Tendoscopic peroneal retinaculum repair for recurrent peroneal tendon dislocation enables earlier return to sports than the open procedure.

PURPOSE: The purpose of this study was to evaluate whether tendoscopic peroneal retinaculum repair for patients with recurrent peroneal tendon dislocation (RPTD) is more useful than an open procedure. METHODS: Twenty-five patients with RPTD were retrospectively reviewed. Twelve patients (13 ankles) with RPTD underwent the open procedure (Group A) between 2008 and 2014, and 13 patients (14 ankles) underwent the tendoscopic procedure (Group B) between 2014 and 2017. Evaluation parameters included clinical results [the Japanese Society for Surgery of the Foot (JSSF) ankle-hind foot scale], opera

The cellular basis of fibrotic tendon healing: now in higher definition.

Tendon injuries are common and heal through the formation of a fibrotic scar rather than regeneration. This poor natural healing results in impaired function and increases the risk of reinjury. In 2019, we reviewed the current state of the field's understanding of the cellular basis of fibrotic tendon healing, focusing on the resident and infiltrating cell populations that appear at key stages and what was known about their role in tendon healing at the time. Since then, advances in transcriptomic technologies and widespread adoption of lineage tracing have refined our understanding of these p

Identifying Psychological Difficulties in College Athletes.

Psychological conditions occur frequently in college students. One contributing factor is the onset of most mental health disorders occurring in late adolescence and early adulthood, as well as the identity formation and individuation that is typical of this developmental stage. Precollege trauma (emotional, physical, sexual, and witness to violence) and lower socioeconomic status can set the stage for psychological difficulties. Some of many stressors that may affect college athletes include peer pressures, independence, need to please family, friends, and coaches, high level of expectations

Nonmodifiable risk factors for anterior cruciate ligament injury.

PURPOSE OF REVIEW: As anterior cruciate ligament (ACL) injury is becoming increasingly prevalent in the population of active children and young adolescents, it is crucial to be aware of both the modifiable and nonmodifiable factors that place this population at increased ACL injury risk. Historically, there has not been a definitive consensus on all of these risk factors-particularly the nonmodifiable ones. RECENT FINDINGS: The present review has accumulated the most recent evidence for the nonmodifiable risk factors in ACL injury focusing particularly on female gender, generalized joint laxi

Dynamic Changes in Local Protein Synthetic Machinery in Regenerating Central Nervous System Axons after Spinal Cord Injury.

Intra-axonal localization of mRNAs and protein synthesis machinery (PSM) endows neurons with the capacity to generate proteins locally, allowing precise spatiotemporal regulation of the axonal response to extracellular stimuli. A number of studies suggest that this local translation is a promising target to enhance the regenerative capacity of damaged axons. Using a model of central nervous system (CNS) axons regenerating into intraspinal peripheral nerve grafts (PNGs) we established that adult regenerating CNS axons contain several different mRNAs and protein synthetic machinery (PSM) compone

Reversal of Neuronal Atrophy: Role of Cellular Immunity in Neuroplasticity and Aging.

Emerging evidence indicates that neuroimmunological changes in the brain can modify intrinsic brain processes that are involved in regulating neuroplasticity. Increasing evidence suggests that in some forms of motor neuron injury, many neurons do not die, but reside in an atrophic state for an extended period of time. In mice, facial motor neurons in the brain undergo a protracted period of degeneration or atrophy following resection of their peripheral axons. Reinjuring the proximal nerve stump of the chronically resected facial nerve stimulates a robust reversal of motor neuron atrophy which

Regenerative Medicine for Equine Musculoskeletal Diseases.

Musculoskeletal injuries and chronic degenerative diseases commonly affect both athletic and sedentary horses and can entail the end of their athletic careers. The ensuing repair processes frequently do not yield fully functional regeneration of the injured tissues but biomechanically inferior scar or replacement tissue, causing high reinjury rates, degenerative disease progression and chronic morbidity. Regenerative medicine is an emerging, rapidly evolving branch of translational medicine that aims to replace or regenerate cells, tissues, or organs to restore or establish normal function. It

Sports Medicine: Fractures, Sprains, and Other Musculoskeletal Injuries.

Family physicians commonly treat athletes with simple fractures and other musculoskeletal injuries that require temporary immobilization. Certain fractures (eg, intraarticular, multiple, open, associated with dislocations or tendon injury) typically are managed by orthopedic subspecialists. During the healing process, a custom-molded or prefabricated orthopedic support, known as an orthosis, may be used to support the injured area and prevent reinjury. To reduce the risk of complications that may develop from orthosis use, timely follow-up is required. Proficiency in specific orthotic applicat

Approaching the ruptured anterior cruciate ligament.

Anterior cruciate ligament (ACL) disruptions are common injuries that currently hold a fearsome reputation among athletes of all abilities and disciplines. Indeed, if the diagnosis is missed at first presentation, it is difficult to attribute ongoing instability and recurrent injury to an ACL tear. Classically, patients then often improve shortly before repeatedly reinjuring their knee. At some point, the knee may lock, necessitating an arthroscopic meniscectomy. Tragically, this then hastens the progression of joint arthrosis and the decline of the joint function. While the burden of responsi

Delayed anatomical repair of ruptured ulnar collateral ligament injuries of the thumb using a dissolvable polylactic acid bone anchor.

BACKGROUND: We describe late anatomic repair of complete distal ulnar collateral ligament (UCL) avulsions of the thumb in the absence of metacarpophalangeal joint arthritis using a dissolvable polylactic acid (PLA) bone anchor in two cases, one of which had a Stener lesion. METHODS: Patients presented for surgery respectively at 5 months and 2 years after injury. Preoperative X-ray films showed subtle volar subluxation of the proximal phalanx. Volar plate release corrected this in both cases and a transarticular k-wire was not used. Satisfactory repair was achieved in both cases using a disso

ACL injuries in the skeletally immature patient.

Anterior cruciate ligament injuries in the pediatric and adolescent population are becoming more common with the trends of increased participation in competitive athletics in younger children. The natural history of this injury is similar to that of the adult and results in frequent reinjury, instability, and debilitation. Delayed reconstruction has proven to be a clinically effective treatment method but requires a prolonged restriction of competitive athletics. Reconstruction in the acute and subacute period has been shown by many authors to be a successful, reproducible treatment regimen. N

Anterior cruciate ligament injuries in the young athlete with open physes.

From 1980 to 1985, 40 patients under the age of 14 with open physes were treated for midsubstance tears of the ACL at the Methodist Sports Medicine Center. In this series, 16 were treated conservatively with rehabilitation, bracing, and counseling on activity modification. The remaining 24 patients underwent arthroscopic examination and either an extraarticular or intraarticular reconstruction based on growth potential. The average followup was 27 months for the conservative group and 26 months for the surgical group. In the conservative group, six patients underwent arthroscopy for meniscal t

A rehabilitation programme performed in darkness after anterior cruciate ligament reconstruction reduces the risk of secondary injuries.

PURPOSE: To evaluate whether integrating partial visual deprivation during the reathletization phase after anterior cruciate ligament (ACL) reconstruction improves proprioceptive recovery and reduces the risk of graft failure or contralateral injuries. METHODS: We conducted a retrospective, single-centre cohort study including patients aged older than 16 years who underwent ACL reconstruction with autograft and concomitant anterolateral ligament reconstruction between November 2017 and March 2020. Patients voluntarily chose between two reathletization programmes starting from the fourth posto

Generalized joint hypermobility does not influence 1-year patient satisfaction or functional outcome after ACL reconstruction.

PURPOSE: The purpose of this study was to evaluate whether generalized joint hypermobility (GJH) influences postoperative results, including return to sport, patientreported outcomes, functional performance (hop tests), muscular strength, and the occurrence of ACL re-injury, in patients 1 year after anterior cruciate ligament (ACL) reconstruction. METHODS: Data was extracted from a regional rehabilitation-specific registry containing information on patients with ACL injury. Patients between the ages of 16-50 years previously undergoing ACL reconstruction with available 1 year follow-up data w

Surgical Treatment of Periprosthetic Acetabular Fractures After Hip Arthroplasty: A Report of Two Cases.

Bipolar hemiarthroplasty (BHA) and total hip arthroplasty (THA) are standard treatments for hip disorders in the elderly. However, a rare postoperative complication known as periprosthetic acetabular fracture (PPAF) can occur, potentially affecting hip joint stability and implant longevity. The management of PPAF is guided by the Della Valle and Paprosky classifications, which assess implant stability and the extent of bone loss. In this study, we report two cases of PPAF occurring after BHA and THA. In Case 1, early open reduction and internal fixation were performed, successfully avoiding st

A comparison of public views about sports concussion recovery with current guidelines: where are the gaps and overlaps?

Background Sports concussion (SC) management guidelines have recently been updated. A key focus is the emphasis on rest (immediately postinjury) followed by gradual resumption of activity (active recovery). This study aimed to explore community views on SC management and compared these with the guidelines. Methods A total of 157 volunteers completed an online SC survey, including listing three pieces of advice for a concussed person immediately postinjury, and after 2weeks (subacute). Quantitative data were statistically compared, and qualitative data underwent content analysis. Results Almost

High quality rehabilitation to optimize return to sport following lateral meniscus surgery in football players.

There is a paucity of studies in the literature pertaining about how to guide rehabilitation and return to sport (RTS) progression for football players that have sustained a lateral meniscus injury and subsequent surgery. These patients are clinically challenging. If they return to sport too soon, before the rehabilitation process is completed and RTS criteria have been met, functional outcomes could be non-optimal and/or associated with higher reinjury risk. The aims of this paper are (I) to provide a brief overview of the current trends in acute lateral meniscus tears surgery in football pla
