of the systemic venous return in the Fontan circulation.

The Fontan procedure provides relief from cyanosis in patients with univentricular hearts. A major clinical unmet need is to understand whether the venous flow patterns of the Fontan circulation lead to the development of congestive hepatopathy and other life-threatening complications. Currently, there is no consensus on whether heart beat or respiration is the main driving force of venous return and which one affects the periodic flow changes for the most (i. e., pulsatility). The present study, for the first time, quantified respiratory and cardiac components of the venous flow in the inferi

Point-of-Care Ultrasound Diagnosis of High Flow Priapism.

BACKGROUND: Priapism can be categorized as low flow or high flow. Low flow priapism is a compartment syndrome and requires immediate treatment to avoid long-term ischemic damage. Alternatively, high flow priapism is not an emergent condition and can be managed as an outpatient. The diagnosis has traditionally been made via cavernosal blood gas analysis; however, this is painful and can cause iatrogenic harm. CASE REPORT: We present a case of high flow priapism whereby point-of-care ultrasound (POCUS) identified the presence of pulsatile cavernosal arterial flow to confirm the diagnosis. This
