Retroperitoneal fibrosis (RPF) is a rare fibroinflammatory disease with idiopathic and secondary causes. Idiopathic disease is more common and is believed to be immune mediated; associations with autoimmune diseases and/or inflammatory disorders such as IgG4 related disease are often present. Common complications include hydronephrosis and venous stenosis and/or thrombosis. Due to its nonspecific clinical presentation, imaging is vital for diagnosis; in addition, imaging may help distinguish idiopathic from secondary causes and can aid in distinguishing early/active disease from chronic/inacti…

We describe a 45-year-old male patient with recurrent retroperitoneal fibrosis (RPF), in whom a pseudocyst in the peri-aortic fibrotic mantle was diagnosed. Without any intervention other than oral treatment with tamoxifen, the pseudocyst showed significant regression. Although rare, pseudocyst formation may sometimes appear in RPF and may mimic other benign and malignant conditions. This is the first paper to describe pseudocyst formation in an RPF relapse.

Idiopathic retroperitoneal fibrosis is a rare disease often causing obstructive uropathy because the fibrosis entraps the ureters. The retroperitoneal tissue is constituted by a fibrous component and a chronic inflammatory infiltrate with the former characterized by miofibroblasts. The infiltrate displayed perivascular. Last years, the immunosuppressive therapy and the decompression of obstructive renal failure have improved the prognostic. We reported 9 patients with Idiopathic retroperitoneal fibrosis in a Centre. We evaluate clinical symptoms at the presentation with the signs of renal and …

Although cases of lymphoplasmacytic sclerosing pancreatitis (LSP) associated with idiopathic retroperitoneal fibrosis have been reported, the association is rare. We describe a 74-year-old man who presented with obstructive jaundice and weight loss. Nineteen months earlier, he had been diagnosed with idiopathic retroperitoneal fibrosis and treated with bilateral ureteric stents. Initial investigations were suggestive of a diagnosis of LSP, however, a malignant cause could not be ruled out. He underwent an exploratory laparotomy and frozen sections confirmed the diagnosis of LSP. An internal bi…

The retroperitoneal fibrosis is a very little frequent pathology that habitually includes the abdominal aorta and ureters. Two thirds of cases the etiology is idiopathic and in a third it is secondary to other causes as drugs, generalized infections or diseases and tumors which are considered to be responsible from 8 to 10%. The most frequent is colorectal adenocarcinoma, being anecdotal the gastric location with six described cases to date. We present a case of a patient with gastric cancer and secondary retroperitoneal fibrosis of difficult diagnosis and handling.

Retroperitoneal fibrosis (RPF) is an uncommon collagen vascular disease characterized by a chronic nonspecific inflammation of the retroperitoneum, which can entrap and obstruct retroperitoneal structures. Although obscure, an autoimmune-mediated or vasculitic etiology has been hypothesized. In the current report, a case of RPF associated with the presence of antibodies against proteinase III, which responded effectively to immunosuppressive therapy, is presented.

Three patients with idiopathic retroperitoneal fibrosis underwent laparoscopic ureterolysis. Two patients were bilateral cases which were performed successfully as a one stage procedure. Another patient, who was unsuccessfully treated, had a long ureteral stricture. Laparoscopic ureterolysis may be a useful alternative to open surgical management especially in bilateral cases, except for patients with a long ureteral stricture.

CONTEXT: Autoimmune pancreatitis is sometimes associated with other autoimmune diseases. We have presented two cases of autoimmune pancreatitis with retroperitoneal fibrosis and compared our findings with those found in the literature. CASE 1: A 71-year-old male developed anorexia and weight loss. Abdominal ultrasonography (US) and computed tomography (CT) showed diffuse swelling of the pancreas and the peritoneal soft tissue surrounding the aorta, associated with right hydronephrosis. Endoscopic retrograde pancreatography showed narrowing of the main pancreatic duct. He was diagnosed as havin…

In case 1, a 24-year-old man was admitted with left lumbago. Abdominal computed tomographic (CT) scan showed the huge mass (8 x 10 x 13 cm i … both common iliac vessels. A retroperitoneoscopic biopsy was performed. We diagnosed both cases as retroperitoneal fibrosis and administered prednisolone for 10 months. The masses decreased in size and the symptoms disappeared in both cases.

The authors report six cases of retroperitoneal fibrosis. The mean age is 32 years. Two patients were treated by corticosteroid and ureteric stents with a good outcome; they have normal kidney function. Four patients underwent ureterolysis by wrapping the ureters in omentum. The mean follow-up is 60 months. In the light of theses six cases and review of the literature, the clinical, diagnosis and therapeutic aspects are discussed.

A 43-year-old man, with a history of central diabetes insipidus diagnosed 3 years previously, complained about reduced libido. An MRI scan s … severe fibrosis around both ureters. Histological examination confirmed the diagnosis of idiopathic retroperitoneal fibrosis. Presumably the suprasellar tumour was the first manifestation of retroperitoneal fibrosis. Once the diagnosis 'idiopathic retroperitoneal fibrosis' is confirmed, fibrotic manifestations and complications involving extra-retroperitoneal tissues incl …

A middle aged man showed retroperitoneal fibrosis with signs of vasculitis verified by biopsy of the retroperitoneal mass. Antineutrophil cytoplasmic antibodies (ANCA) with cytoplasmic staining pattern (cANCA) were strongly positive, although there were no clinical or histological signs of Wegener's granulomatosis. No cases of cANCA associated retroperitoneal fibrosis have been described except in connection with Wegener's granulomatosis. ANCA have been found to associate with any inflammatory disorders. This observation may broaden the clinical spectrum of cANCA positive vasculitides.

A 49-year-old male patient underwent an exploratory laparoscopy for a lumbosacral prevertebral mass which caused right hydronephrosis by extrinsic ureteral stricture. The diagnosis of idiopathic retroperitoneal fibrosis (IRPF) was made by the laparoscopic biopsy. We then proceeded to perform successfully laparoscopic ureterolysis and intraperitonealization of the ureter on the patient. Laparoscopic approach provides a less invasive alternative in diagnosis and treatment of the patient with IRPF.

Ureterolysis and subsequent intraperitonealization were performed laparoscopically in 2 patients with unilateral idiopathic retroperitoneal fibrosis. The operative procedure was easily accomplished, and the outcome was excellent in the patient who had a ureteral stricture of 2 cm in length, whereas the operation was technically more complicated with a prolonged operation time in the other patient who had a stricture more than 5 cm in length. The convalescence period after the operation was short in both cases. Laparoscopic ureterolysis is a less invasive alternative to conventional open surger…
