Shoulder dislocations in patients older than 40 years of age.

Shoulder dislocations in patients over 40 years of age are common and are frequently associated with serious injuries to adjacent structures. Associated injuries such as brachial plexus injury, rotator cuff tear, axillary artery injury, fractures about the shoulder, and recurrent dislocation can make shoulder dislocations challenging problems for the clinician. Early diagnosis and treatment of associated injuries and treatment complications are the mainstays in prevention of morbidity and, even, mortality.

Overuse syndromes in adult athletes.

Approximately 50% of all sports participants will be injured at some point, and at least half of these injuries will be attributed to overuse. This article provides an in-depth review of the presentation, diagnosis, and treatment of the most common overuse injuries encountered in the adult population: epicondylitis, shoulder impingement/rotator cuff tears, patello-femoral dysfunction, and Achilles tendinitis. Stress fractures, the "ultimate" overuse injuries, are also discussed.

Evaluation of cartilage degeneration in a rat model of rotator cuff tear arthropathy.

BACKGROUND AND HYPOTHESIS: Rotator cuff tears are the most common injury seen by shoulder surgeons. Glenohumeral osteoarthritis develops in many late-stage rotator cuff tear patients as a result of torn cuff tendons, termed "cuff tear arthropathy." However, the mechanisms of cuff tear arthropathy have not been fully established. It has been hypothesized that a combination of synovial and mechanical factors contribute equally to the development of cuff tear arthropathy. The goal of this study was to assess the utility of this model in investigating cuff tear arthropathy. MATERIALS AND METHODS:

Editorial Commentary: Magnetic Resonance Imaging Reveals Rotator Cuff Tear Size, Retraction, Length, and Geometry; Muscle Volume and Degeneration; and Tendon Quality.

Magnetic resonance imaging (MRI) of the shoulder is commonly used for evaluating muscle bulk and fatty degeneration, as well as tendon tear size, geometry, retraction, and length. However, MRI can also be used to evaluate tendon quality. Increased rotator cuff tendon signal on T2-weighted fat-suppressed MRI appears to be a marker of tendon degeneration and potentially of impaired healing potential. Tendon signal intensity merits closer attention and may be especially relevant when selecting chronic degenerative tears for repair in patients with other risk factors for nonhealing.

Current concepts on imaging diagnosis of rotator cuff disease.

Rotator cuff disease is common and may be clinically silent or a cause of shoulder pain. Evaluation of the rotator cuff before and after surgery is challenging for the orthopedic surgeon, but the radiologist can make an accurate diagnosis and facilitate appropriate management. This article reviews the current concepts on imaging diagnosis of rotator cuff disease, beginning with a discussion of the complex anatomy of the rotator cuff, continuing into the normal and pathologic MR imaging appearances of the rotator cuff including tendinosis and tearing, and concluding with a review of the postope

Arthroscopic transosseous (anchorless) rotator cuff repair.

UNLABELLED: The traditional open transosseous rotator cuff repair gives excellent results for the fixation of tendon to bone and has represented the gold standard for rotator cuff surgery with excellent long-term results. In the last few years, different arthroscopic techniques using suture anchors have been developed to increase the tendon-bone contact area in an attempt to reconstitute a more anatomic configuration of the rotator cuff footprint while providing a better environment for tendon healing. However, the anchor-based techniques have still not replicated the traditional open transoss

[The shoulder cuff tears].

The cuff tears are frequent. The cuff has a fundamental role for the shoulder stabilisation. Shoulder impingement causes pathologies from simple tendinosis to rupture with excentric arthritis. Classicaly, we describe three stades. It is fundamental to have a good knowledge of the stades and to evaluate them with clinical examination and provocating tests. The radiologic examen is very important to confirm the pathology and to evaluate the lesions. Each stade has a surgical treatment if the conservative treatment gives bad results. Choice of the surgical procedure depends of the patient age and

Incidence and risk factors of pseudoparesis in massive rotator cuff tears: a systematic review and meta-analysis.

BACKGROUND: Among patients with massive rotator cuff tears (MRCT), some present with minimal pain but an inability to actively elevate the shoulder beyond 90°, a condition that frequently impairs quality of life. Comprehensive analyses of the risk factors for pseudoparesis in MRCT are lacking, and the role of certain factors remains controversial. This systematic review and meta-analysis aimed to quantify the incidence of pseudoparesis in MRCT and to identify its associated risk factors. METHODS: This systematic review was conducted in strict accordance with the PRISMA guidelines. A comprehen

Impact of smoking on structural failure after arthroscopic rotator cuff repair: a systematic review and meta-analysis.

BACKGROUND: Rotator cuff tears cause significant shoulder pain and functional limitation. Arthroscopic rotator cuff repair improves symptoms, yet structural failure rates remain substantial. Smoking may impair tendon-to-bone healing, but clinical studies report mixed findings due to heterogeneous methodology. Therefore, a systematic synthesis of imaging-confirmed outcomes is needed to clarify the association between smoking and structural failure after arthroscopic rotator cuff repair. METHODS: This review followed PRISMA 2020 and was registered in PROSPERO (CRD420251246197). PubMed, Embase,

Beyond Tissue Origin: Rethinking Experimental Model Selection in Rotator Cuff Research.

Rotator cuff tears are among the most common tendon disorders and a major cause of shoulder pain and functional impairment. Increasing evidence indicates that rotator cuff pathology is a complex biological disease involving inflammation, oxidative stress, hypoxia, apoptosis, cellular senescence, and dysregulated extracellular matrix (ECM) remodeling. Consequently, in vitro studies using rotator cuff-derived cells have become important tools for investigating disease mechanisms and therapeutic targets. Although rotator cuff-derived cells are widely used and generally regarded as physiologically

When Does a Rotator Cuff Tear Hurt? A Critical Review of the Literature.

With increasing age, rotator cuff tears are common in symptomatic and asymptomatic patients. Yet, there remains a limited understanding of the etiology of pain associated with rotator cuff pathology. Recent research has associated patient perception, resilience, repair integrity, tear size, biomechanics of the humeral head, and muscle kinematics with pain levels. However, the evidence remains largely inconclusive in answering the fundamental question regarding why some patients experience symptoms severe enough to warrant surgery and others remain completely asymptomatic. A deeper understandin

Failed and Revision Rotator Cuff Repair.

With an increasing incidence of rotator cuff repairs (RCRs), and by extension, RCR failures, surgeons must be facile in the diagnosis and management of this complication. A detailed history and physical exam as well as familiarity with the patient-specific, anatomic, and technical variables that increase a patient's risk of RCR failure is critical. Modifiable factors should be addressed prior to revision RCR and non-modifiable factors should be examined as they may disqualify an individual from a revision attempt. A methodical surgical approach is critical. In general, outcomes following revis

Investigating the multifactorial etiology of supraspinatus tendon tears.

The purpose of this study was to develop a multivariable model to determine the extent to which a combination of etiological factors is associated with supraspinatus tendon tears. Fifty-four asymptomatic individuals (55 ± 4 years) underwent testing of their dominant shoulder. Diagnostic ultrasound was used to assess for a supraspinatus tendon tear. The etiological factors investigated included demographics (age and sex), tendon impingement during shoulder motion (via biplane videoradiography), glenohumeral morphology (via computed tomography imaging), family history of a tear (via self-report)

[Research progress of graft application in promoting rotator cuff tendon-bone healing].

The incidence of re-tearing after rotator cuff repair is very high. The main reason is that the tendon-osseous junction after the operation is scar healed. In response to this problem, research in recent years has focused on the application of grafts, including cell transplantation, periosteum transplantation, cartilage transplantation, and biosynthetic transplantation. Cell transplantation is mainly a variety of stem cells from different sources. The current research has confirmed that it can achieve better results. The combined application of exosomes and stem cells may be the future develop

An unreported infraspinatus muscle variation-two-headed infraspinatus minor muscle and three-headed fusion with the teres minor muscle.

The infraspinatus muscle is situated under the scapular spine in the infraspinous fossa and inserts into the greater tuberosity of the humerus. It is a component of a crucial shoulder muscle group, the rotator cuff. There are a few interesting additional muscles in the infraspinal region. In the literature they are called the infraspinatus superficialis, infraspinatus minor and infraspinatus accessory muscles. The infraspinatus minor muscle is described as a superficial muscle bundle running under the scapular spine. During routine anatomical dissection, an unreported variation of the infraspi
