Emerging treatment paradigms of ocular surface disease: proceedings of the Ocular Surface Workshop.

The objective of the Ocular Surface Workshop in Rome, Italy, on 6 February 2009, was to enhance the understanding of ocular surface disease (OSD) through an exploration of the nature of its complexities and current treatment paradigms across Europe. It was hoped that the peer-to-peer discussions and updates regarding common knowledge, clinical practices and shared experiences at this workshop would subsequently shape future treatment approaches to OSD.

Refractive surgery in the United States.

In a mere 8 years, refractive corneal surgery in the United States has become a commonly practiced procedure. The major procedures being performed today are keratotomy for the correction of myopia and astigmatism and epikeratoplasty for the correction of aphakia. Current, on-going clinical studies involving intrastromal implantation of hydrogel materials have just begun. Clear lens extraction for myopia and intraocular lens implantation in high myopia at the time of cataract surgery are being used more commonly each day. Recognition of and modification of pre-existing corneal astigmatism at th

Deep anterior lamellar keratoplasty for the management of iatrogenic keratectasia occurring after hexagonal keratotomy.

Iatrogenic keratectasia has been reported subsequent to refractive surgery or trauma. Hexagonal keratotomy (HK) is a surgical incisional technique to correct hyperopia. A number of complications have been reported following this procedure, including irregular astigmatism, wound healing abnormalities and corneal ectasia. When visual acuity is poor because of ectasia or irregular astigmatism and contact lens fitting is not possible, penetrating or lamellar keratoplasty can be performed. Since incisions in refractive keratotomy are set at 90-95% depth of cornea, intraoperative microperforations a

Surgical management of presbyopia.

Presbyopia, the gradual loss of accommodation that becomes clinically significant during the fifth decade of life, is a physiologic inevitability. Different technologies are being pursued to achieve surgical correction of this disability; however, a number of limitations have prevented widespread acceptance of surgical presbyopia correction, such as optical and visual distortion, induced corneal ectasia, haze, anisometropy with monovision, regression of effect, decline in uncorrected distance vision, and the inherent risks with invasive techniques, limiting the development of an ideal solution

Advanced Optical Wavefront Technologies to Improve Patient Quality of Vision and Meet Clinical Requests.

Adaptive optics (AO) is employed for the continuous measurement and correction of ocular aberrations. Human eye refractive errors (lower-order aberrations such as myopia and astigmatism) are corrected with contact lenses and excimer laser surgery. Under twilight vision conditions, when the pupil of the human eye dilates to 5-7 mm in diameter, higher-order aberrations affect the visual acuity. The combined use of wavefront (WF) technology and AO systems allows the pre-operative evaluation of refractive surgical procedures to compensate for the higher-order optical aberrations of the human eye, …

Refractive Nightmares Revisited: Calcification of a Multifocal Intraocular Lens.

BACKGROUND/AIM: Cataract is the leading cause of reversible blindness and visual impairment worldwide. Although cataract surgery using phacoemulsification and intraocular lens (IOL) implantation is one of the commonest surgical procedures, IOL opacification remains a potential complication that can affect the visual outcome of the operation. CASE REPORT: A 50-year-old female patient presented to our clinic complaining of glare and blurry vision in her right eye over the previous 6 weeks. She had undergone bilateral refractive lens exchange elsewhere 9 months earlier. Her unaided distance visu

[Intracorneal ring segments in keratoconus management].

Intracorneal ring segments (ICRS), used in the management of keratoconus since the 2000s, have enjoyed significant technological development. Various types of ICRS exist, whose arc length, thickness, and diameter can be chosen according to the desired effect on the spherical equivalent, keratometry and asphericity. Individualized implantation strategies, based on each patient's topographic and tomographic pattern, are constantly evolving. The surgical procedure is standardized, and complications remain very rare. Combined procedures (corneal collagen cross-linking and refractive photokeratecto

Modification of corneal curvature following radial keratotomy in primates.

Radial keratotomy was performed on six stumptail monkey eyes (Macaca speciosa) and 22 owl monkey eyes (Aotus trivirgotus). Changes in keratometry, specular microscopy, and tonometry have been studied postoperatively for three to six months. Sixteen radial incisions in stumptail monkey eyes resulted in a significant mean corneal flattening of 2.75 diopters (P less than 0.005). This effect was stable with a mean 2.50 diopters flattening remaining six months after surgery. Sixteen radial incisions in ten owl monkey eyes resulted in a much greater short-term effect, with a mean 10.50 diopters of f

Update on astigmatism management.

PURPOSE OF REVIEW: Astigmatism is a common refractive error that affects a significant portion of the population. This is a review of the most salient topics on assessing astigmatism as well as a discussion of the latest developments in surgical options and newest technologies to improve outcomes. RECENT FINDINGS: Laser corneal ablation continues to be highly effective for correcting low-to-moderate levels of astigmatism and may be the best option for the younger patient population. For eyes with astigmatism and cataracts, both peripheral corneal-relaxing incisions and cataract extraction wit

Technique of intrastromal pocket creation in the Visumax Femtosecond Laser System - The Ganesh pocket.

Intrastromal pocket creation for purposes of lenticule-assisted corneal refractive surgery in the treatment of high hyperopia, keratoconus, and corneal inlay implantation has shown promising results. Several femtosecond laser machines have preprogrammed software that aids in the creation of such an intrastromal pocket. The Visumax Femtosecond Laser System (FSL) (Carl Zeiss Meditec, Germany), however, does not, although it is an excellent system for lenticule extraction procedures such as ReLEX Small Incision Lenticule Extraction. Here, we describe a novel surgical technique using the Visumax F

Current perspectives on infections following refractive kerato-lenticule extraction (KLex) surgery.

Refractive kerato-lenticule extraction surgery (KLex), also known as "small incision lenticule extraction (SMILE)," or "smooth incision keratomileusis" (SILK), represents a significant advancement in refractive surgery with its minimally invasive approach and impressive success rates. Despite these advancements, postoperative infectious keratitis, though rare, poses a critical challenge and profoundly impacts visual outcomes. Unlike infections following surface ablation procedures, which benefit from direct drug penetration into the site of infection, KLex and LASIK face hurdles due to the dep
