Excellent functional outcome following selective dorsal rhizotomy in a child with spasticity secondary to transverse myelitis.

PURPOSE: Selective dorsal rhizotomy (SDR) is a neurosurgical procedure used to treat spasticity in children with cerebral palsy (CP). The vast majority of studies to date suggest SDR is particularly effective in reducing lower limb spasticity in spastic diplegia with long-lasting effect. METHOD: We report, to the best of our knowledge for the first time, the case of a teenager who underwent SDR for the management of spasticity secondary to transverse myelitis. RESULTS: This is an unusual indication for SDR which resulted in completely loose lower limbs and an excellent functional outcome. At…

Changes in gait at 1 year post-selective dorsal rhizotomy: results of a prospective randomized study.

We performed a prospective randomized trial to compare the effects of selective dorsal rhizotomy with intensive physical therapy to intensive physical therapy alone in a group of children with mild spastic diplegic cerebral palsy. Instrumented gait analysis was carried out upon enrollment into the study and after 1 year. Changes in ambulatory status, time/distance parameters, and gait kinematics were observed for both groups. Considerable variability was present in both groups. Changes in ankle dorsiflexion, foot progression angle, and hip and knee extension in stance were significantly better…

Selective sacral rhizotomy for the management of neurogenic bladders in spina bifida patients: long-term followup.

Selective sacral rhizotomy was introduced for the management of high pressure neurogenic bladders commonly encountered in myelodysplastic patients. In 1992, 2 of us (I.F. and W.K.) first reported results with selective sacral rhizotomy and cord untethering in 8 spina bifida patients. We report long-term followup of our original 8 patients and 3 additional patients. This followup demonstrates remarkable success in maintaining bladder volume and low pressures after rhizotomy and cord untethering. Uninhibited contractions resolved in all patients postoperatively. A more favorable response occurre…

Interconnections of the upper ventral rami of the human sacral plexus: a reappraisal for dorsal rhizotomy in neurostimulation operations.

The extension of a dorsal rhizotomy in bladder stimulation patients is partly determined by connections between the ventral rami of the second, third, and fourth sacral spinal nerves. The literature is inconclusive on interconnections of these ventral rami in the human sacral plexus. The sacral plexuses of ten human cadavers were dissected in this gross anatomy study. In nine cases a branch connecting the ventral rami of the second and third sacral spinal nerves was found. Electron microscopy demonstrated the presence of thick myelinated fibers in this branch. In the male plexuses this branch …

Microvascular decompression and partial sensory rhizotomy in the treatment of trigeminal neuralgia: personal experience with 220 patients.

The results of the treatment of trigeminal neuralgia by neurovascular decompression or partial sensory rhizotomy in a personal series of 220 patients are presented. Microvascular decompression was performed in 178 patients and partial sensory rhizotomy in 42. The mean follow-up was 5.2 years. Immediate pain relief was achieved in 94% of all patients, but the rate dropped to 84% during the follow-up period. The recurrence rate in the microvascular decompression group was 6% and in the PSR 49%. Permanent sequelae occurred in 4 patients (loss of hearing, 1; loss of corneal reflex, 1; lesion of th…

Dorsal selective rhizotomy through a limited exposure of the cauda equina at L-1. Technical note.

The technique of the dorsal selective rhizotomy as originally developed by Professor Fasano and by the author is described. The rhizotomy is performed through a one-level laminectomy at L-1. Exposure of the conus medullaris and the cauda equina at this level is adequate to thoroughly assess the reflex electrical response to stimulation of the lumbosacral roots and to section the selected roots/rootlets. This approach, based on over 100 surgical procedures, obviates the need for an extensive laminectomy in a growing child without compromising the clinical results.

Selective dorsal rhizotomy long-term effects on functional motility in Jordanian children with spastic cerebral palsy.

INTRODUCTION: Spasticity management in children with cerebral palsy (CP) is a challenge for healthcare providers worldwide. In the US and Europe, treatment options include non-surgical and surgical (i.e., selective dorsal rhizotomy, SDR) procedures, with beneficial effects on functional motility. SDR was introduced in Jordan in 2019. We performed the first assessment of the long-term effects on motor function in Jordanian children with spastic CP (SCP) who underwent SDR. METHODS: A retrospective cohort study of 43 patients (28 boys, 15 girls, mean ± SD age at surgery, 6.2 ± 2.5 years, 67.4% w…

Postoperative outcomes and stimulation responses for sectioned nerve roots during selective dorsal rhizotomy in cerebral palsy.

BACKGROUND: Cerebral palsy (CP) is the most cause of motor dysfunction in children. Selective dorsal rhizotomy (SDR) plays a major role in long term spasticity control. However, limited data exists on the effect of SDR on postoperative spasticity treatment requirements and supraspinal effects, and the stimulation responses of dorsal nerve roots in those with CP. METHODS: The current study included the outcome for 35 individuals undergoing SDR for motor functional outcome, spasticity, baclofen dose changes, botulinum toxin injection frequency, and spasticity related orthopedic procedures. We a…

Surgical Outcomes of Single-Level Bilateral Selective Dorsal Rhizotomy for Spastic Diplegia in 150 Consecutive Patients.

OBJECTIVES: Selective dorsal rhizotomy (SDR) is used to improve spasticity, gait, and pain in children with spastic diplegia. There is growing evidence supporting its long-term benefits in terms of functional outcomes, independence, and quality of life. There is, however, little contemporary work describing the surgical morbidity of this irreversible procedure. The purpose of this study is to evaluate the surgical outcomes and complications of SDR at a single United Kingdom center. METHODS: Demographics, surgical, postoperative, and follow-up data for all patients undergoing SDR between 2011 …

Technical communication: percutaneous radiofrequency mandibular nerve rhizotomy guided by high-speed real-time computed tomography fluoroscopy.

We present a new method of percutaneous radiofrequency mandibular nerve rhizotomy for pain relief in the mandibular region, in which needle placement is guided by high-speed real-time computed tomography (CT) fluoroscopy. Eleven patients (13 procedures) with idiopathic trigeminal neuralgia underwent the procedure. CT fluoroscopy simultaneously provided 3 slices (1-mm interval series, craniocaudally) in 1 fluoroscopic view, allowing for accurate needle placement. Trigeminal neuralgia improved in all patients without severe complications. The mean numerical rating scales of pain intensity (+ or …

Impaired micturition reflex caused by acute selective dorsal or ventral root(s) rhizotomy in anesthetized rats.

PURPOSE: To clarify the contributions of parasympathetic inputs and outputs to the micturition reflex. MATERIALS AND METHODS: Intra-vesical pressure (IVP), external urethral sphincter electromyogram (EMG), pelvic afferent nerve activities (PANA), and pelvic efferent nerve activities (PENA) as well as the time-derived IVP (dIVP, an index of bladder contractility) were evaluated in intact and acute dorsal or ventral root(s) rhizotomized (DRX and VRX, respectively) rats. RESULTS: In DRX rats, when compared with that in intact stage, the voiding frequency was decreased (75 +/- 15% of intact, P <
