A case of superior laryngeal nerve paresis and psychogenic dysphonia.

A psychogenic voice disorder co-occurred or evolved with the symptoms and signs of unilateral superior laryngeal nerve paresis. We speculated that the former was a manifestation of a musculoskeletal tension or conversion reaction disorder, whereas the latter was a sequela to a self-limiting inflammatory process. Voice therapy proved effective for alleviating the psychogenic dysphonia after the signs of the neuropathy had resolved, whereas psychotherapy offered strategies for stress management. A multidisciplinary approach to this patient provided for differential diagnosis and efficacious trea

Development of the "Kalmer" relaxation intervention: co-design with stroke survivors with aphasia.

PURPOSE: Anxiety is common after stroke and more prevalent in survivors with aphasia. Relaxation is an effective first-line therapy. The current study aimed to obtain the perspectives of stroke survivors with aphasia to inform the development of an accessible, technology-based, relaxation intervention. MATERIALS AND METHODS: Qualitative co-design methods were used with twelve people with aphasia after stroke. The "Kalmer" Relaxation treatment package materials were iteratively based on participants' experiences and preferences; barriers and facilitators to treatment compliance were explored.

Complementary Therapy Learning in the Setting of Lung Transplantation: A Single-Center Observational Study of Appropriation and Efficacy.

Transplanted patients could benefit from complementary techniques. This prospective single-center, open study, performed in a tertiary university hospital, evaluates the appropriation and efficacy of a toolbox-kit of complementary techniques. Self-hypnosis, sophrology, relaxation, holistic gymnastics, and transcutaneous electric nerve stimulation (TENS) were taught to adult patients scheduled for double-lung transplantation. Patients were asked to use them before and after transplantation, as needed. The primary outcome was appropriation of each technique within the first three postoperative m

Effect of rajyoga meditation on chronic tension headache.

Chronic tension-type headache (CTTH) is the most common type of headache with no truly effective treatment. This study was designed to correlate the additive effect of meditation on CTTH patients receiving medical treatment. 50 patients (aged 18-58 years) presenting with a clinical diagnosis of CCTH, were divided in 2 groups. Group 1 (n=30) received 8 lessons and practical demonstration of Brahmakumaris spiritual based meditation known as Rajyoga meditation for relaxation therapy, in addition to routine medical treatment (analgesics and muscle relaxants). Group 2 (n=20) patients received analg…

Adverse effects of depolarising neuromuscular blocking agents. Incidence, prevention and management.

Muscle relaxants block neuromuscular transmission, acting at nicotinic acetylcholine receptors of the neuromuscular junction. Suxamethonium (succinylcholine) is a depolarising agent, whereas all other relaxants in clinical use are nondepolarising. The desired neuromuscular block results from the structural similarity of muscle relaxants to acetylcholine, enabling the interaction with receptors at the neuromuscular junction. Adverse effects of suxamethonium are generally related to its agonist mode of action. Autonomic cardiovascular effects may result. Other adverse effects include anaphylacti

Alternative therapies for tension-type headache.

Treatment of tension-type headache remains very challenging. In addition to conventional therapies, alternative methods such as physical therapy, acupuncture, and botulinum toxin have been studied. In this article, recent literature is reviewed and discussed and challenges for the evaluation of these approaches are considered. Although the clinical evidence is still incomplete, certain treatments are promising and the active ongoing research hopefully will soon yield more answers. Of note, the specific issue of psychologic therapy is dealt with elsewhere in this issue.

Personalized Virtual Reality Compared With Guided Imagery for Enhancing the Impact of Progressive Muscle Relaxation Training: Pilot Randomized Controlled Trial.

BACKGROUND: Empirical evidence has shown that virtual reality (VR) scenarios can increase the effects of relaxation techniques, reducing anxiety by enabling people to experience emotional conditions in more vivid settings. OBJECTIVE: This pilot randomized controlled study aims to investigate whether the progressive muscle relaxation technique (PMRT) associated with a personalized scenario in VR promotes psychological well-being and facilitates the recall of relaxing images more than the standard complementary intervention that involves the integration of PMRT and guided imagery (GI). [Interdisciplinary multimodal pain therapy in headache disorders].

Primary headaches are among the most common pain disorders. They include migraines (prevalence 15%), tension headaches (up to 80%), and others, including trigeminal autonomic headaches (about 0.2%). Migraine, in particular, leads to significant impairment of personal life and high societal costs. Therefore, the need for effective and sustainable therapeutic procedures is high. This article provides an overview of psychological procedures in headache therapy and critically summarizes the empirical evidence for the effectiveness of interdisciplinary multimodal pain therapy (IMST) consisting of p

The effectiveness of relaxation training in reducing treatment-related symptoms and improving emotional adjustment in acute non-surgical cancer treatment: a meta-analytical review.

Cancer patients often have to deal with severe side effects and psychological distress during cancer treatment, which have a substantial impact on their quality of life. Among psychosocial interventions for reducing treatment-related side effects, relaxation and imagery were most investigated in controlled trials. In this study, meta-analytic methods were used to synthesize published, randomized intervention-control studies aiming to improve patients' treatment-related symptoms and emotional adjustment by relaxation training. Mean weighted effect sizes were calculated for 12 categories, treatm

Pain control after surgery: a patient's guide, Agency for Health Care Policy and Research.

What is pain? Pain is an uncomfortable feeling that tells you something may be wrong in your body. Pain is your body's way of sending a warning to your brain. Your spinal cord and nerves provide the pathway for messages to travel to and from your brain and the other parts of your body. Receptor nerve cells in and beneath your skin sense heat, cold, light, touch, pressure, and pain. You have thousands of these receptor cells, most sense pain and the fewest sense cold. When there is an injury to your body--in this case surgery--these tiny cells send messages along nerves into your spinal cord an

Effectiveness of the Transcendental Meditation program in preventing and treating substance misuse: a review.

This article reviews 24 studies on the benefits of Transcendental Meditation (TM) in treating and preventing misuse of chemical substances. Studies cover noninstitutionalized users, participants in treatment programs, and prisoners with histories of heavy use. All the studies showed positive effects of the TM program. Some of the survey-type studies were unable to exclude the possibility of self-selection or responder biases. However, longitudinal, random-assignment studies with objective measures also showed positive results. Taken together, these and other studies indicate the program simult

Integrating music in breathing training and relaxation: II. Applications.

Psychophysiological observations, especially PETCO2 and EEG, during relaxation training with deep-diaphragmatic breathing and mental imagery, suggest that the addition of certain types of music "deepens" breathing and quickens relaxation: PETCO2 "normalizes" with decreased respiration rate, and EEG shows decreased average theta and increased alpha. The combined psychophysiological indices suggest that music potentiates the hypometabolic counterarousal state. And clients unanimously report that they enjoy it.

A four-step hypnotherapy model for Gilles de la Tourette's syndrome.

Hypnotherapy may sometimes be helpful in ameliorating the behavioral characteristics of Tourette's Syndrome. This paper presents a case study of an adolescent male with Tourette's Syndrome who was referred by his physician for hypnotherapy for his disorder. A four-step treatment model involved progressive relaxation, finger-tip temperature feedback using a biotic finger band, Spiegel's eye-roll procedure, and imagery. The adolescent was in treatment for nine sessions, from October 1985 to March 1986, a period of 6 months. At the end of the nine sessions, his tics and vocalizations were minimal

Panic attacks during relaxation and relaxation-induced anxiety: a hyperventilation interpretation.

This paper explains how a hyperventilation theory of panic disorder accounts for panic attacks during relaxation and relaxation-induced anxiety. The explanation is based on the observation that chronic hyperventilators maintain a steady state of low pCO2 (arterial carbon dioxide tension) and are, therefore, sensitive to relatively small increases in ventilation when metabolism is low and to relatively sudden reductions in metabolism when ventilation is relatively constant. Thus, if minute volume of air breathed remains constant while the metabolic production of CO2 decreases, as in the case of

Progressive relaxation training in cardiac rehabilitation: effect on psychologic variables.

The purpose of this study was to investigate the effect of progressive relaxation training as a stress management technique for cardiac patients who were participants in a cardiac exercise program. After pretesting, 18 patients received 3 weeks of relaxation training in addition to their exercise therapy; a control group of 19 patients was not taught the technique. Pretesting used two instruments to measure stress levels-the Spielberger State-Anxiety Scale and selected dimensions of the Symptom Checklist-90-Revised. At the completion of the relaxation training program, both groups of patients

Delayed recovery in the patient with a work compensable injury.

Under certain circumstances patients who are receiving compensation for an injury will have a disproportionate disability and delayed recovery because of reinforcers provided by the accident. Successful treatment depends on early recognition that the primary problem is emotional rather than due to concurrent physical problems. A few general rules can be helpful in the diagnosis and treatment of this type of patient. A thorough psychosocial history should be taken, and return to work as soon as possible is essential. With awareness of this process, the physician can work with the company, the t

A comparison of two relaxation procedures for reducing cognitive and somatic anxiety.

The effectiveness of two different relaxation procedures was compared in treating snake anxious people who expressed anxiety primarily in a somatic or a cognitive manner. Forty moderately snake phobic subjects were categorized as either "somatically-anxious" or "cognitively-anxious" on the basis of a questionnaire. Half of the subjects in each group were given modified progressive relaxation training, and the other half received training in Agni-Yoga for four sessions. After completion of training, approach distance, subjective fear and pulse rates were measured during a snake approach test. I
