[An autopsy case of Gerstmann-Sträusser-Scheinker's disease with spastic paraplegia as its principal feature].

A 38-year-old woman developed slowly progressive gait disturbance. At age 39 years she was admitted to our department because she could not walk without assistance. On neurological examination she was alert with normal mental functions. Horizontal nystagmus on both sides, minimal clumsiness in the left upper extremity on the finger-to-nose test and moderate degree of spasticity in bilateral lower extremities without evident motor weakness were present. The gait was spastic with small steps on a wide base. There was no sensory abnormalities. The deep tendon reflexes were hyperactive on both sid

Motor neurone disease in Thailand: the clinical aspects of 77 patients.

This is the first large scale case series of motor neurone disease (MND) in Thailand. Seventy-seven patients were identified between 1978 and 1984 at Siriraj Hospital Medical School, Bangkok, Thailand. Fifty-five patients were male (71.43%) and the mean age of the patients was 51.55 (SD 14.26) years with the range of 17 to 78 years. Clinical classification of MND was categorized as progressive bulbar palsy (PBP), 26 patients (33.77%); amyotrophic lateral sclerosis (ALS), 42 patients (54.54%); and progressive spinal atrophy (PSA), 9 patients (11.69%). The mean age of PBP, ALS and PSA were in th

[A case of arterio-venous malformation associated with agenesis of unilateral internal carotid artery (author's transl)].

A case of arterio-venous malformation (AVM) at the right parietal area in association with agenesis of the left internal carotid artery (ICA) is described. A 27-year-old male complained of severe headache and vomiting on September 27th in 1980. He lost his consciousness and became stuporous within several minutes after the ictus. He was then transferred to Kurume University Hospital. On admission, he was semicomatose, anisocoria and left hemiparesis marked in the lower limbs, and bilateral Babinski reflex were noted. A emergency CT scan revealed a round high density area suggesting intracerebr

Unilateral contrast-induced encephalopathy with contrast medium exudation: A case report.

BACKGROUND: Contrast-induced encephalopathy (CIE) is a rare transient, reversible abnormality in the structure or function of the nervous system caused by the intravascular use of contrast agents. CIE can present with a range of neurological manifestations, including focal neurological deficits (hemiplegia, hemianopia, cortical blindness, aphasia, and parkinsonism) and systemic symptoms (confusion, seizures, and coma). However, if not accurately diagnosed and treated in a timely manner, CIE can cause irreversible damage to patients, especially critically ill patients. CASE SUMMARY: A male in

Mechanism of action of extradural analgesia.

The factors governing the spread and site of action of local anaesthetics in the extradural space are discussed. Local anaesthetic drugs injected into the extradural space may cause nerve blocks at three possible sites: (a) On the spinal nerves in the paravertebral space. This probably occurs only in young subjects. (b) On the spinal nerves intradurally. This is probably the essential site of blockade. (c) On the spinal cord.

A de novo CTNNB1 Novel Splice Variant in an Adult Female with Severe Intellectual Disability.

The catenin beta-1 (CTNNB1) gene, encoding a sub-unit of the cadherin/catenin protein complex that is involved in the Wnt signalling pathway important for proper interneuron development, is considered to be causative for the rare autosomal dominant mental retardation syndrome, formerly called MRD19 but later renamed neurodevelopmental disorder with spastic diplegia and visual defects (NEDSDV). Its main characteristics are moderate to severe intellectual disability (ID), disruptive autistic behaviours, microcephaly, absent or limited speech, facial dysmorphisms, peripheral hypertonia/spasticity

[An autopsied case of progressive supranuclear palsy, initially diagnosed as spinocerebellar degeneration with severe olivopontocerebellar involvement].

A Japanese female patient presented with dysarthria and gait disturbance with ataxia at the age of 63. She was initially diagnosed with spinocerebellar degeneration because a head CT showed atrophy of the cerebellum and brainstem, and dilatation of the fourth ventricle. Symptoms including abnormal behavior, dementia, vertical gaze palsy, dysphagia, retrocollis, axial rigidity, grasp reflex and positive Babinski's sign were gradually observed. Tegmental atrophy of the brainstem and dilatation of the third ventricle were apparent on images. The diagnosis was modified to progressive supranuclear

Surgical evacuation of an isolated interhemispheric subdural hematoma presenting with falx syndrome in the subacute period: A case report.

BACKGROUND: Traumatic isolated interhemispheric subdural hematoma (IHSDH) is rare, and falx syndrome is an uncommon but distinctive manifestation. Although many cases improve with conservative management, the optimal timing of surgery remains controversial. CASE DESCRIPTION: An 89-year-old man receiving edoxaban developed progressive left lower-extremity weakness 2 days after a minor head injury. Computed tomography (CT) showed a right isolated IHSDH measuring 14 mm in maximal thickness. Neurological examination revealed left lower-extremity monoparesis with persistent foot drop. Deep tendon

Hyperbaric oxygen-assisted neurological recovery in a patient with classic heat stroke: a case report.

BACKGROUND: Heat stroke (HS) is a critical condition involving severe central nervous system injury. While standard treatments-such as cooling, ventilation, and fluid resuscitation-can stabilize patients, they are often inadequate in addressing long-term neurological complications, including impaired consciousness and paralysis. The persistent deficits in neurological function remain a major clinical challenge. Hyperbaric oxygen therapy (HBOT), which improves brain oxygenation and supports neural repair, offers a potential solution. In the case we present of a patient with HS and brain injury,

[A case of rapidly progressive HTLV-I-associated myelopathy (HAM)].

We report a 65-year-old woman with HAM who showed rapid progression of the clinical symptoms. The initial symptom was lumbago and she became unable to walk within 4 months after the onset of the lumbago. When seen on admission, she had flaccid paraplegia and areflexia was seen in the lower extremities with positive Babinski and Chaddock reflexes. She had numbness below the level of the navel, vibratory sensation was decreased in both lower limbs, and there was a hyperesthesic zone at the tenth thoracic vertebral level. She had a difficulty in urination and defecation. Laboratory examination re

[Clinical signs, neurophysiological evaluation, and medication of spasticity--review].

Spasticity is usually observed along with paralysis, hyperreflexia, Babinski sign and abnormal associated movements associated with dysfunction of central motor tracts. In spasticity, exaggeration of the stretch reflex results in increased resistance during passive movements. Therefore, spasticity is pathophysiologically described as increased muscle tone whose pathognomonic sign is decreased passivity. Resistance is more strongly felt during rapid passive movements than during slow movements. The resistance felt at the beginning of the passive movement abruptly diminishes, which is well known

Beyond Nerve Hyperexcitability: Potential Implications of Cerebrospinal Fluid CASPR2 Antibodies for Central Pain Processing in Isaacs Syndrome: A Case-Based Systematic Review.

INTRODUCTION: Isaacs syndrome (IS), or acquired neuromyotonia, is a rare immune-mediated neuromuscular disorder characterized by increased peripheral nerve excitability. Clinically, it manifests with fasciculations, cramps, excessive sweating, and the presence of CASPR2 autoantibodies in the serum. IS can occur as a paraneoplastic condition or may remain idiopathic. METHODS: We report an adult case of IS that prompted a case-based literature review investigating the association between cerebrospinal fluid (CSF) CASPR2 antibodies and IS. A systematic literature search of PubMed and Scopus was
