Management of recurrent fracture of central incisor with internal resorption using light transmitting (luminex) post.

The normal root canal anatomy may be altered in various pathological processes and making it very difficult and at times impossible to achieve ideal obturation by normal methods. Internal resorption is one among them. There are several treatment protocols advised for this pathological condition. A crown-root fracture is defined as a fracture involving enamel, dentin and cementum and accounts for 5% of all traumatic injuries to the permanent dentition. In anterior teeth, these fractures are usually caused by direct trauma and often complicated in fully erupted teeth. In cases where the fracture

Conservative Management of External Replacement Resorption in a Posterior Tooth: A Case Report.

External replacement resorption (ERR) is an uncommon yet aggressive form of root resorption in which bone replaces the root following damage to the periodontal ligament. Early recognition is critical, as progression is typically silent and may ultimately result in tooth loss. This report describes a 25-year-old male patient who presented with a "sunken" lower right first molar teeth three years after biting a hard object. Clinical examination revealed infra-occlusion, a high-pitched metallic percussion tone, and delayed pulp responses. Periapical radiography and cone beam computed tomography d

Failure of Guided Endodontic Access in a Severely Obliterated Root Canal: A Case Report of Drill Deviation.

Pulp canal obliteration (PCO) increases the risk of procedural errors during canal localization. Guided endodontics improves access accuracy, but its success depends on precise guide design, proper drill-sleeve alignment, and material stability. When these factors are compromised, deviations and unsuccessful canal location may occur. The present clinical case reports the management of a severely obliterated root canal in a maxillary lateral incisor in which static-guided endodontic failed to achieve canal localization, emphasizing the technical limitations and the successful conventional appro

Comparative Evaluation of Wettability and Contact Angle Between Endodontic Sealers and Root Canal Lumen After Using Different Irrigants: A Goniometric Study.

Introduction The success of root canal treatment depends on effective chemomechanical preparation and the ability of the obturation materials to establish a durable seal with the root canal dentin. The physicochemical interaction between root canal irrigants and endodontic sealers influences sealer wettability, which plays an important role in dentinal adaptation and sealing ability. Contact angle measurement is a reliable method for evaluating wettability, with lower contact angle values indicating greater spreading ability and surface affinity. The present study evaluated the influence of tw

A Multidisciplinary Approach in the Management of a Grade 3 Endo-Periodontal Lesion.

Endodontic-periodontal lesions involve a pathological connection between the pulpal and periodontal tissues, often complicating both diagnosis and treatment. Accurate identification and classification are essential to determine an appropriate therapeutic approach. This case report describes the multidisciplinary management of a grade 3 endo-periodontal lesion without root damage. A 43-year-old female presented with pain and swelling in the left mandibular first molar. Clinical and radiographic findings indicated a failed root canal treatment, deep periodontal pockets, and furcation involvement

Microleakage in endodontics.

A three-dimensional obturation and complete coronal and apical seal is one the important aims of root canal treatment. Since microorganisms may remain in the root canal system after instrumentation, a tight apical seal is desired to prevent bacteria and their by-products from invading the apex. A perfect apical seal is also desired to prevent apical percolation. One of the major objectives of tooth restoration is the protection of exposed dentine against bacteria and their toxins. The interface between the restoration and dental hard tissue is an area of clinical concern as insufficient sealin

The complete endodontic obturation of an avulsed immature permanent incisor with mineral trioxide aggregate: a case report.

The use of mineral trioxide aggregate (MTA) for complete endodontic obturation of an immature maxillary central incisor is reported. The injured tooth of a 9-year-old male was avulsed secondary to a sports-related traumatic event. Early attempts at pulpal revascularization of the replanted tooth proved unsuccessful, as evidenced by radiographic signs of external inflammatory root resorption in the middle to cervical thirds of the root. Calcium hydroxide apexification over a 1-year period promoted cessation of the resorptive process, but was unable to adequately form an apical root canal barrie

Management of Discolored Endodontically Treated Maxillary Central Incisors With Nonsurgical Retreatment and Internal Bleaching: A Case Report.

Tooth discoloration in endodontically treated anterior teeth represents a clinically significant problem, particularly in cases with high esthetic expectations. When pain, percussion sensitivity, or radiographic findings accompany such teeth, the existing endodontic status should be reassessed before esthetic treatment. This case report presents the clinical management of a 28-year-old patient who presented with pain, percussion sensitivity, and marked coronal discoloration in previously endodontically treated maxillary central incisors, teeth #11 and #21. After removal of the existing root ca

Significance of Early Detection and Management of Trauma-Induced Inflammatory Root Resorption: A Case Report.

This case report highlights the importance of early detection and timely management of trauma-induced inflammatory root resorption (external inflammatory root resorption, EIR) following dental injuries. A nine-year-old boy presented with an extrusive luxation injury of teeth #11 and #21, accompanied by an uncomplicated crown fracture. Our proposed treatment plan was to opt for root canal treatment for both central incisors to prevent EIR; however, the patient's guardian opted for root canal treatment only for tooth #21 and to keep tooth #11 under observation, as it was vital at the time. Unfor
