An immunization trial carried out on 1310 persons professionally exposed to Q fever confirmed the suitability of a chemovaccine for field use. Its immunogenicity and reactogenicity differed in three groups of subjects who differed in their previous experience of Q fever and who were given different lots of the vaccine. There was some correlation between reactogenicity of the vaccine and pre-immunization positivity in the microagglutination and skin tests as indicators of a previous exposure. The skin test was more reliable for predicting post-immunization reactions.

Q fever remains a neglected zoonosis in many developing countries including Pakistan. The causing agent Coxiella (C.) burnetii is resistant to environmental factors (such as drying, heat and many disinfectants), resulting in a long-lasting infection risk for both human and animals. Coxiella burnetii is the causative pathogen of the zoonotic infection Q fever. Most patients with Q fever experience a non-specific febrile illness, hepatitis or pneumonia. Q fever has recently been described as a cause of prosthetic joint septic arthritis, but remains very uncommonly reported. We present a case of Q fever prosthetic joint septic arthritis that has responded to a combination of two-stage surgical exchange and prolonged medical treatment with doxycycline and hydroxychloroquine.

Q fever is caused by the bacterium Coxiella burnetii and has both acute and chronic forms. The acute disease is a febrile illness often with headache and myalgia that can be self-limiting, whereas the chronic disease typically presents as endocarditis and can be life threatening. The normal therapy for the acute disease is a 2 week course of doxycycline, whereas chronic disease requires 18-24 months of doxycycline in combination with hydroxychloroquine. In the Netherlands, more than 4000 patient cases of acute Q fever have been reported since 2007. Approximately 20% of these patients subsequently developed long-term fatigue.
